[Temperature-dependent pseudoleukocytopenia and in vitro studies on the underlying mechanism].

Spuriously low electronic white-cell count was obtained on EDTA-anticoagulated blood from a 54-year-old male suffering from liver cell carcinoma. In an attempt to understand better the phenomenon, in vitro studies were performed. The blood smear anticoagulated with EDTA revealed remarkable agglutinations of leukocytes, mainly neutrophils but not lymphocytes. The EDTA-treated blood showed maximal leukocytes-agglutinations at room temperature but no agglutination at 37 degrees C. The agglutinated leukocytes, moreover, were dispersed by incubating the blood at 37 degrees C. Although agglutination occurred in some degree in the blood anticoagulated with Na-heparin or Na3-citrate, it was much less pronounced compared to the EDTA-treated blood. Remarkable leukocytes-agglutinations were induced when EDTA-anticoagulated blood from healthy donor was mixed with serum from the patient at room temperature. These results suggest that the phenomenon of in vitro leukocyte agglutination and consequent pseudoleukocytopenia is due to leukocyte agglutinin in serum from the patient. The mechanisms were discussed comparing with pseudothrombocytopenia caused by platelet agglutinin.

[Resemblance of magnesium and zinc distribution in soft tissues and bone of rats fed unbalanced mineral diets in situ].

Although the role of zinc (Zn) and magnesium (Mg) in situ may be of a critical importance in replication, transcription and repair of DNA, to our knowledge, the relationship between Mg and Zn has been described. This study, therefore, was designed experimentally to investigate the relationship between Mg and Zn in soft tissues and bone of rats after fortification of unbalanced mineral diets, and minerals and metals in soft tissues and bone were determined by inductively coupled plasma emission spectrometry. In the groups fed low calcium (Ca) diet, low Ca-Mg diet and low Ca-Mg plus high Al diet, serum Mg level was lower in the group fed low Ca-Mg diet and low Ca-Mg plus high Al diet than those in the groups fed standard diet and low Ca diet. However, there was no significant difference in Mg content of CNS tissues in rats fed unbalanced and standard diets except for that in the spinal cord of rats fed low Ca-Mg plus high Al diet. Whereas, Zn content of CNS showed lower values in the unbalanced mineral diet groups, especially in all CNS regions of the low Ca-Mg plus high Al diet group, than those in the standard and the low Ca diet group. And Zn contents in heart, kidney and abdominal aorta of rats fed low Ca-Mg diet and low Ca-Mg plus high Al diet were more decreased than those of the standard and the low Ca diet groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[Eosinophil colony stimulating activity induced by administration of interleukin-2 into the pleural cavity of patients with malignant pleurisy].

Systemic administration of interleukin-2 (IL-2) in patients with malignant diseases induces peripheral eosinophilia. In the present study, to clarify the mechanisms of eosinophilia induced by IL-2, we examined the changes in the number of eosinophils and in eosinophil colony stimulating activity (Eo-CSA) in the pleural fluid of six patients with malignant pleurisy caused by lung cancer or malignant mesothelioma, during and after intrapleural administration of IL-2. Results showed that intrapleural administration of IL-2 induced marked eosinophilia in the pleural fluid and mild eosinophilia in the peripheral blood, and that during IL-2 administration, marked Eo-CSA appeared in the pleural fluid before increase in the number of eosinophils. The Eo-CSA seemed to be a polypeptide or protein because it was trypsin-sensitive and had a molecular weight of 40-60 kDa.

Cell-mediated suppression of human hematopoiesis: evaluation by limiting-dilution analysis of hematopoietic progenitors.

