[The classification of Lyme borreliosis (Lyme disease)].

A new version of Lyme's disease classification based on the authors' experience and other classifications is proposed. It distinguishes periods of the disease (acute, subacute, chronic) and stages (I--isolated erythema migrans, II--local disseminated infection, III--generalized disseminated infection) as well as the signs which are significant in Lyme's disease diagnosis: erythematous and nonerythematous form, seropositivity or seronegativity against Borrelia burgdorferi. Subclinical (latent) infection, complications of Lyme's disease (fibromyalgia syndrome, chronic fatigue syndrome, etc.) and mixed-infection with tick-borne viral encephalitis are included as well.

[The "second disease" and "second nature"].

An idea about the second disease as a metamorphosis when the phenomenon of one nature is transformed into phenomenon of the second nature is proposed. There are three stages in the development of the second disease: origin, when clinical and morphological features of initial and new nosologic forms coexist; formation-distinct groups of features of a new nosologic form which preserved a link to the first disease; separation of this new nosologic form with its typical clinical and morphological features. The second disease is metamorphosis of the first disease which develops into a new nosologic entity under the influence of exogenous or endogenous factors.

[Various aspects of the classification, clinical course and diagnosis of monocytoid and histiocytoid hemoblastoses].

Analysis of the reported data and authors' observations (41 patients with monocytoid leukemias and 21 patients with histiocytoses) made it possible to conclude that in most cases, histiocytoses are multifocal tumors with diverse morphological manifestations. A classification of histiocytoid tumors is offered with regard to the process extent, the nature of bone marrow damage, morphological features, the degree of final differentiation of tumor cells, and the clinical and laboratory signs of neoplastic activity.

[State of microcirculation and the system of hemostasis in Sjögren's disease, Sjögren's syndrome and chronic parenchymatous parotitis].

Microcirculation and hemocoagulation were examined in 88 patients with Sjögren's disease and Sjögren's syndrome (SD and SS) in 20 patients with chronic parenchymatous parotitis (CPP) without a "dry syndrome", and in 20 healthy donors by conjunctival angioscopy, capillaroscopy of the nail matrix, and histological study of the salivary glands. Hemostasis was explored with the use of 37 tests. SD and SS patients manifested significantly more pronounced microcirculatory disorders versus donors (P less than 0.001) and CPP patients (P less than 0.001). In patients with hypergammaglobulinemic purpura, microcirculatory disorders were found to be enhanced (P less than 0.05). An indirect relationship between these disorders and the degree of lacrimation lowering was established. Intravascular disturbances increased as the process activity rose and diminished under the effect of prednisolone therapy. It is assumed that microcirculatory and hemocoagulation disorders play an important role in the pathogenesis of SD and SS.

[Clinical application of the international histological and cytological classification of hematologic and lymphatic neoplasms].

The first part of the International classification of neoplastic diseases of haemopoietic and lymphatic tissues by Mathé et al is discussed. The classification is exhaustive but the Table does not include all varieties of hemoblastoses mentioned in the explanatory text and shown in illustrations. Some of the terms seem disputable or inaccurate. The structure of leukemias (623 cases) based on the data of the Sverdlovsk regional haematological centre for 1975-1979 is presented, as reconsidered according to the above classification. The role of trepanobiopsies in diagnosis of leukemias is emphasized.

[Opisthorchiasis in Tobolsk based on 1950-1987 autopsy data].

Retrospective analysis of medical documentation of the Tobolsk integrated prosectorium in 1950-1987 was made. Uncomplicated opisthorchiasis as the major and direct cause of death was not encountered. In thanatogenesis the complicated opisthorchiasis generally appeared as a mixed abnormality. No evolving opisthorchiasis-induced pathology was found. Positive pathomorphism of opisthorchiasis developed.

[Medico-expert qualification of iatrogenic diseases].

Medical expert qualification of iatrogenic disease is developed on the basis of insurance assessment of events (accident, realized risk, error) and classification of their causes according to the principle of the triad patient-medical professional-environment taking into account medical technology (diagnosis, treatment, rehabilitation, prophylaxis). Medical error is considered as an integral term and include inadequate (erroneous) professional thinking and/or action. Therefore, it is suggested to qualify a medical error either as a fallacy, or negligence, or omission. Medical-technological standards, apart from legitimacy, are necessary for realization of an expert activity.
