[Partial lipodystrophy (cephalothoracic type) with asymptomatic dense deposit disease (author's transl)].

We report a case of partial lipodystrophy (P.L.D.) of the cephalothoracic type associated with low C3 serum level. In spite of the absence of renal symptom a renal biopsy was performed which demonstrated dense deposits disease characterized by the lack of cellular proliferation and highly segmentary deposits similar to those observed in remission after renal transplantation for these disease. A review of the literature shows the frequent occurrence of this pathological association. In general P.L.D. and complement abnormalities are thought to precede nephropathy. However chronological relationship between the former and the later must be interpreted cautiously as our case shows that dense deposits disease can be associated with P.L.D. in the lack of clinical evidence of renal disorder.

[Vascular lesions of Fabry's disease. Optical, histochemical and ultrastructural studies].

On the basis of a new case of Fabry's disease and the various cases published in the literature, the authors analyse the vascular lesions of this angiodysplasia related to an error in lipid metabolism. They emphasise the involvement of lymph vessels (in particular those of the small bowel), and the ultrastructural characteristics and the mode of formation of deposits within the endothelial cells of the vascular walls.

[Compensatory hypertrophy of the kidney. I. - Search for the existence of a renal inhibitory substance (author's transl)].

In view of the theory of autoregulation of cell multiplication in a growth organ, we attempted to inhibit renal tubular cell hyperplasia in the remaining kidney 48 hours after controlateral nephrectomy. To do this we drew up a method of measurement of hyperplasia by measuring the rate of uptake of tritiated thymidine in the organ undergoing compensatory growth. We then injected renal cytosol. Whereas the rate of uptake of tritiated thymidine doubles in the growing kidney compared with control animals, the intravenous administration of renal cytosol reduces the renal uptake of the tracer to its basal level. We correlated this observation with the microscopic count of mitotic figures, and noted the existence of parallel fluctuations. Our results seem to support the initial working hypothesis but they do not exclude the possibility of a more complex regulatory mechanism in which serum stimulant factors may also play a role.

[Familial cancer of the parathyroid glands. Importance of angiography in the diagnosis of regional recurrences. Considerations on 2 cases].

The authors report two new cases of functional parathyroid carcinoma, the course of which varied between 5 and 7 years. The main fact was their familial character, as the disease affected two siblings. No similar case was found in the world literature. These two cases were characterised by symptoms in the bones or pancreatic symptoms which dominated the whole clinical course. Finally, the second case was studied by angiography which permitted the authors to localise a lymphnode metastasis on the opposite side. As far as treatment was concerned, the frequency and severity of local and regional spread justifies wide removal at an early stage. This attitude is necessary in any case of parathyroid tumor with an infiltrating character.

[Temporal artery syndrome in amyloidosis AL associated with myeloma].

The authors report a case of amyloidosis AL associated with light lambda chain myeloma, mimicking giant cell temporal arteritis. The normality of inflammatory proteins, the primary resistance to corcicosteroids and the results of temporal artery biopsy (amyloid deposits without giant cell granuloma) excluded a diagnosis of giant cell arteritis associated with amyloidosis. The most probable mechanism was infiltration of branches of the external carotid artery. In patients with temporal arteritis, amyloidosis must be suspected and sought, particularly in case of monoclonal dysglobulinaemia or manifestations that are not typical of giant cell temporal arteritis. The prognosis of vascular amyloidosis seems to be more favourable than that of the classical visceral types. The primary amyloidosis of these patients must be treated with melphalan combined with prednisone and colchicine.