We have used limiting-dilution clonal analysis (LDA) in microwells to study the inhibitory effects of T lymphocytes (T-cells) or natural killer (NK) cells on human marrow progenitor cell growth. In four subjects with normal hematopoiesis, the growth of progenitors showed single-hit kinetics both before and after T-cell removal, indicating that, in the presence of colony-stimulating activity, T-cell have no effect on progenitor growth. In a patient with marrow hypoplasia associated with thymoma, hypogammaglobulinemia, and an increased number of suppressor T-cells (Good's syndrome), the progenitor growth deviated from linearity, demonstrating the presence of cells with suppressor activity. After T-cells were removed from this sample, the progenitor growth showed single-hit kinetics. The suppressive action of E-rosette-positive cells with NK or cytotoxic activities was also suggested in a patient with severe combined immune deficiency and in a patient with T gamma lymphocytosis. Poor progenitor-cell growth in three other patients with aplastic anemia was not significantly altered by T-cell removal. Thus, LDA of human hematopoietic progenitors is useful for evaluating cell-mediated interactions affecting hematopoiesis. This method may facilitate elucidation of mechanisms of myelosuppression in clinical settings.

[Pilot combination phase II study of mitomycin C plus cisplatin for non-small cell lung cancer].

Fifteen patients (six patients with adenocarcinoma, seven patients with squamous cell carcinoma, and two patients with large cell carcinoma) with advanced non-small cell lung cancer (NSCLC) were evaluable for mitomycin C (MMC; 8 mg/m2 day 1, 8, every 3-4 weeks) plus cisplatin (CDDP; 80 mg/m2 day 1, every 3-4 weeks). Ten patients had had prior chemotherapy. Among 15 evaluable patients, no patient achieved complete response, and two patients showed partial response. The response rate of MMC plus CDDP against NSCLC was 13.3%. Toxic effects included anorexia (80%), nausea and vomiting (67%), leukopenia (53%), anemia (47%), nephrotoxicity (47%), thrombopenia (27%), liver injury (27%), and fever (7%). These toxic effects were reversible and manageable. The combination of MMC and CDDP appears to be valuable regimen against advanced NSCLC.

[Calcium regulating hormone in the pathological changes of bone in rheumatoid arthritis].

To further elucidate the pathogenesis of calcium regulating hormones and bone Gla-protein (BGP) on the deteriorated bone changes in rheumatoid arthritis (RA), the degree of the appendicular bone atrophy, using microdensitometry method in patients with RA, and the concentrations of midchain mainly recognized parathyroid hormone (p-PTH1-84, intact parathyroid hormone (in-PTH1-84) and BGP in sera of patients with RA were measured. With concomitant progression in X-ray stage in RA, the levels of p-PTH1-84, in-PTH1-84, BGP, 25 (OH) D, and 24, 25 (OH)2D were significantly increased in sera of RA patients. The levels of 1,25 (OH)2D were not shown significant differences among RA patients classified by bone stage. These results might be concluded that the pathogenesis of the deteriorated bone change in RA seemed to be implicated in hyper-turnover bone presumably caused by the actions of parathyroid hormone and abnormal vitamin D metabolism.

[In vivo effect of etoposide on the function of bone marrow-derived macrophages].

Proliferative and colony-forming abilities of bone marrow cells to M-CSF and the function of the bone marrow-derived macrophages were examined 2, 7, 14 and 21 days after intraperitoneal injection of etoposide. Only two days after administration of etoposide (50 mg/kg of body weight), the number of bone marrow cells per femur was significantly decreased, but completely recovered by day 7. In contrast, proliferative and colony-forming responses of the bone marrow cells to M-CSF was increased after 2 days, but there-after returned to the normal. The abilities of bone marrow-derived macrophages to adhere to plastics and to produce monokines (IL-1 and TNF) were the same as those of controls before and after treatment with etoposide. These data suggest that activation of macrophage progenitors in bone marrow may be useful in combination with chemotherapy for treatment of malignant diseases.

[The measurements of parathyroid hormones in sera of elderly osteoporotic patients with serum high alkaline phosphatase activity].

