Inhibition of heterotopic osteogenesis in rats by a new bioerodible system for local delivery of indomethacin.

A study was done to evaluate the effect of a system for the local delivery of indomethacin on demineralized bone-induced formation of heterotopic bone in the abdominal muscles of rats. Two separate investigations were conducted on a total of forty-eight Wistar rats. In both series, two types of implants were used: polyorthoester and demineralized bone (Group A, the control group) and polyorthoester with 5 per cent indomethacin and demineralized bone (Group B, the experimental group). In the first series, host-tissue responses and osteoinduction were evaluated histologically at two, three, and four weeks after the implantation. In the second series, the formation of bone was quantified on the basis of uptake of 85Sr at four weeks after the implantation. The polyorthoester system for the local delivery of indomethacin significantly inhibited demineralized bone-induced heterotopic formation of bone, as demonstrated by light microscopy and by uptake of 85Sr. The polyorthoester, with or without the drug, caused little tissue reaction and was resorbed almost completely at four weeks.

The effect of a composite of polyorthoester and demineralized bone on the healing of large segmental defects of the radius in rats.

The effect of a composite of demineralized bone mixed with polyorthoester on the healing of large segmental defects in the rat radius was studied. Sixty male Wistar rats were divided into four groups, A through D, and an osteoperiosteal diaphyseal defect of 50 per cent of the length of the bone was made in the right radius of each rat. In Group A, the defect was filled with polyorthoester and demineralized bone; in Group B, demineralized bone; and in Group C, polyorthoester. No material was implanted in the defects in the Group-D rats. The rats were killed fifty days postoperatively. The formation of bone in the defects was quantified with computer-assisted measurements of the area on radiographs. The host-tissue response was evaluated with light microscopy. Defects that had been filled with the composite of polyorthoester and demineralized bone or with demineralized bone alone showed regeneration of bone corresponding to 93.6 and 77.6 per cent of the area of the defect, respectively. Defects that had no implant or that had been filled with polyorthoester alone showed significantly less formation of bone. No inflammation was seen with light microscopy, and only traces of the polyorthoester could be detected in the defects that had been filled with the composite or with polyorthoester alone.

Effect of local hemostatics on platelet aggregation.

The platelets play an important role in the normal hemostasis, and it is known that both natural and synthetic macromolecules may induce platelet activation and aggregation. Thus, the purpose of the present study was to investigate the platelet aggregating effect of five different local hemostatics. Platelet aggregation was assessed by aggregometry. Unwoven fleece of bovine collagen polymer in fibrillar form induced aggregation in combination with small amounts of platelet agonists; ADP and adrenaline. Ordinary, nonabsorbable bone wax also induced aggregation in combination with the agonists, but larger concentrations of agonists were needed. Bioerodible polyorthoester with physical properties such as bone wax, oxidized cellulose and gelatin sponge did not promote platelet aggregation.

[Symptomatic pelvic girdle relaxation. Clinical aspects].

In pregnancy a relaxation of the sacroiliac joints and the symphysis is a physiologic phenomenon which usually occurs without symptoms. In some cases, however, these changes cause characteristic symptoms, with pain and difficulty in walking: the symptomatic pelvic girdle relaxation. The condition may be confirmed by tenderness of the joints and provocation of pain by the sacroiliac joints tests. Usually the symptoms disappear shortly after giving birth, but some women suffer for several months, and a few suffer persistent symptoms, the pelvic joint syndrome.

[Rationalization of the recording of case histories using a modular system].

Delay often occurs in preparing the case history and treatment summary for transmission to the general practitioner. At the Hagavik Orthopaedic Hospital this delay is reduced to about five days by preparing the summary on a modular system consisting of four modules; the biographical data of the patient, admission and discharge dates and the treatment classified, an abstract of the patient's hospital record and discharge notes. Since 1986 the two first modules have been prepared by computer, which has reduced secretarial work by about one week per 1,000 summaries.

Tissue reactions to wear products from polyacetal (Delrin) and UHMW polyethylene in total hip replacement.

The tissue reaction to implanted materials and their wear products is important for the long-term fixation of a total hip replacement (THR). Recent reports indicate a relatively high incidence of aseptic loosening with Christiansen total hip prostheses. We examined tissue reactions to wear products in two groups of prostheses: 16 Christiansen prostheses with polyacetal (Delrin) sockets, and 18 Charnley-Müller total hip prostheses with ultrahigh-molecular-weight polyethylene (UHMWP) sockets. Specimens from these 34 patients were compared in a combined qualitative and semiquantitative morphological study. We saw similar histological changes in specimens of both groups, but specimens from the bone-cement interfaces of the acetabulum and femur shaft in the polyacetal group showed more inflammation and necrosis.

Spontaneous correction of femoral torsion. Diaphyseal osteotomies studied in kittens.

Inward or outward torsion of about 25 degrees was produced in the right femur of kittens by midshaft osteotomy and plate fixation. The left femurs served as controls. The anteversion angle of the femoral necks was measured in axial radiographs taken preoperatively and up to 58 weeks postoperatively. As the bones grew in length, the difference in the anteversion angle between matched femurs was gradually corrected.

Wear of the acetabular socket. Comparison of polyacetal and polyethylene.

