SEM observations on the cellular transmigration through the wall of blood vessels in cancer of the human larynx.

Human larynges, removed surgically for advanced planoepithelial carcinoma were used for study. After employing appropriate preparation procedure the obtained material was examined in scanning electron microscope (SEM). The predominant findings were: the human cancer cells transmigrate through the venule lining at/or near the sites of the endothelial intercellular junction, capillaries and postcapillary venules constitute a territory within the transmigration of lymphocytes, piercing the body of the endothelial cell, occur in both directions, the cellular elements of the immune system are relatively more numerous gathered in the peripheral areas of the primary tumor, the obtained results are consistent with in vivo and in vitro experimental data.

Bronchial asthma and pathological processes in the nasopharynx. I. Clinical analysis during remission and attack.

In 54 patients with bronchial asthma and a control group of 15 subjects laryngological, bacteriological and cytological examinations of the nasopharynx were performed. This region was found to be inflamed in 38% of the patients with asthma; in a group of asthmatic patients hypersensitive to aspirin, the percentage rose to 87%. In the asthmatic patients, as compared with the control group, there was a diminished neutrophilic exudate, a marked epithelial desquamation reaction and an abundant lymphocyticosinophilic exudate in a considerable percentage of cases.

Sequential advancing flexion retention attachment: a locking device for the wrist-driven flexor hinge splint.

The Sequential Advancing Flexion Retention Attachment (SAFRA) was developed for cervical spinal cord-injured patients with a muscle grade of poor to fair in the wrist extensors. It is used on a standard wrist-driven flexor hinge hand splint with adjustable tenodesis bar. Through the development of the SAFRA, maintained prehension can be obtained without externally powered devices such as CO2 or electrically powered orthoses. The patient's increased functional abilities are discussed and advantages summarized.

[Epidemiological study of chronic bronchitis in a population of workers in a fertilizer factory].

The authors investigated the influence of working conditions--with particular reference to dust and fluorite pollution--on the epidemiology of chronic bronchitis in 197 subjects working in a fertilizer producing plant in Krakow (Poland). The investigation included history, respiratory function tests (VC, TVC), height and weight measurement, rhinolaryngologic examination, nasal mucosa cytology and physical examination. The environmental survey included measurements of the dust respirable fraction and fluorite concentration in the air. The prevalence of chronic bronchitis was found to increase with the length of working activity and to be much higher in smokers than in non-smokers. Furthermore, in subjects working at the sites with the worst environmental situations it proved to be twice as high as in the remainders. Likewise higher proved to be the prevalence of laryngitis and sinusitis. The prevalence of chronic bronchitis was found to be much higher in workmen with more marked spirometric changes. In subjects affected with chronic bronchitis alterations of the nasal mucosa epithelium were also found.

Blood vascular system in cancer of the larynx.

Injection methods were used for the study of the blood vascular system in 50 specimens from total or subtotal laryngectomy in patients with extensive cancer of the larynx, and in 24 normal postmortem larynges. All specimens were injected via the superior laryngeal arteries. A silicone rubber technique was found to be the most suitable for the aims of this study. A chaotic distribution of abnormal vessels that differ in shape, a rich neovascular network within the tumor, and capillary hypertrophy in its vicinity are the main features of the blood vascular system in laryngeal cancer. The results obtained indicate great tumor-angiogenic ability that can be indirectly explained by action of an angiogenesis factor diffusing from cancer cells. This may open new possibilities for the treatment of carcinoma of the larynx based on anti-angiogenesis.

Aged

Vascularization of ossified cartilage and the spread of cancer in the larynx.

Twenty specimens for laryngectomy in extensive cancer of the larynx were examined after injecting the blood vessels with silicone rubber. The behavior of the vessels during the process of destruction of the laryngeal cartilages by the neoplasm were evaluated, as was the part played by the vascular system in spreading cancer in the larynx and its passage beyond the anatomical borders of this organ. Cartilages are damaged by cancer mainly in ossified and vascularized sites. The nonossified and avascular cartilages, on the contrary, are maintained in an almost unchanged condition amid the extensive tumorous masses surrounding them.
