[Systen--transdermal estradiol for substitution following surgical menopause].

The authors investigate the clinical effect of transdermal system delivering 17 beta-estradiol for treatment of 23 patients after bilateral ovariectomy for benign genital tumors. During four to six months treatment Kuppermann index falls and karyopyknotic and maturation indices rise significantly. The side effects are extremely rare. The authors recommend the widely use of Systen for replacement therapy in surgically induced menopause.

[Clinical study of a low-dose contraceptive--Femodene].

The authors present their clinical experience with a low-dose monophasic contraceptive consisting of 30 mcg ethinylestradiol and 75 mcg a gestagen--gestodene (Femoden, Schering). 112 menstrual cycles of 22 women in fertile age were followed. The results show 100% contraception, a good control of the menstrual cycle and tolerance of the preparation.

[A clinical trial with the monophasic contraceptive Marvelon].

The authors present their clinical experience with a monophasic preparation consisting of 30 mcg ethynyl estradiol and 150 mcg desogestrel (Marvelon, Organon). 121 cycles of 24 fertile women were followed. The results show a full contraceptive success, a stable cycle control as well as a low incidence of the side effects.

[Transdermal estradiol--the possibility for replacement therapy in the postmenopause].

The author presents his experience in the treatment of 21 early postmenopausal women (mean age of 49.2 yrs) with transdermal delivery of 17 beta-oestradiol (Estraderm TTS, Ciba). During the treatment Kuppermann index falls from 28.5 to 10.9. Karyopyknotic index increases from 6.8 to 23.4 and maturation index moves from 39:52:9 to 0:71:29. Genital bleeding occurs in 52% of the patients. The author concludes that the treatment with transdermal delivery of oestradiol could be used more widely for replacement therapy in the postmenopause.

[The formulation of the current management of patients with atypical hyperplasia (AH)].

The authors study is the receptor status of 16 patients with AH. The amount of ER and PR in patients with AH is compared to the amount of the same receptors in patients with highly differentiated adenocarcinoma. A progestin treatment is proposed for AH together with dynamic evaluation of the amount of ER and PR. In patients ER, PR, even if they have not complete their reproduction, hysterectomy is proposed. The cases ER+, PR+ are actively followed up. Every patients should undergo complex investigation and evaluation in which together with the receptor status extremely important is the routine biopsy.

[Kliogest--the possibility of hormonal substitution in the postmenopause].

The author presents his personal experience of treatment with a combination of natural estrogen--17 beta-oestradiol 2 mg and a gestagen--norethisterone acetate 1 mg on 38 climacteric women of mean age of 53.8 years. The effect on both climacteric symptoms (index of Kuppermann) and hormonal vaginal smears (karyopyknotic and maturation indices) was very good. The preparation Kliogest (Novo Nordisk A/S) appears to be one of the preparations of choice for HRT in postmenopausal women.

[Our clinical experience in treating endometriosis genitalis externa with the preparation Danoval].

By means of the serum level of estradiol and the vaginal smear indexes the antigonadotropic activity of the preparation Danoval (2-3-isoxasol-17-ethinyltestosterone) is indirectly evaluated on 15 infertility patients with laparoscopic proved endometriosis. After 5 month's course of treatment with 400 mg daily the level of estradiol decreased from 1.54 to 0.31 nmol/l, IA from 20.8 to 10.4 and IP from 32 to 17.3. The side effects are slight and descend from the pharmacological properties of the preparation. Two patients became pregnant in three months after the treatment.

[The social adaptation of older primiparae].

A social-psychological study was carried out on 96 pregnant women divided into three groups: I (control)-20-25 year-old, II-26-34 year-old and III (experimental)-over 35 year-old, the so called elder primiparous women. Giessen test was performed. It was established a positive social resonance in a correspondence with better contact possibilities and a good social potency in the control group. The third group has normal role interrelations in slightly increased intraphysical control and positive social resonance leading to some obedience and depression which explains the embarrassment for the establishment of stable relation and marriage. It is drown a conclusion that elder primiparous women are high risk group from psychological and social-psychological point of view.

[The treatment and prevention of premenopausal uterine hemorrhages with Trisequens].

