Secondary carcinoma of the liver from parotid gland tumor.

Secondary tumors of the liver from primary tumors arising in organs of the head and neck are rarely diagnosed during the patient's lifetime, though they should be suspected. A case of parotid mucoepidermoid carcinoma with liver metastases, treated by liver resection, is described. The clinical features and biological behavior of this secondary tumor are similar to those of the rare primary mucoepidermoid carcinoma of the liver. This case is unique because it is such a very rare occurrence.

Correlation between cytochrome P-450 system and liver function tests during experimental liver transplantation.

The aims of this investigation are: 1) to assess the function of the hepatocyte in transplanted porcine liver, immediately after reperfusion, by monitoring both LFTs and the MEGX levels; 2) to search for correlation between MEGX and LFTS, in an effort to evaluate the metabolic mechanisms occurring in the early liver transplantation revascularization phase. The MEGX test was found to be less than 50 micrograms/ml in all the recipients and all the LFTS tested have been reported to be out the normal range. Furthermore our data has shown a statistically significant correlationship between the MEGX values and those of alkaline phosphatase and prothrombin and a highly significant correlationship with cholinesterase.

[The surgical treatment of liver metastases from colorectal carcinoma: our experience].

Data from twelve patients who had hepatic resections for colorectal liver metastases were retrospectively analyzed to determine: 1) whether the use of the ultrasonic surgical dissector and the Argon laser can significantly simplify major hepatic resections and decrease both perioperative blood loss and postoperative morbidity and mortality, and 2) whether an adequate patients selection for surgery can effectively determine an improvement in recurrence rate. We performed 4 bisegmentectomies (2 of V and VI; 2 of VI and VII); 1 trisegmentectomy (V, VI, VII); 2 left lobectomies; 1 right hepatectomy and 4 wedge resections, using both the ultrasonic surgical dissector to fractionate and aspirate the hepatic parenchyma and to clear major vascular and biliary structures and the Argon laser for the coagulation of minor vascular and biliary vessels. The resected metastases averaged 5.5 cm (range: 1.5-7.5); blood transfusion requirements were significantly reduced from previous reports, averaging only 1.25 units (range: 0.3); the average operative time was 238 minutes (110 to 420 minutes). There were no operative deaths, operative morbidity rate was 16.6. The results indicate that the ultrasonic surgical dissector and the Argon laser have made a significant contribution to our marked decrease in the average blood loss and transfusion requirement. The long-term results seems to be improved by an adequate patients selection.

[Polypropylene mesh in the surgical treatment of inguinal and crural hernia: our experience with 500 cases].

The authors guided by their personal experience (612 abdominal hernioplasties performed between 1992 and 1998) after using for the cure of inguinal and femoral hernias diverse prosthesis illustrate the characteristics of the polipropene prosthesis which in their opinion possesses every requirement necessary to enable it to become the defining ideal prosthesis. The authors underline again the use of this routinely applied prosthesis, also in a local anaesthesia it has favoured the development of the tension-free and suture-less technique which hes allowed hernioplasty to spread in day-surgery.

[Surgical treatment of bleeding peptic lesions: our experience].

The authors report their experience on surgical treatment of peptic bleeding lesions. From January 1994 till April 1999 they observed and surgically treated 35 patients (mean age 65) suffering from bleeding gastroduodenal ulcer. Complications linked to surgical treatment had an incidence of 17.5%, while those ones linked to the patient's general conditions of 21%; mortality was 20%. Surgery has been gradually substituted by endoscopy which represent the principal examination for diagnosis of bleeding gastroduodenal ulcer with the aid of different hemostatic techniques, so that surgery has been relegated to the last place in uncontrollable bleeding treatment. Observed results, following those ones of other authors, show the unfavourable prognosis linked to patient's different conditions when surgeon operates.

Hernia of the posterior lamina of the rectus abdominis muscle sheath: report of a case.

A case of hernia of the posterior lamina of the rectus abdominis muscle sheath in a 30 years old female, six months pregnant, is reported. The symptomatology was almost exclusively characterized by a persistent abdominal pain, located in the right costal margin at the intersection with the right adsternal line and aggravated by changes in position and by increases of intra-abdominal pressure. No bulge or specific hernial defect was clinically appreciable. The key to diagnosis, in this case, was an echography of soft tissues performed in the area where the pain was greater: with the patient in orthostatic position, it was possible to demonstrate a defect in the posterior sheath of the rectus abdominis muscle, that, increasing the intra-abdominal pressure, let pass preperitoneal fat between sheath and muscle. Both the predisposing (anatomic and clinical) factors and the provocateurs ones, probably involved in the genesis of this peculiar case, are discussed.

[Ambulatory surgical treatment of primary hernia: our experience].

The authors report their personal experience (169 pz.) of the primary inguinal hernia repair in day surgery treated in local anaesthesia and by applying prosthesis according to latest "Tension-free and Suture-less" techniques. Results surely confirmed the validity of the method and the utility in creating centres concerning this type of surgery so that we are able to grant to the patient the best comfort that is to say a relevant reduction of the recurrence and a faster reintegration of patient's activities daily living.

[Dual mesh-plus for wall reconstruction in incisional and umbilical hernia in the aged].

