The effects of duration of stimulation, preferred hand and familial sinistrality in dichotic monitoring.

Ear asymmetries in dichotic monitoring performance were assessed in 105 subjects (77 M, 28 F). A right ear advantage (REA) was found more often in right than mixed or left handed subjects. Similarly, more left than right handers exhibited left (LEA) or no ear advantages. Handedness did not affect the magnitude of asymmetry. Familial sinistrality could only be examined in male left handers, and was found to increase the incidence of REA's. Non familial sinistrals were equally distributed in terms of left, right and no ear advantage. Overall target detection rates decreased and RT increased as dichotic stimulation progressed, but the ear advantage remained constant throughout the duration of stimulation. Responding hand did not interact with ear stimulated. Dichotic monitoring appears to be a useful non-invasive tool for determining hemispheric dominance for speech perception.

[Comparative studies during continuous epidural analgesia in obstetric (author's transl)].

From 1975 on, continuous epidural analgesia has been administered at the University Department of Anaesthetics in Ulm to women for pain relief during labour. One group of 350 pregnant women received bupivacaine hydrochloride, from 1. 5. 1976 to 30. 4. 1977, another group of 350 women received bupivacaine carbonate during the period from 1. 5. 1977 to 30. 11. 1977. Within these two groups the different effects of both local anaesthetic agents were compared in respect of the course of labour, the indications for epidural analgesia, the practicability of epidural analgesia, complications during labour, the rate of pain relief, the frequency of operative deliveries and the clinical condition of the newborn. The main advantages of bupivacaine carbonate for obstetric analgesia purposes are demonstrated.

Language laterality assessed by unilateral ECT and dichotic monitoring.

A dichotic monitoring test for the lateralisation of speech dominance was compared with testing for dysphasia after unilateral electroconvulsive therapy to each side of the head in 31 patients, and the intracarotid amylobarbitone test in four epileptic patients. Twenty-eight patients shown to be left speech dominant also showed right ear advantages in the dichotic test. Four of the five right speech dominant patients showed left ear advantages. Two patients with bilateral speech representation showed nonsignificant ear asymmetries. The noninvasive dichotic monitoring task is quick and simple and produces large mean ear advantages (14 to 25%) in normal subjects as well as patients.

[Effects of diazepam and flunitrazepam on the undesired postoperative side-effects of ketamine anaesthesia (author's transl)].

150 patients undergoing minor gynaecological procedures (curretages) were anesthetized with ketamine and either diazepam flunitrazepam or placebo in order to establish the best combination for the prevention of postoperative psychotomimetic effects. From observations, up to 24 h after operation, it was found and statistically proven that the combination of ketamine/flunitrazepam as compared to the combinations ketamine/diazepam and ketamine/placebo led to a remarkable reduction of psychotomimetic side reactions.

[Possibilities and limitations of peripheral-venous parenteral feeding].

The possibilities of peripheral-venous nutrition as regards its substrate dosage are limited due to the vein tolerance (osmolarity) of the infusion mixtures. It is, therefore, imperative to achieve an optimum composition of the solution for obtaining the most favourable utilization rate for the organism. We have shown that a dosage below 0.6 g amino acids/kg BW per day does not have a positive influence on the nitrogen balance in comparison to a control group not nourished. The situation of a patient in the post-aggression phase results in 30 to 40% of the supplied amino acids being utilized in the energy metabolism and excreted as urea, in spite of a sufficient simultaneous energy supply. Not only does this uneconomical loss alone necessitate a higher amino acid dosage, but also the additionally increased catabolism of endogenous protein reserves in the post-operative phase. It remains to be tested whether the sole substitution of amino acids in a solution without carbohydrates over a short infusion period constitutes a compromise between the intended peripheral-venous tolerance and an effect on the nitrogen balance.

[The effects of naloxone on oxygen uptake, pulmonary ventilation and cardiac performance in adult non-anaesthetised persons (author's transl)].

