We report on our experiences with a powerful neodymium:YAG-laser in treatment of benign and malignant skin tumors as well as various other conditions such as removal of tattoos. The rapidity of the laser treatment which can be carried out under local anaesthesia, allows the treatment of even large tumors in critical locations as well as in the elderly. The cosmetic results are comparable to those obtained by dermatologic X-ray therapy. Before complete healing of the coagulated area is achieved, however, a rather long period elapses. The cosmetic outcome after removal of tattoos largely depends on the level of the dermis the pigment is deposited in; all in all, our results are not superior to other treatment schedules.
Treatment of large condylomata of the glans penis with a neodymium-YAG-laser is reported. The method offers certain advantages over more conventional means of therapy, but requires expensive equipment, experience and that precautionary measures be taken when the high-power laser is operated.
The investigation of patients with leg ulcers, treated with a concentrated benzoylperoxide solution, revealed a sensitization to the substance in nearly half of the probands. Therefore and because of the probable promotion of neoplastic epidermal proliferation, benzoylperoxide-containing solutions should be used for the treatment of leg ulcers in selected cases only.
Data on the prevalence of leprosy in the Federal Republic of Germany obtained by questionnaire from numerous departments of dermatology were compared to those from the German Federal Health Office to which leprosy cases have to be reported. Between 1962 and 1980, the Federal Health Office cases numbered 86, whereas 106 were reported to us by questionnaire. Of the latter, 85 patients were male, 21 female. A sharp increase in cases was seen in the late 1970s, mainly due to the growing number of refugees from Southeast Asia and to foreign workers and their family members from southern Europe. Patients from Africa and Latin America were only infrequently observed. A total of 16 patients were Germans. The lepromatous type of leprosy predominated, followed by the tuberculoid type, and the borderline cases are the least frequent. Although no secondary cases have been reported so far with certainty in this country and the risk of transmission seems small in a highly developed country such as the Federal Republic of Germany, a certain amount of awareness should be maintained in view of the important role early diagnosis plays in the fate of the patient.
This paper reports a case of malignant syphilis (man, 39 years old) in whom ultrastructural investigations of a typical nodule revealed an extremely low amount of bacteria with the characteristics of Treponema pallidum in poorly differentiated cells of the dermal infiltrate with plasma cells, stimulated lymphocytes, and neutrophils as predominating cell types. Most of the microorganisms bore signs of disintegration. Vascular changes and exocytosis were only demonstrable by light microscopy in a second nodule. Together with the high production rate of immunoglobulins and an excessive inflammatory reaction, these findings point to an aberrant biologic reaction pattern of those patients who develop malignant syphilis. Unfortunately, further investigations concerning a possible impairment of cellular immunity as supposed in the literature, had not been possible in the present case.

The case of a 51 year old woman, working in the field of agriculture, is reported; she developed a photoallergic dermatitis and a fulminant hepatitis after the use of the herbicide phenmedipham (Betanal). Photopatch-testing revealed sensitization to phenmedipham and, additionally, to maprotiline hydrochloride (Ludiomil), an antidepressant taken by the patient at about the same time. The possible relationship of these reactions with the clinical symptoms are discussed.
The present paper describes the microcomputer-based allergy data storage and retrieval system, realized with the relational database system INFORMIX on a Siemens MX2 computer. The main features of the kind and structure of data stored are discussed, as well as some general aspects of data storage. Additionally, the activities of the German Contact Dermatitis Group with regard to computer-based documentation are mentioned.