The article reports the author's experience about the use of Gore-Tex Dual Mesh Biomaterial in large incisional hernia operations in patients over seventy. From January 1996 at present day we operated on 23 patients for treatment of abdominal wall defect. In all patients we used a Dual Mesh. Overall mortality was O. Morbility was 9.5%. Follow-up is too short for definitive considerations about the incidence of recurrences but our initial experience with this material encourages us to use it again for replacement of abdominal wall defects especially in the old patient.

[Surgical treatment of mucosal hemorrhoidal prolapse using a circular stapler].

We present a retrospective clinical study concerning our personal experience with the circular stapler in the treatment of hemorrhoids; the aim of this study was to evaluate the results of this surgical procedure, in terms of operative time, postoperative pain and rate of both short and long-term complications. Twenty-seven patients with grade 3 or 4 hemorrhoids, from January 1999 to June 2001, were included in the study. The main technical details of this procedure, requiring only a short learning period, are described and both short-term complications (such as severe postoperative pain, bleeding, urinary and fecal retention) and long-term ones (such as persistent or recurrent haemorrhoidal prolapse, anal stenosis) are analyzed. The reported results show that, in the presence of appropriate local anatomic conditions, this procedure is able to reduce the operative time, is almost painless and is characterized by low rate of complications.

[Treatment of postoperative entero-cutaneous fistula: personal experience].

The authors present their experience on surgical treatment of intestinal fistulas based on personal cases treated from 1981 to 1990. The cases are divided into two groups: the first: the first group of patients was treated only surgically (1981-1984); the second group was treated with surgery and artificial nutrition (1984-1990). The mortality rates are much lower in the second group. The conclusion is that is always necessary to associate a form of artificial nutrition to surgery because the percentage of healing is statistically higher.

[Sentinel-node biopsy for breast cancer: our experience].

The method of sentinel lymphnode is a new procedure, suitable for the cases of early diagnosed breast cancers: identifying the first lymphatic station of the cancer, it can avoid the axillary lymphadenectomy in those women with primitive invasive and unifocal neoplasis of not more than 1.5 cm in diameter and without lymphnodal invasion. So it is reduced the frequency of postoperative troubles (sensitive and motor diseases, lymphedema). In our study, we have researched and identified, using radioactive isotopes, the sentinel lymphnode in 20 women suffering from subclinic breast cancer. We have obtained a percentage of 5% of false negatives. The optimal level of identification of the sentinel lymphnode and the good level of predictivity about the axillary lymphatic status, that we have obtained, represent a further confirmation of the surety and validity of this method.

[Inguinal hernioplasty with PHS: our experience].

Three hundred patients underwent inguinal hernia repair from January 2003 to December 2004; 40 patients, randomly selected, received the PHS (Prolene Hernia System) mesh. Surgery was performed under local anesthesia in 80% of cases and under spinal anesthesia in the remaining 20%. There were 29 men and 11 women; mean age was 58,5 years (range 39-78). Postoperative pain was light and morbidity was low; all patients had early return to normal activities. Particularly, there were differences between the self-employed and the employees: the former began work again 2-4 days after surgery in 85% of cases, while the latter began after 15-20 days. Immediate complications were rare and always minor: they included seroma (1%) and ecchymosis (1%). Follow-up examinations did not show any recurrences.

[Treatment of pancreatic pseudocysts: personal experience].

The authors reported their experience of 13 patients with pancreatic pseudocyst observed within the period 1982-1987. Three patients underwent external drainage, 9 internal drainage and 1 exeretic operation. On the base of the results the authors conclude that the internal drainage should be considered the choice operation of pancreatic pseudocysts because of the lower mortality and morbidity.

[Ogilvie's syndrome].

Ogilvie's Syndrome is defined as acute distension of the colon without any mechanical obstruction. We reported two new cases who underwent surgical treatment and the revision of the literature. Four hundred eight-six cases are described until now. The physiopathology is unknown. The diagnosis is done with abdominal x-ray, barium enema must be used with attention. The more frequent symptoms are nausea, vomiting, diarrhea, and abdominal pain. The perforation of the colon is possible with a diameter of 9-12 cm. The mortality was 11.3% in two hundred ninety patients (59.6%) who underwent conservative treatment. In one hundred ninety-six patients who underwent surgical treatment the mortality was 28.5%. The treatment is conservative, medical or endoscopic. The best surgical treatment is the tube cecostomy.

Follow-up study in 402 patients after parietal cell vagotomy for duodenal ulcer.

In a recent six-year-period, a total of 402 patients underwent parietal cell vagotomy (PCV) for duodenal ulcer. An overall clinical assessment by Visik grading placed 328 (82%) in grade I, 56 (14%) in grade II, 12 (3%) in grade III and 4 (1%) with recurrent ulcer in grade IV. Results of the gastric secretory tests showed that the basal acid output (BAO) was reduced by 83.9% of the preoperative value, after 3 months, by 78.2% after one year, and by 65.3% and 66.4% after three and five years respectively. The maximal acid output (MAO) was reduced by 75.9% of the preoperative value after 3 months, by 45.3% after one year, by 40% and 42% after 3 and 5 years respectively. The radiological gastric emptying time showed no significant difference when compared to the preoperative value. The parietal cells showed significant ultrastructural changes six months after PCV, corresponding to maximum functional secretory depression. Two years after the operation, the parietal cells had regained their preoperative morphology.