Frictional characteristics of polyacetal (Delrin) sockets and of ultra-high molecular weight polyethylene sockets retrieved in revision after aseptic loosening were compared with measurements of friction in new sockets. Friction in retrieved polyacetal sockets was twice as great as in retrieved polyethylene sockets. We also found that frictional characteristics of polyacetal changed as the material aged in vivo. In contrast, friction in polyethylene sockets remained fairly constant, even though most of them contained bone-cement particles. Friction in polyacetal sockets may be important for the relatively high incidence of socket loosening of the Christiansen prosthesis. Measurements of wear of the polyacetal sockets showed a mean annual dimensional change of 240 mm3, four times greater than that reported for the Charnley polyethylene acetabular prosthesis.

Wear of the Christiansen hip prosthesis.

The wear was examined in 39 Christiansen total hip prostheses, which were removed because of mechanical loosening after being used 5 (3-11) years. In the polyacetal acetabular cups, the head had made an eccentric defect, the mean volume of which was 680 (180-3310) mm3. The mean penetration of the head into the wall of the cup was 0.8 (0.1-3.2) mm. In two additional cups the head had penetrated right through the wall of the cup. In three prostheses the polyacetal sleeve of the trunnion was so worn that the head bore directly on the stem. There was a positive correlation between wear and the time the prostheses had been used before symptoms of loosening presented.

Relief from pain in Sudeck's posttraumatic syndrome by fasciotomy. A clinical, radiographic, and scintigraphic study.

Twenty-three consecutive patients aged 33-80 years with a presumed Sudeck's syndrome of one hand or one foot were seen. A fracture initiated the syndrome in three-quarters of them, and the median duration of suffering was 3.5 months in the hand and 7 months in the foot. Osteoporosis and marked 99mTc-labeled methylene diphosphonate uptake were seen in radiographs and scintigrams respectively. Thirteen of the patients were operatively treated; distal fasciotomy on the volar aspect of the forearm or the ventral aspect of the lower leg gave rapid relief from pain at rest in nine of ten patients thus affected. All the patients became symptom-free, except two who underwent closed treatment. At follow-up 2-8 years later radiographic and scintigraphic findings were usually normal.

The Charnley versus the Christiansen total hip arthroplasty. A comparative clinical study.

Charnley total hip arthroplasties (THA) and Christiansen THA were carried out in two concurrent groups of patients, consisting of 113 hips in 87 patients and 90 hips in 81 patients respectively. The mean age of the patients was about 65 years, two thirds had idiopathic arthrosis, and only two had rheumatic hip disease. The two groups of patients were similar with regard to sex, age, weight, etiology of arthrosis, and standard laboratory tests, but different with regard to the surgeons who operated on them. Postoperative films showed that the stems of the prostheses were in the neutral and the varus position in the Christiansen group, and in the valgus, neutral and varus position in the Charnley group. Neither group had any deaths, or deep infections in the postoperative period. Only minor postoperative complications were noted. At follow-up, 5-8 years later, the failure (revision) rate of Charnley THA to Christiansen THA was 1 to 7.7 and 1 Charnley cup was revised as against 19 Christiansen cups. The methodological weaknesses in the comparative clinical trial did not explain the inferior long-term results of the Christiansen THA.

Features resisting primary treatment of congenital club foot.

The aims of this study were: (1) to see whether the number of relapses of hindfoot deformity in congenital club foot in the first year of life could be reduced by operative treatment, and (2) to try by surgery to pinpoint features of congenital club feet that make them resistant to treatment. As compared to treatment of 95 feet with manipulation and plaster cast only or with heel cord tenotomy added, early tenotomy of both the heel cord and the tibialis posterior tendon in 23 feet markedly reduced the number of relapses of hindfoot deformity, and the need for additional treatment. The results indicate that the achilles and the tibialis posterior tendons, and their corresponding muscles, are the main dynamic features that need to be dealt with when treating the hindfoot deformity in congenital club foot.

Lack of effect of indomethacin on ordered growth of the femur in rats.

The effect of indomethacin on ordered growth in length and width of the femur was studied in 48 adolescent rats given 2 mg/kg/day of the drug, a regimen previously shown to inhibit fracture healing. The animals were given either indomethacin suspension, or the vehicle alone, orally for 6 weeks. All the animals tolerated the treatment well. During drug treatment the femur distal to the intertrochanteric crest grew about 7 mm in length. Indomethacin plasma levels were about 1 microgram/ml in the indomethacin-treated animals. Indomethacin did not inhibit ordered growth in either length or width. This indicates that indomethacin does not inhibit the normal homeostasis of the skeletal system in adolescent rats.

Inhibition of partial closure of epiphyseal plate in rabbits by indomethacin.

The efficiency of indomethacin (10 mg/kg/day) in inhibiting recurrent partial closure of the epiphyseal plate was tested in rabbits. An epiphysiodesis was done laterally in the distal left femoral epiphyseal plate in 10 adolescent rabbits. This produced a valgus deformity in 14 of them. The bone bridge was then removed operatively and the rabbits were treated with either indomethacin or vehicle for 21 days postoperatively. Indomethacin plasma levels were about 180 ng/ml. The valgus deformity improved in indomethacin-treated rabbits, whereas it became worse in the controls (P = 0.004).