This study was carried out to investigate effects of drugs treated in the elderly osteoporotic patients on measurements of carboxy terminal, mid-region and intact PTH, by using the various kinds of PTH kits. The drugs such as 1 alpha(OH)D3, calcitonin and so on, in the treatment of osteoporotic patients, was brought about no significant inter-relationships among carboxy terminal, mid-region and intact PTH in sera of the elderly osteoporotic patients with serum high alkaline phosphatase activity.

[Relationship between parathyroid hormone and magnesium in sera of the dementia patients].

The dementia patients with central nervous or neuro-cerebro vascular disorders seemed to be decreased serum Mg due to abnormal Mg uptake in diet. An increment of PTH level was paralleled with a rise of serum Mg and hypomagnesemic group was shown to have a lower PTH level in these patients. Our results suggested that secondary hypoparathyroidism was likely to be induced by normo-calcemic hypomagnesemia.

[Influences of serum magnesium on measurements of serum parathyroid hormone].

The amino sequence recognizable parathyroid hormones (PTH) in lower and non-lower magnesemic groups in randomized samples were measured by using various kinds of the well established PTH kits. The levels of carboxy terminal, mid-region and intact PTH in lower magnesemic group were more decreased than these in non-lower magnesemic group. It is likely that the shortage of magnesium in serum makes a suppression of PTH secretion.

[Clinical evaluation of osteocalcin in sera of the patients with neoplasms of bone].

The clinical significance of osteocalcin in sera of bone tumors were measured in sera of various kinds of bone tumors. The levels of osteocalcin were higher in bone tumors than those in normal subjects. Serum osteocalcin levels in hypercalcemic groups of bone metastasis and myeloma showed a remarkable increase in a parallelism with serum ALP and LDH levels. These results suggested to be directly or indirectly activated on osteoblast by bone tumors and to be synthesized osteocalcin in osteoblast.

[Magnesium, parathyroid hormone and osteocalcin in the sera of hospitalized patients with chronic alcoholism].

Levels of parathyroid hormone (PTH) and osteocalcin in hypomagnesemic sera of chronic alcoholism patients were carried out to evaluate the roles of chronic alcohol poisoning on bone metabolism. Serum gamma-GTP activity showed inverse relationships among serum Mg and osteocalcin in these patients and PTH levels were also more decreased in these patients than in healthy subjects. These results indicated that the direct actions of alcohol might be brought about by the deterioration of bone metabolism, due to the decrease of serum PTH and osteocalcin levels by chronic alcohol poisoning.

[Randomized comparative study of CE (CDDP plus etoposide) and CE-AVN (ACNU, VCR plus procarbazine) as combined anticancer chemotherapy in small cell cancer of the lung].

A randomized comparative study of anticancer chemotherapy CE (CDDP plus etoposide) and CE-AVN (ACNU, VCR plus procarbazine) was carried out on 27 patients with small cell lung cancer (SCLC) without previous chemotherapy. In CE therapy, 12 patients received injection of CDDP (80 mg/m2 on day 1) and etoposide (75 mg/m2 on day 1-5) every 4 weeks (Protocol 1). Fifteen patients received 2 courses of CE and 1 course of AVN therapy (ACNU 100 mg/m2 on day 1, vincristine 0.7 mg/m2 once a week and procarbazine 50 mg/day, daily) for 6 to 8 weeks (Protocol 2). One patient (8%) and 2 patients (20%) achieved complete response with Protocol 1 and 2, respectively. Seven patients (58%) and 9 patients (60%) achieved partial response with Protocol 1 and 2, respectively. The median survival time (MST) was 12 and 14 months, and duration of remission was 4.5 and 7 months in patients treated with Protocol 1 and 2, respectively. No significant difference in MST and duration of remission in each group was observed. However, 2 patients (13%) treated with Protocol 2 survived more than 3 years. Both protocols were well tolerated with only moderate gastrointestinal symptoms, mild bone marrow toxicity and alopecia.
