[Predictors of behavioural disorders and consultation-liaison psychiatry in patients with HIV disorders].

OBJECTIVE: The aim was to identify the features of patients admitted into an Infectious Disease Unit in a Hospital due to pathology related to HIV and/or addiction that originate the demand for consultation liaison psychiatry. METHOD: During the first six months of 1997, 232 admissions into the Infectious Disease Unit at Ramón y Cajal Hospital (191 patients) were systematically evaluated by the nursery staff. A specific questionnaire was designed for this interview. After having performed univariants analysis, a logistic regression was used to identify the most relevant variables in the claim for consultation liaison. RESULTS: The claim for consultation liaison was associated to: consume in the unit OR (yes/no)= 7.9, confusional syndrome OR (yes/no)= 5.6, social worker consultation liaison OR (yes/no)= 2.1. use of benzodiazepines OR (yes/no)= 2.4. No medical treatment respect to bad accomplishment OR= 3.6. Only taking into account the known features after the first examination: use of benzodiazepines OR (yes/no)= 2.1, use of cocaine OR (yes/no)= 1.8, recognized income (yes/no)= 2.2, no medical treatment due to bad accomplishment OR= 2.1. CONCLUSIONS: In our environment the demand for consultation liaison psychiatry is related to behavioural problems due to the use of substances and marginality. The variables which predict the demand and may be identified at the time of admission are: the use of benzodiazepines, cocaine, the lack of a recognized income and the absence of medical treatment for the HIV infection. Recognizing these features will allow us to identify patients who are going to have behavioural problems and demand psychiatric assistance.

[Is closed mitral commissurotomy still indicated? Patient selection and postoperative problems. Analysis of 221 cases].

Indications, findings, complications, and immediate results in 221 patients in whom a closed mitral commissurotomy was practiced are presented. Emphasis is placed on the rigidity of the clinical criteria to select these patients with the object of diminishing to a minimum the accidents or problems and unexpected findings. The results are considered satisfactory and comparable to those reported in other great centers. Finally we considered that the results obtained with this criteria for selection were very satisfactory, with reduction of the immediate mortality to 0% in comparison with others reported, and maintaining an index of complications which we consider minimum in its clinical repercussion to the patient and less than in other series.

[Conservative management of infected and/or exterior pacemakers. Analysis of 35 cases].

Thirty-five cases of infected pacemakers were revised comparing the results obtained through the conservative attitude in comparison to the traditional attitude. It was pointed out a way to better even further the results obtained through the conservative attitude. Some surgical and medical problems were analized and the way to came through them with respect to this complication, the literature that is in favor of this conduct was also analyzed.

[Crime-related aspects in heroin addict patients: relationship between crime and delinquency].

BACKGROUND: Criminological studies have been specially influenced by those social factors relationated with drug dependence. Data from Criminology and Law insist on relationship between social factor and criminological events, but not defining the direction of this correlation. OBJECTS: We present data about criminological situation of an heroin addict patients sample evaluated by consultation-liasion psychiatry in Ramón y Cajal hospital in Madrid. We study legal, social and clinical variables and their relationship. METHOD: We use descriptive analysis and logistic regression to study social, legal and clinical variables of a 411 patients sample, using clinical report protocol. CONCLUSIONS: Those patients affected by HIV-1 have higher criminality index, with lower social, economic and educational level than those not affected. These results suggest an interdependence between delinquency and marginality with an early beginning of both.

[Opiate dependence users profile. A decade review].

INTRODUCTION: Substance dependence, especially heroin dependence, has become one of the most prevalent psychiatric diseases during recent years. In spite of the numerous related publications, very few articles offer a heroin users profile based on an extensive sample and a broad range of variables of opiate dependent patients in Spain, thus limiting the possibility of generalizing the different findings published. OBJECTIVE: Present a description of an opiate dependence user profile, based on the analysis of a broad range of variables in an extensive population sample. Methodology. An observational, retrospective study, with treatment group (n=1177) and no control group was established. Descriptive and survival techniques were used to analyse different variables (sociodemographic data, medical and psychiatric history, drug consumption record, risk behavior in regards to HIV infection, previous dehabituations, legal situation, treatment carried out for addiction, etc.). RESULTS: We describe a heroine dependent user pattern that coincides in the different variables with the data offered by previous reviews in Spain. CONCLUSIONS: The profile defined can be considered as representative of opiate users in Spain, and it may be used as a reference for future articles.

[Treatment with naltrexone in opiate dependents: 2 years' follow-up].

Outcome of a maintenance treatment with naltrexone (350 mg/week) are examined in a sample of 365 patients with opiate dependence disorder. Treatment was followed in an outpatient facility, in a setting similar to patient's own environment. The average attendance rate was of 198 days. Six months after the onset of the treatment, 52% of the patients still remained drug-free. Results of treatment was correlated with family support, HIV, work adjustment, use of other drugs and partner using drugs.

[Maintenance treatment for opiate dependence at a naltrexone clinic].

The outcomes of a maintenance treatment with Naltrexone (350 mg/week) are examined in a sample of 50 patients with withdrawal symptoms to opioid dependency. The therapeutic program did not include specific type of rehabilitation. Patients were instructed to keep a quiet life, restful and exercise-free. The treatment was followed in an outpatient facility, after an initial inpatient treatment, in a setting similar to patients own environment. The average attendance rate was of 6.36 months. Six months after the onset of the treatment, 46% of the patients still remained drug-free. A follow-up study found that, overall, the naltrexone treatment caused an increase in alcohol consumption whereas there was a noticeable decrease in cocaine use. This pattern continued even months after the treatment had concluded. Results yielded in the fenfluramine stimulation test showed a high basal level of stress in drug addicts (as assessed by their basal secretion of cortisol). On the contrary, they failed to respond to external straining stimuli to an equal extent as the control group.

[Update on the morbidity of psychiatric disorders in patients with AIDS: results of a clinical follow-up].

A follow-up study after one or two years of treatment was carried out on 107 AIDS patients who were seen by the Consultation-Liaison Psychiatry Unit of the Ramon y Cajal Hospital. The sample was reduced to 52 patients (49%) mainly due to the lack of check-ups in 43% and to the death of 8% of the cases. 36% of patients suffered from Delirium or Dementia and these had the greater mortality rate. Grade IV patients and those with a diagnosis of Adjustment Disorders came to the check-ups most frequently.
