[Nephroblastomatosis and Wilms' tumor. Apropos of a case].

This case report illustrates the different anatomopathologic patterns and evolutive potentials of perilobar nephroblastomatosis described by Beckwith. The latter differs from those of an intralobar nephroblastomatosis. The evolutive possibilities of nephroblastomatosis underline the importance of imaging technology in the diagnosis and control of such lesions, as well as the role of chemotherapy and above all the caution required in the indications for surgery.

Recurrent nephrotic syndrome following renal transplantation in patients with focal glomerulosclerosis. A one-center study of plasma exchange effects.

Forty seven renal allografts performed over a period of 12 years in 43 recipients with chronic renal failure due to biopsy-proved focal glomerulosclerosis (FGS, 5.34% from 888 cadaveric renal transplantations) were reviewed. Recurrence of the disease was suspected in 14 grafts (29.8%) on the basis of immediate proteinuria, but recurrence of FGS lesions was demonstrated in only 7 patients. The remaining 7 patients had minimal-change nephropathy or mesangial hyperplasia. The duration of renal disease before transplantation was a clear predictive risk factor of FGS recurrence, and mesangial hyperplasia in the native kidneys was associated with 50% risk of FGS recurrence. Some reports have suggested that plasma exchange may be beneficial in the treatment of FGS recurrence. Our experience, in 9 patients, indicates that plasma exchange initiated early in the course of recurrent proteinuria leads to transient but significant disappearance (2 cases) or decrease (5 cases) of proteinuria, with a return to pre-plasma exchange levels within 2 weeks after the end of treatment. In 2 cases, there was no beneficial effect on proteinuria. Plasma exchange efficacy was correlated with proteinuria levels relative to disease severity. Although plasma exchange does not seem to improve the outcome of FGS recurrence, it demonstrates the possible presence of circulating factor(s) and argues for the characterization of humoral mediator(s).

[Emphysematous pyelonephritis and perinephritis].

The authors report an eighth case of emphysematous perinephritis and a new case of emphysematous pyelonephritis. The general features of these two diseases and their similarities are described: middle aged diabetic women, clinical picture of sepsis, gas images projected over the renal area with delayed excretion on intravenous pyelography, computed tomography reveals the exact site of the emphysematous images (perinephritic and or intraparenchymal). Emphysematous perinephritis may be an early form of emphysematous pyelonephritis. Surgical drainage of the renal compartment allows conservative management at this early stage.

[Linitis plastica extending through the entire digestive tract. A case report].

A 43-year-old man who presented a perplexing diagnostic challenge, had diffuse linitis plastica involving the entire gastrointestinal tract (autopsy). Although the usual primary site is the stomach in case of linitis plastica, invasion of the large and small bowel is rarely seen, but must be searched with proctoscopic exam and barium enema. We review the literature and discuss the pathogeny of this disorder.

[Nephrogenic metaplasia of the bladder in renal transplant patients].

Nephrogenic metaplasia of the bladder is rare; less than one hundred cases have been reported. Four personal cases and 16 cases reported in the literature concerning renal transplant patients demonstrate that the frequency of this disease is much higher in this group than in the general population. The symptoms and signs, age of onset, macroscopic and microscopic appearance of the lesions correspond to the description in non-transplant patients, apart from the preferential site adjacent to the reimplanted ureter and the presence, in two cases, of cells with virus-like inclusions, one of which corresponded to CMV infection. The traumatic factor in these patients is not sufficient on its own to explain the relative frequency of this lesion in renal transplant patients. The role of immunosuppression, resulting in a reduction of "immunological surveillance", is possible; this metaplasia usually has a benign course, but this high risk population must be carefully monitored for the development of this lesion.

[Tubulopapillary tumors of the kidney. Apropos of 20 new cases and a review of the literature].

Tubulo-papillary tumors constitute a particular group of renal tumor. A study of 20 cases treated personally and review of 130 cases reported in the literature showed: hypo or heterogenic ultrasound images, hypovascularization on angiography, particular density on CT scan image. Histologically they can be distinguished from clear cell epithelioma not only by their appearance (papillary and tubular structures, cystic and hemorrhagic changes) but also by the frequency of stage I on histology. The latter feature is not the only explanation for a more favorable prognosis: other significant factors are necrotic lesions and degree of anisocaryocytosis of epithelial proliferation.

[Prognosis of hemolytic uremic syndrome in children. Importance of extrarenal involvement].