10 normal persons were given 0.4 milligrams of Naloxone intravenously, and the following measurements were taken: oxygen uptake, CO2 production, respiratory quotient, respiratory minute volume, tidal air, dead space, effective dead space, specific ventilation, alveolar O2 partial pressure, end-expiratory CO2 partial pressure, heart rate, systolic and diastolic blood pressure and the Burger Index. Analysis of the results established that administration of Naloxone was followed by a statistically significant, but clinically not relevant, reduction in O2 uptake and CO2 production. Higher doses produced the same results. The findings appear to contradict those observed in anaesthetised patients who are given Naloxone during the postoperative stage. However, closer examination of the results suggests that the so-called tachytrophic effect of Naloxone is attributable to its antagonistic action on the analgesic and sedative effects of opiates rather than to a specific pharmacodynamic property of Naloxone.

[Aspiration of meconium stained amniotic fluid (author's transl)].

Respiratory distress following aspiration of meconium stained amniotic fluid can be found in about 3% of newborn infants. The lethality rate of this disease is high. Pathophysiology and variability of the clinical course as well as radiological findings and further diagnostic procedures are being described. The therapeutic concept requires recognition of meconium stained amniotic fluid as early as possible, early suctioning of the nasopharynx, intubation and endotracheal suction. Further therapy consists in physical procedures, i. e. lavage, suctioning, positioning, vibration and percussing of the chest, and inhalation. Antibiotics can only be recommended if infection of the amniotic fluid has been proven.

[The anaesthesia outpatient clinic. A new concept of preanaesthetic preparation and treatment after two years experiences (author's transl)].

The anaesthesia outpatient clinic can be a new concept of preanaesthetic preparation and treatment. Related to the figures of 1977, a report is given about our 2-years experience. Patients from the usual outpatient clinics, like gynecology, urology, ENT, who are expected to be operated upon are immediately sent to the anaesthesia outpatient clinic for preanaesthetic check up. After the overall state of the patient has been examined, the original outpatient clinic and/or the family doctor are given information about the patients condition, and probably proposals for a preanaesthetic treatment are made. In 1977, 15% of all anaesthetized patients were checked as outpatients, another 40% were only examined after entering the hospital, whereas as few as 17% were not checked in the anaesthesic outpatient clinic. In patients, checked as outpatients, the period between the first day of hospitalisation and operation was evidently shorter than in patients, having been hospitalized at the time of preanaesthetic examination. This gives the opportunity to save money and beds. Furthermore, the anaesthesia outpatient clinic improves the organisation and efficiency of surgery, urology, gynecology etc., improves the relationship between patients and anaesthetists, decreases the risk of anaesthesia and operation, saves personnel and gives a qualified additional background for the purposes of graduation and student training.

Age distribution and serological reactivity of viral antigen in brains of mice infected congenitally with LMC virus.

In continuation of previous studies on the age distribution of viral antigen in spleen and kidneys of mice infected congenitally with LCM virus, the brain was subjected to a similar investigation since it participates to some extent in "late onset disease" in such animals. The antigen curve obtained differed from that for the spleen but resembled the curve for the kidneys in that an initial decrease of the antigen content in young mice was followed by a continuous rise later in live. Cerebral antigen titers in newborns and in old mice were considerably higher in carriers of the "neurotropic" strain "WCC" than in those infected with the "natural" strain "W". In old carriers of these strains there was a clear correlation between the cerebral antigen titer and the frequency and intensity of nervous symptoms which, in general, were not very striking but clearly recognizable to an experienced observer. In young congenital carriers cases of "early onset disease" characterized by spastic symptoms were observed. The latter were missing in old mice. "Early onset disease" and the temporary decrease of the cerebral and renal antigen titers in young animals are attributed to an abortive cellular immune response not strong enough to eliminate viral antigen completely from the organs. Specific antibodies, demonstrable by indirect CF in 43% of the mice reaching the age of 6-7 months, were likewise ineffective in this respect. In quantitative direct CF tests, immune sera from non-tolerant mice and, to a lesser degree, an immune serum from guinea pigs reacted better with brain extracts from congenitally infected mice than with extracts of spleens or kidneys in spite of the lower antigen content of the brain. This result seems to indicate a qualitative difference between the respective antigens.
