[The clinical manifestations and diagnosis of Lyme borreliosis].

Lyme borreliosis (LB), an infectious disease occurring in the western and eastern hemispheres, is described. The disease that had been observed retrospectively for several decades was verified in the 1980 s. The causative agent of the disease is a new species of Borrelia transmitted with ticks. LB is characterized by staged development and impairment of some systems of the body. Erythema annulare occurring after the sucking of the ticks is a typical skin manifestation of LB. In the USSR, 90 patients with tick-borne erythema underwent examinations. 64 percent of the patients demonstrated nervous system injuries (serous meningitides, neuritis of craniocerebral nerves, and lesions of the peripheral nervous system). Serologic tests performed in part of the patients with LB in the USSR and USA identified similar responses to the causative agent of the disease.

[Clinical characteristics of tick-borne spirochete infection (Lyme disease)].

A total of 54 patients with tick-borne annular erythema (TAE) were investigated. In 41 patients different nervous system involvement signs were observed: radicular syndrome, serose meningitis, limbs paresis. Epidemiological and clinical signs of TAE in the USSR are much like those of Laim disease in USA and Western Europe. Laboratory investigations that were performed in 13 TAE patients have detected antibodies to the infecting agent of the Laim disease in 8. Treatment with broad spectrum antibiotics had a beneficial effect in these cases.

[Mental disorders in encephalitis caused by the herpes simplex virus].

The article presents the results of a combined (clinicopsychopathological, neurological, computed tomography) investigation of 28 patients with encephalitides caused by the virus of herpes simplex type 1 at various periods of the disease. The peculiarities of psychopath-like signs at the initial stage of the disease are shown in their relation with wekefullness level fluctuations and epileptic reactions. Diagnostic and prognostic values of distinct psychopathologic signs are evaluated. Major variants of psychoorganic signs in reconvalescence and later periods are described. Besides the impact of the factor of severity of neurological disorders in the disease, the role of a local factor in shaping the clinical picture of deficit-related psychopathological disorders is stressed: preferential lesioning of frontotemporal cortex and the limbic system structures.

[Immunoglobulins in the cerebrospinal fluid and blood in acute neuroinfections].

The authors have examined 42 patients with viral encephalitides and other central nervous system lesions using a complex of clinical and viroimmunological methods of examination. The main emphasis has been laid on measuring immunoglobulins A, M, and G in the blood serum and cerebrospinal fluid. The results have shown marked changes in humoral immunity. The degree of these changes is directly correlated with severity of encephalitis. Investigation into humoral immunity in patients with neuroinfections and other nervous system diseases contributes to the development of differential diagnostic criteria and better understanding of the relationship between severity and outcome of diseases.

[Humoral immunity in subjects with a history of tick-borne encephalitis].

A total of 137 patients with a history of focal and meningeal forms of tick-borne encephalitis (TBE) were examined in the period from 1 to 7 years after the acute stage of the disease. In their examination the authors used hemagglutination inhibition test in the acute and the long-term periods and neutralization test in the late periods. Antibodies to the TBE virus were detected in individuals living in the western region of TBE prevalence (Latvian SSR). The results of the clinical and immunological investigation help to obtain a more comprehensive understanding of the peculiarities of TBE pathogenesis, the stability of the post-disease immunity, as well as to solve more successfully the questions of immunoprophylaxis and medical expert examination of the patients.

[Incidence and features of neuroinfections induced by lymphocytic choriomeningitis virus].

A follow-up examination of blood sera and cerebrospinal fluid was carried out in 413 patients with various neuroinfections and related diseases. The modern immunological methods were employed: the complement fixation test, the fluorescent antibody test as well as immuno-enzymic and radioimmunoassays. It was established that 8.5% of serous meningitides and 12% of encephalitides were induced by lymphocytic choriomeningitis (LCM) virus. The verified diseases were subjected to a clinical analysis. It is emphasized that the immunological examination of the cerebrospinal fluid in patients with LCM infection contributes to a more detailed study of the pathogenesis of the disease.

Appearance of antibodies to two viruses in cerebrospinal fluid of patients with aseptic meningitis.

Two cases of aseptic meningitis with simultaneous presence of antibodies to lymphocytic choriomeningitis (LCM) and mumps viruses were observed. In the 1st patient antibodies to both viruses appeared in sera and cerebrospinal fluid (CSF). The 2nd patient displayed rising serum antibodies to both viruses and CSF antibodies to mumps virus. Immunological findings and clinical features of the patients are discussed.

[Clinical and computer tomographic diagnosis of necrotic encephalitis].

Twenty-five patients with a suspicion of necrotic encephalitis were examined by computer-aided tomography (CT). Brain tumors were detected in 5 patients. Three patients had negative results of CT examination and later other etiology was established in these cases. Fifteen patients showed characteristic foci of diminished density in the cerebral substance; in 3 cases necrosis was verified postmortem. The time-course of the encephalitis process was traced in the majority of patients. The diagnosis based on laboratory findings made it possible to confirm the etiological role of herpes simplex virus in the development of encephalitis in 2 patients with negative results of CT examinations. Possible causes of the negative results are discussed. Possible involvement of the lenticular nuclei, the brain stem and the cerebellum in necrotic encephalitis is demonstrated. The method is recommended for the early diagnosis of necrotic encephalitides.

