Small bowel double-contrast enema in stage III ovarian cancer.

The efficiency of small bowel double-contrast enema in the detection and localization of tumor- or therapy-induced lesions of the intestine was studied retrospectively in 43 patients with stage III ovarian carcinoma. The radiographic findings in 62 examinations were verified by operative and autopsy findings and by the clinical course. Postoperative changes in the small bowel were noted in 69% of the patients (63% moderate, 6% severe). Signs of acute radiation enteritis were found in 36% (all moderate). Signs of chronic radiation enteropathy were detected in 71% (53% moderate, 18% severe). Small bowel obstruction due to recurrent tumor was correctly identified in 9%. Nonobstructing peritoneal implants were detected in 27% of the patients. The small bowel double-contrast enema is accurate in localizing lesions resulting from adhesions, acute and chronic radiation enteritis, or obstructing tumor; it is less efficient in detecting nonobstructive peritoneal metastases. The major clinical value of this examination is its ability to differentiate "dysfunctional intestine," which is managed conservatively, from focal obstruction requiring surgery. The radiographic features of chronic radiation enteritis on double-contrast enema examination are discussed in detail.

Barium Sulfate↗

The adhesiometer: a simple device to measure adherence of barium sulfate to intestinal mucosa.

A simple, inexpensive device assessing barium sulfate adherence to alimentary tract mucosa was tested in an animal study using pigs and dogs. Interaction of gastric, intestinal, and colonic mucosal lining with three different barium preparations was studied. In both pigs and dogs, barium adherence to gastric mucosa was significantly stronger when compared with colonic mucosa.

A logical approach to renal stone removal.

On the basis of experience with over 350 percutaneous stone extractions, a logical approach assuring a high success rate and minimizing complications is described. The importance of the position of the kidney in the body, the caliceal anatomy, the renal arterial anatomy, and the instrumentation used on puncture site selection are discussed. Strategies for removal of caliceal, pelvic, ureteral, and staghorn calculi, rationale, and treatment of complications are also described.

Percutaneous transluminal angioplasty for treatment of renal artery stenosis in children.

Angioplasty was carried out in four children with hypertension and renal artery stenosis, including one patient with neurofibromatosis and one patient with multiple arterial stenoses. Two patients did not respond. In the patient with neurofibromatosis the stenosis persisted unchanged despite multiple balloon inflations. In one patient hypertension persisted despite successful dilatation, and was probably related to longstanding contralateral kidney disease. Two patients had excellent blood pressure response to dilatation. One of these patients required redilatation of bilateral recurrent stenoses.

Sonography of the gallbladder in bone marrow transplant patients.

Nonshadowing opacities in the gallbladder (sludge) occurred in nine of 44 bone marrow transplant patients as a nonspecific finding. Sludge occurring within 2 wk of bone marrow transplant was transient. Later, sludge accompanied hepatic graft versus host disease in seven of 10 patients with this complication of bone marrow transplant. During the course of graft versus host disease, disappearance of sludge matched clinical improvement. Persistence of sludge in patients with hepatic graft versus host disease was associated with a poor prognosis. The gallbladder of one patient who underwent cholecystectomy exhibited histopathologic findings of graft versus host disease.

Detection of intestinal diverticula by double-contrast small bowel enema: differentiation from other intestinal diverticula.

This report documents the roentgenographic diagnosis of diverticula in the jejunum and ileum. The experience has been gained from 7 Meckel's and 8 other diverticula shown by the small bowel enema in 400 patients. Diverticulum detection rate by this technique approaches the incidence in autopsy series. Differentiation between Meckel's, acquired, and pseudodiverticula by means of specific radiologic criteria is discussed.

[Radiologic and endoscopic correlation in Crohn's disease of the colon].

A comparative assessment was carried out in 47 patients with double-contrast irrigoscopy (DCI), colonoscopy and endoscopy, the confirmed diagnosis being Crohn's disease in each of the patients. However, the histological finding of Crohn's disease was established in 19% only of the endoscopic biopsies. DCI enabled a more comprehensive assessment of the entire colon than was possible via endoscopy, since the right half of the colon could be visualized in less than 50% of the patients by the endoscopic method, whereas roentgenologic visualization of this part of the colon was possible in more than 90% of the patients. There was good agreement in respect of parts of the colon visualized both via endoscopy and via x-ray examination. As far as the assessment of deeper mucosal lesions was concerned, radiology proved superior, whereas endoscopy offered advantages in respect of changes on the level of the mucosa.

Work in progress: transcatheter embolization with cyanoacrylate and nitrocellulose.

The authors describe a modified form of isobutyl-2-cyanoacrylate (IBCA) which has been mixed with less than or equal to 4% w/w of nitrocellulose. Transcatheter embolization was carried out in rabbits and dogs without complications. The glue was readily injectable without balloon catheters. Because of its delayed polymerization, it was carried into smaller peripheral arterial branches by the bloodstream, resulting in complete organ ablation without development of collateral circulation. Other advantages of the modified compound include improved plasticity, adherence to not only ionic but also dry non-ionic surfaces, and the ability to control the duration of polymerization.

Patency of the ductus arteriosus after balloon dilatation: an experimental study.

Balloon dilatation of the ductus arteriosus was carried out in vivo in newborn pigs and on postmortem specimens from humans and piglets. The ductus was functionally closed in all newborn animals but patency resulted in all animals after balloon dilatation. Left-to-right shunts of 50% to 70% were found with anatomic lumen sizes of 3 to 5 mm. Patency was demonstrated up to 6 weeks after dilatation. Histologic examination showed splitting of the internal elastic layer and media, areas of hemorrhage confined to the media, and preservation of the adventitia. Mediastinal hemorrhage did not occur. This new technique is useful as an animal model of patent ductus arteriosus and could theoretically be used for palliative treatment of ductus-dependent congenital heart disease.

A new percutaneous vena cava filter.

A new, percutaneous vena cava filter was developed and tested in 11 dogs. Nitinol wire (0.045 cm diam) spirals were straightened and passed through an 8 French Teflon catheter into the inferior vena cava. The filter resumed its original spiral shape on warming to body temperature and was left in place up to 6 weeks. Long-term patency and capture of injected radiopaque clots were demonstrated. This new filter design may be a simple alternative to currently used implants.

[Experience with double-contrast examination of the small intestine].

Experiences and results of 600 small bowel examinations in double-contrast technique are reported. Pathologic lesions were found in 40%. The double-contrast technique has proved to be an efficient method in detecting small bowel pathology. Its specific value lies in precise delineation of pattern, extent and intensity of disease.

[Results of double-contrast examination of the colon].

Experiences and results of double-contrast enemas are reported. The accuracy of double-contrast enemas is proved by 500 consecutive investigations. Correlation of endoscopic and roentgenologic investigations showed, that the double-contrast enema is a reliable method concerning the detection of polyps, carcinomas and inflammatory colon diseases. Advantages and disadvantages of roentgenology and endoscopy of the colon are discussed.
