We report a case of a newborn with a congenital heart-block. Clinical picture began with bradycardia and heart failure from birth. The mother had a systemic lupus erythematosus in remission during pregnancy suffering a reactivation after delivery. This report deals about the greater incidence of congenital heart-block in the lupus-mother's newborns and we expose some etiopathogenic hypothesis. Also we point-out the easy clinical and electrocardiographic diagnosis that shows a third degree heart-block with atrioventricular dissociation and atrial frequency two or three times greater than ventricular. It's established the differential diagnosis among neonatal bradycardias and congenital heart-blocks. The treatment with isoproterenol was ineffective. A pacemaker was need.
The authors present a review of the epidemiology, pathology, diagnosis and treatment of candidiasis in the child. Their studies on the favoring factors in cutaneous forms as well as their experiences in pulmonary forms are emphasized.
AIM: To relate structural and functional findings in one adolescent dizygotic twin pair with prenatal alcohol exposure. METHOD: Neuropsychological and volumetric magnetic resonance studies were carried out on a 13-year-old preterm dizygotic twin pair with prenatal alcohol exposure. RESULTS: Neuropsychological and brain structural findings differed between the twins. The child with the more affected phenotype had large-scale cognitive deficits and presented significant atrophy in several brain structures. Both subjects had white matter volume reductions relative to the whole cerebral volume. CONCLUSION: The neuropsychological and neuroimaging data reflect long-term consequences of prenatal alcohol exposure.
The use of beta-adrenergic antagonists for the control of high blood pressure associated to pregnancy is frequent. Their use is related with the appearance of undesirable effects of the fetus. The case of neonatal cardiac arrest attributed, to the administration of labetalol to the mother is presented. The high transplacentary passage, the different pharmacokinetics of the drug in the newborn and the clinical evolution of the patient suggests its involvement. It is concluded that labetalol may cause severe undesirable effects in newborns and fetal heart rate of the mother and neonate should be monitored upon use of this drug.