The authors describe the results from 30 women with premenopausal uterine bleedings and accompanying neuro-autonomic climacteric symptoms, treated with the combined estrogestagenic preparation (Trisequens--Novo Nordisk A/S). The mean age of the contingent is 47.64 years. The clinical characteristics of pseudomenstrual function, climacteric symptoms, fatty metabolism and carbohydrate tolerance are examined. Stable pseudomenstrual function, discontinued or sharply reduced climacteric symptoms, lack of substantial changes in the fatty and carbohydrate metabolism are found after treatment. It is necessary that similar treatment could be used considerably more widely in the daily gynecological practice.

[The results of a clinical trial of the preparation Ossopan].

The preparation Ossopan was used for period of one month in 20 pregnant women at 28 to 40 weeks' gestation with pain syndrome in the long bones, lumbosacral region and disturbed dental status. Its effect was studied on second group of 13 women with established clinically and roentgenologically climacteric osteoporosis, using a therapeutic course of 1 pill daily for a period of 6 months. A considerable clinical improvement of pain syndrome and dental status was found in the end of the treatment. The positive therapeutic effect, easy way of administration, its good tolerance and lack of side make it convenient for treatment of disturbances in calcium-phosphorus balance.

[The use of the triphasic contraceptive preparation trisiston for therapeutic purposes].

After a brief review of the qualities of the new triphasic contraceptive preparation Trisistone and some of its advantages in comparison with the other combined contraceptive drugs the authors report on the results from the first study, carried out in our country, on the therapeutic effect of Trisistone. Thirty-eight women with various gynecological diseases were treated. The largest group was that of women with irregular bleedings. In this group the effect was not good in all women. It was found that Trisistone had limited possibilities to control irregular uterine bleedings, which most probably was connected with its comparatively low dose of steroid hormones. The side effects were comparatively slight and not quite frequently. The most frequent side effect was the occurrence of irregular bleeding in 9.5% of the treated women. The state without any side effects was improved in 30.1% of the patients. Best results were found in women with postcastration syndrome, hypoestrogenism and congenital anomalies in the development of ovaries--primary ovarian dysgenesis. Trisistone is especially suitable for substituted treatment in women with ovarian hormonal deficit.

[Dynamic gamma-camera excretory scintigraphy in the diagnosis of a rare anomaly in the development of the internal genitalia (the Slotnik-Goldfarb syndrome with uterus unicornis)].

The combination of a missing ovary and uterine tube on the one side (the syndrome of Slotnik-Goldfarb) with ipsilateral absence of the uterine horn is a rare anomaly, undescribed in our obstetric-gynecological literature. The difficulties in the diagnosis of this anomaly are connected with the necessity to use invasive methods for establishment of respective changes. The method was used in a woman, suffering from sterility, in connection with examining the possibilities of dynamic gamma-camera scintigraphy to discover ovarian parenchyma. Hysterosalpingography proved the presence of a hypoplastic uterine horn on the right side, an uterine tube on the same side (changed and nonpatent). Dysovulatory syndrome was established in this woman clinically. The performed scintigraphy confirmed the presence of ovarian parenchyma on the right side as well as a hypoplastic uterine horn and uterine tube and complete lack of accumulation of radionuclide on the left side, which suggested absence of ovarian parenchyma in the sideé in which the uterine horn and uterine tube were undeveloped as well. It is evaluated that the scintigraphic technique, described by the authors, could be used for establishment of such rare anomalies by noninvasive way.

[The secretion of the main ovarian steroids during bleeding in the premenopause].

The authors study the secretion of the basic ovarian steroids (estradiol, progesterone and testosterone) during dysfunctional bleedings in 46 women at the premenopausal phase of climacteric (mean age of 47.02 years). The results show that the premenopausal bleeding occurs on the background of manifested hypoestradiolemia (2.5 times lower than that at the early follicular phase), hypoprogesteronemia (relatively higher that at the early follicular early follicular phase) and relatively high testosteronemia. Determination of the hormones during three successive days show lack of definite dynamics in the secretion of these ovarian steroids. Considerable individual differences are described.