[Lesions of the nervous system in tick-borne erythema annulare].

The authors examined thirty-two patients with tick-borne erythema annulare which developed after tick bites. The disease had a peculiar nosological form and was characterized by tick sticking, an incubation period, the presence in the majority of patients of the temperature reaction and manifestations of general infection in the acute period, the development of migrating erythema annulare, frequent lesions (26 patients) of the nervous system in the form of the radicular symptoms, serous meningitis, pareses of the facial muscles, etc. Electrophysiological examination of patients revealed marked changes. Serological tests for tick-borne encephalitis proved negative. The time-course of the disease in the majority of patients was favourable. The disease was characterized by all features typical of an infection lesion, with neurological disorders being predominant, which requires further investigation.

[Diagnostic significance of immunologic study of cerebrospinal fluid in neuroinfections].

An increase in the antibody titre in the blood serum, previously considered sufficient for determining the etiology of neuroinfection can no longer be regarded as a satisfactory index in the light of the contemporary level of our knowledge. The literature and the authors' own data show the importance of a simultaneous examination of antibodies in the cerebrospinal fluid and the blood serum in some neuroinfections. For example, the determination in the cerebrospinal fluid of antibodies to herpes simplex virus in herpetic encephalitis is considered sufficient (in the presence of the characteristic clinical picture) to make the diagnosis of this severe disease. The examination of antibodies to herpes simplex virus in the cerebrospinal fluid of 35 patients with a suspected herpetic encephalitis revealed their presence in 34% of those studied. The data obtained suggest that immunoassay of the cerebrospinal fluid and blood sera should be used on a broader scale in patients with acute and chronic neuroinfections. The method plays an the early diagnosis of these diseases and early administration of the appropriate treatment.

[Sequelae of different clinical forms of the acute stage of tick-borne encephalitis].

One to 38 years after the disease onset follow-up examinations were carried out involving 311 survivors of acute tick-borne encephalitis (TBE). Subjects from different regions of the Soviet Union were examined. Three forms of acute period were identified: focal (FTBE), meningeal (MTBE) and pyretic (PTBE). Of 104 patients with a history of FTBE, 52 displayed the progression of the disease at some time while the rest developed the residual organic syndrome of varying severity. Patients who had suffered MTBE (173) and PTBE (34) showed signs of organic and functional disorders of the central nervous system. All the forms of TBE were marked by a prolonged asthenic period. Survivors of TBE are in need of a long-term outpatient observation irrespective of the form of acute TBE.

[Electroencephalographic changes in patients with acute encephalitis].

EEG was used to examine 118 patients aged 17-60 with different types of acute encephalitis. In addition to EEG, clinical and laboratory methods, special (virological, serological, etc) ones were used for disease etiology specification. Necrotic (herpetic) encephalitis was diagnosed in 18 patients. EEG changes differed in their characteristics and correlated with the status gravity. Hypersynchronous slow activity was marked in the most grave patients. In acute encephalitides, EEG was considered to be a sufficiently informative differential-diagnostic and prognostic method of patients' examination.

[Acute necrotic encephalitis in adults].

Twenty-five patients aged 15 to 68 years suffering from acute necrotic encephalitis were subjected to combined examination involving epidemiological, clinical, electroencephalographic, echoencephalographic, pathomorphological, virological, immunological, and computerized tomography studies. The results obtained showed both the distinctive characteristics of the pathogenesis of this disease (with vascular and allergic disorders being its constant feature) and a probable etiological role played by Type I herpes simplex virus and various respiratory viruses in more than half of the cases. Efficacy of pathogenetic therapy is shown. Further areas of research in the field of necrotic encephalitides are also discussed.

[Structure of progressive forms of tick-borne encephalitis].

On the basis of long-term follow up (from 2 to 22 years) of 175 patients with various syndromes of progressive forms of tick-borne encephalitis (TBE), evaluation criteria of TBE progression are systematized. Two basic forms of disease progression are identified: amyotrophic and hyperkinetic, each of them breaking down into a series of leading syndromes. Important for characterizing progressive forms of tick-borne encephalitis (PFTBE) are the time when the disease began to progress and the pattern of progression as well as its stage and severity. PFTBE are correlated with the acute period syndromes. In long-term follow-up, 68% of patients with PFTBE display transformation of clinical forms of the disease, with the formation of the lateral amyotrophic sclerosis syndrome in the overwhelming majority of these patients.

[Encephalitis related to influenza in adults].

The results of clinical, virological, and pathomorphological examinations of 25 patients with nervous system involvement into the morbid process caused by influenza infection are presented. The influenza-associated encephalitis is regarded as a manifestation of an infectious-allergic process, in the pathogenesis of which the vascular factor plays the leading role. Therapy with vasoactive drugs combined with glucocorticoids has appeared to be the most efficacious.
