[Antibody determination and differentiation using papain treated test erythrocytes in the gel centrifugation technique (ID-Microtyping System)].

For one year we have used the gel centrifugation method (ID-Microtyping System, Fa. Diamed, FRG) in our routine laboratory for antibody screening. The two-stage papain test in the gel centrifugation system revealed some interesting phenomena: 1) In some cases the sensitivity of the papain gel test is higher than that of all other methods in the gel centrifugation method or in the tube centrifugation method. 2) There are reactions which seem to be directed against the patient's own antigens (in the Rhesus or Kidd system), but there are no positive direct antiglobulin test and no clinical signs of hemolysis. 3) Some sera show reaction patterns which seem to have a distinct specificity, but they cannot be adjoined to any of the antigens in the manufacturer's antigenogram. Further declarations in the worksheet of the panel would be desirable.

[Adaptation of monoclonal test sera for antigen determination in the positive direct Coombs test].

Monoclonal reagents of IgM type now allow to test some red blood cell antigens (K; Jk a; Jk b) by the tube centrifugation method at room temperature for which an indirect antiglobulin test was necessary before. This enables to test antigens also when the direct antiglobulin test is positive, provided that there are no disturbances by supplements contained in the sera. We examined monoclonal blood group sera of different manufacturers for their ability to test antibody-coated erythrocytes; sometimes we found great differences between the different products. Therefore we recommend to add this test to the other criteria of quality control, especially when it is necessary to test antigens in patients with positive direct antiglobulin test.

[Demonstration os isoagglutinins in intravenously applicable immunoglobulin preparations].

Intravenous immunoglobulins (IvIg) contain not only the declared antibodies against pathogenic microorganisms but also all the other antibodies of the blood donors, e.g. against erythrocytic antigens. We tested 14 IvIg from 7 manufacturers (together 40 charges) for isoagglutinins and irregular blood group antibodies. To ameliorate the reading of our tests we used the gel-centrifugation method (ID-Microtyping System, Fa. Diamed, Bensheim, FRG). Isoagglutinins in the IvIg can influence blood group serologic tests. Therefore we point to the potential danger of misinterpretation of a positive direct antiglobulin test. In rare cases, hemolytic reactions after administration of high doses of IvIg might occur.

[Complications of and after hysterectomy. Retrospective study of 5,676 patients--guidelines for preoperative patient education].

PROBLEM: Over the last three decades, litigation resulting from gynecological operations has risen considerably. This is attributed to no small extent to inadequate preoperative information of the patient by the physician. METHODS: The medical histories of 5,676 patients submitted to hysterectomy were evaluated retrospectively and all intraoperative and postoperative complications analyzed. RESULTS: In order to be able to give the patient comprehensive information, the physician must be familiar with the nature and incidence of possible complications, on the basis not only of his own personal experience, but also of the data provided by extensive case material. Forensic disputes can be reduced only by ensuring that the patient is fully informed and thus that her agreement to the operation is legally binding.

[Drug therapy in severe tumor pain. Comparative study of a new combination preparation versus diclofenac-Na].

In a multicentric, interindividual, double-blind study, the analgesic action, duration of effect, tolerability and side effects of the new combination preparation, Combaren (diclofenac-Na 50 mg+codeine phosphate 50 mg), were compared with those of diclofenac-Na 50 mg (Voltaren 50) in 184 patients with severe tumor-related pain. The results show that Combaren is a highly effective preparation for the treatment of severe tumor pain. The combination of diclofenac-Na with codeine phosphate leads to a clear, statistically significant, augmentation of the effectiveness of additionally used analgesics on pain severity, and the general effectiveness of the combination is more positively assessed that that of monotherapy with diclofenac (also effective). In the staged approach to the treatment of malignancy-related pain in which the aim is to provide continuous, preventive analgesia rather than ad hoc treatment of newly developing or worsening pain, this combination preparation will presumably find a permanent place in stage I/II of the generally accepted staged pain-treatment scheme.

[Improved diagnosis of endometrial cancer].

Over the last few decades, carcinoma of the endometrium, the most common genital carcinoma in women, has become more and more important. For a long time, intensive efforts to improve its early detection remained unsuccessful. Not until the introduction of vaginal ultrasonography was a decisive step forward achieved in the early detection of this carcinoma on the basis of an assessment of endometrial thickness. In clinical studies, measurement of endometrial thickness by intravaginal ultrasonography has proved an outstanding parameter for the early detection of endometrial neoplasia, even in asymptomatic postmenopausal women.

[Liberation of the T-antigen in bacteremia].

Many microorganisms are able to produce neuraminidase, which can uncover the T antigen on red blood cells and cause hemolysis. We studied T activation in 224 patients with positive blood cultures. Only those patients were included who had real bacteremia according to clinical parameters and microbiological results. None of our patients showed T transformation. We conclude that T activation is a rare or a very passing phenomenon which has less importance in routine diagnosis of sepsis.

[Overweight newborn infants--incidence, causes and clinical significance].

A retrospective study of 7711 births revealed 741 (9.6%) overweight (greater than 4,000 g) neonates. Statistical analysis shows, that neonatal macrosomia closely correlates with maternal age, parity, and weight. The pathogenesis of neonatal macrosomia appears to be influenced primarily by genetic factors. 19% of the cases show a pathological oral glucose tolerance test and/or hypertension, whose role in the development of overweight neonates has yet to be determined. With the exception of shoulder dystocias, postpartal atonia and hyperbilirubinaemia, the rate of obstetric or paediatric complications was not increased compared to the control group. The delivery of overweight neonates of non-diabetic mothers does not appear to carry an increased risk of intra- and postpartal complications.