The prognosis of hemolytic uremic syndrome was studied in 37 children hospitalized between January 1980 and January 1985. 75% of affected children were less than 3 years of age. Twenty-two (60%) presented with anuria or oliguria (mean duration of anuria: 10.7 days). In this last group, severe extrarenal manifestations occurred: 12 cases of neurological involvement (6 involving hypervolemia), gastrointestinal involvement in 5 cases, including a case of ileal necrosis; pancreatic involvement in another. Three children died during the first month of the disease (overall death-rate: 8%). Thirty-two children were followed with a mean 24 months follow-up (6 months to 60 months). Six (19%) presented with sequellae, of which 3 were severe: one severe arterial hypertension and two with chronic renal failure of which one terminal with severe neurologic sequellae. The existence of extrarenal manifestations implies a poor prognosis. Among the 22 children with anuria or oliguria, 8 had no extra-renal manifestations. All of these recovered. Three of 11 children with one extrarenal manifestation had an unfavorable outcome. The course was unfavorable in the 3 cases with multiple extrarenal involvement.

[Recurrent polychondritis with renal microaneurysms and segmental necrotizing glomerulonephritis].

The authors report a case of relapsing polychondritis, in a 41 year-old man, with renal complication. Immunofluorescence study of this segmental necrotizing glomerulonephritis showed an important fibrin fixation without immunoglobulins. Selective renal arteriography, preliminary to the biopsy, showed lobular arterial aneurysms. Because of other exceptional cases of literature, relation between periarteritis nodosa and relapsing polychondritis is discussed.

[Granulomatous hepatic localizations in Whipple's disease. Apropos of 2 cases].

In two patients "granulomatous hepatitis" was diagnosed upon examination of liver biopsy specimens taken because of prolonged fever with poor general condition. Observed lesions were small nodules without necrosis: follicular adentitis was also found in one patient. Diagnosis of Whipple disease was established by examination of a biopsy specimen of the intestinal mucosa. Study of the hepatic lesions after Gram and PAS staining, done in one patient, did not demonstrate the suggestive intrahistiocytic particles. Confrontation of clinical evidence and histologic findings in lymph node and intestinal biopsy specimens is required for diagnosis.

[Peroperative circulatory arrest caused by a tumoral pulmonary embolism following hepatectomy for tumor. Apropos of a case].

The authors report a case of pulmonary tumor embolism during a hepatectomy for hepatoma, leading to a fatal cardiac arrest. This cause of cardiac arrest during operation has never been reported in the literature, but systematic autopsies of patients dying from liver cancer, especially primary or metastatic tumors, shows that the incidence of these embolisms is frequently underestimated. Any anomaly of the hepatic vein appearing in the X-rays implies a danger of intra- or post-operative embolism. Allowance must therefore be made for this in the surgical technique, which should include complete or partial cross-clamping of the vena cava.

[Bronchopulmonary manifestations of Horton's disease. Apropos of a case].

A 78 years old woman developed a pulmonary recurrence four years after the diagnosis of temporal arteritis. The lung biopsy showed the same giant-cells granulomatous lesions with central vessels as the initial biopsy. The recurrence in giant cells-arteritis has already been published but the pulmonary involvement had never been illustrated before by histological data. The difficulties of diagnosis in case of granulomatous angiitis on lung samples is emphasized.

[Silicone deposits in macrophages of patients on chronic hemodialysis].

A retrospective study was carried out in search of silicone deposits in 11 periodic hemodialysis patients (1 liver biopsy, 4 splenectomies and 6 autopsies) treated between 12 and 60 months with equipment containing a silicone pump segment. Silicone particles were found in the macrophage cells of all patients, especially in the liver and spleen, but also in lymph nodes, bone marrow and even an artery wall. In some cases, macrophage response had formed sarcoid-like granulomas. Silicone deposits in 3 patients were associated with a hypersplenism syndrome totally or partially improved by splenectomy. No relationship could be established between foreign material in the liver and clinical or biological abnormalities. Three patients who had died subsequent to peritoneal dialysis or hemodialysis involving polyvinyl chloride materials served as controls. No silicone inclusions were found in their tissues at autopsy.

[Anatomo-clinical correlations in Berger's disease: evaluation of the prognosis by analysis of one hundred and one cases (author's transl)].

In order to evaluate the prognosis of IgA nephropathy semi-quantitative analysis of tubulo-interstitial and vascular alterations was performed. Three groups of patients are described. In the first group the prognosis has remained favourable until the present time. The prognosis is variable in the second group. In the last group the prognosis is always unfavourable. However, the limited duration of the study may elicit certain reservations concerning the long-term prognosis for the first and second groups.
