Archives of Medical Research: an historical and subject coverage overview.

A bibliometric study about the subject content of the articles published in the Mexican scientific journal Archives of Medical Research is reported. The journal, published by the Mexican Institute of Social Security (IMSS), is comprised of 100 regular issues and 12 special supplements giving a total amount of 1,424 reports on medical research performed in Mexico during the last 25 years. According to the type of studies published during this period, we found that there is a similar percent of biomedical and clinical reports in the journal (47 and 42%, respectively) and a low proportion of epidemiological and medical educational reports (8 and 3%, respectively). Six thematic areas of research have been permanently published in this journal: investigations on infectious and neurological diseases being the areas mainly represented (34% of the total, corresponding to 17% in each area), followed by studies in reproductive biology (10%) and endocrine (7%), oncological (5%) and cardiovascular (3%) diseases. The tendency of the subjects covered by the journal during this period shows an increment in reports on infectious and parasitic disorders together with an increase in publications related to medicinal plant pharmacology; reproductive biology and endocrine studies show also an increasing tendency. On the other hand, a moderate decrease in studies related to neurological, oncological and cardiovascular diseases is observed. The origin of contributions during the last five years has balanced the proportion of papers published from IMSS scientists, other Mexican biomedical researchers and foreign contributions, thus reflecting favorably the recent changes in the journal's policies. This journal represents a clear example of a scientific publication edited in a developing country, originating as a national publication that evolved progressively into an international biomedical journal.

[Striatocapsular infarct in a young patient with heterozygous familial hypercholesterolemia and Klinefelter's syndrome].

Familial hyperlipidemia has received little attention as a possible cause of stroke in young patients. Some recent studies have demonstrated that lipoprotein (a) is a key factor for atherogenesis in familial hypercholesterolemia. Hypogonadism may also contribute to the elevation of serum lipids, but their influence as a risk factor for stroke is still less understood. A 34-year-old patient with heterozygous familial hypercholesterolemia presented with a left pure motor hemiparesis secondary to a right striatocapsular infarction. Arteriography showed atherosclerotic lesions in both internal carotid arteries. High levels of cholesterol, cLDL, apo B, and lipoprotein (a) were found. Clinical signs of hypogonadism were present and the karyotype led to the diagnosis of Klinefelter's syndrome (47,XXY). The early clinical course was excellent, and the levels of serum lipids were normalized with diet, lipid-lowering drugs and androgens. The importance of hyperlipidemia as a risk factor for stroke in the young, specially when it occurs in the context of familial hypercholesterolemia with elevated lipoprotein (a) levels, as well as the possible contribution of hypogonadism to the development of accelerated atherosclerosis in young patients, are discussed upon.

Immunohistochemical location of prothymosin alpha in regenerating human hepatocytes and hepatocellular carcinomas.

In the present paper we analysed the presence of prothymosin alpha (ProT) in human liver. In normal liver, ProT immunostaining was found in the nuclei of bile duct cells, but not in the hepatocytes. In contrast an intense immunoreactivity was observed in regenerative hepatocytes of chronic hepatitis, cirrhosis and in hepatocellular carcinomas. In all cases the immunostaining was restricted to the nuclei, but the nucleoli were always negative. Similar results were obtained for proliferating cell nuclear antigen. These findings confirm that ProT is related to cell proliferation and provides a new immunohistochemical proliferation marker for routinely processed samples.

Effects of free fatty acids on somatostatin secretion, content and mRNA levels in cortical and hypothalamic fetal rat neurones in monolayer culture.

The effects of free fatty acids on somatostatin secretion, content and mRNA levels in fetal rat hypothalamic and cortical cell cultures were investigated. Somatostatin secretion and content were quantified by radioimmunoassay. Somatostatin mRNA levels were measured by Northern blot hybridization using a cRNA probe. Treatment with either caprylic acid (5 x 10(-3) M) or oleic acid (5 x 10(-5) M) for 90 min inhibited basal somatostatin secretion in both hypothalamic and cortical cell cultures. In addition, the increase in somatostatin secretion induced by incubation with veratridine (10(-4) M) or carbachol (10(-4) M) for 90 min was significantly reduced by the addition of caprylic acid, but somatostatin release stimulated by 5.6 x 10(-2) M KCl was not affected. Furthermore, treatment with these free fatty acids for 90 min markedly decreased somatostatin mRNA levels in both types of neurone culture. These inhibitory effects were transient, being observed after 90 min, but not after 5 h. These results support the probability that there is a role for free fatty acids in the regulation of somatostatin mRNA levels and somatostatin secretion in both hypothalamic and cortical cell cultures.

[Generalized lymphadenopathy with morphologic findings of multicentric angiofollicular ganglionic hyperplasia in a patient with AIDS].

We report a case of a 33-year-old man, intravenous drugs abuser, HIV-positive, with peripheral lymphadenopathy, hepato-splenomegaly and fever, in which a ganglionic biopsy showed a histology with morphologic features of multicentric Castleman's-like disease, and minute foci of Kaposi's sarcoma ganglion, without cutaneous lesions. Given the interrelationships between this morphology of angiofollicular lymph node hyperplasia, the development of Kaposi's Sarcoma, and the aggressive clinical course seen in our patient and those in the literature, the use of lymph node biopsy may be an important prognostic tool for the patients with the acquired immunodeficiency syndrome.

Diagnosis of subacute ventricular wall rupture after acute myocardial infarction: sensitivity and specificity of clinical, hemodynamic and echocardiographic criteria.

When ventricular free wall rupture after acute myocardial infarction is not followed by sudden death, it is referred to as subacute ventricular rupture. The sensitivity and specificity of clinical, hemodynamic and echocardiographic diagnostic variables obtained at bedside are unknown and were therefore prospectively studied in 1,247 consecutive patients with acute myocardial infarction including 33 patients with subacute ventricular rupture diagnosed at operation (group A) and 1,214 patients without ventricular rupture (at operation, postmortem study or at discharge) (group B). The incidence of syncope, recurrent chest pain, hypotension, electromechanical dissociation, cardiac tamponade, pericardial effusion, high acoustic intrapericardial echoes, right atrial and right ventricular wall compression identified in two-dimensional echocardiograms and hemopericardium demonstrated during pericardiocentesis was higher in group A than in group B (p less than 0.00001). The presence of cardiac tamponade, pericardial effusion greater than 5 mm, high density intrapericardial echoes or right atrial or right ventricular wall compression had a high diagnostic sensitivity (greater than or equal to 70%) and specificity (greater than 90%). The number of false positive diagnoses was always high for each diagnostic variable alone (greater than 20%), but the combination of clinical (hypotension), hemodynamic (cardiac tamponade) and echocardiographic variables allowed a sensitivity of greater than or equal to 65% with a small number of false positive diagnoses (less than 10%) and provided useful information for therapeutic decisions. The diagnosis of subacute ventricular rupture requires a surgical decision. Twenty-five (76%) of the 33 patients with subacute ventricular rupture survived the surgical procedure and 16 (48.5%) are long-term survivors. Thus, subacute ventricular wall rupture is a relatively frequent complication after acute myocardial infarction that can be accurately diagnosed and successfully treated.

Prothymosin alpha expression in lymph nodes and tonsils: an optical and ultrastructural study.

Prothymosin alpha (ProT alpha) distribution in human and rat lymph nodes and human tonsils was studied by means of immunohistochemical methods, using specific antibodies raised against thymosin alpha 1. We observed ProT alpha immunoreactivity in lymphoid cells of the germinal centers both in humans and rats. In human tonsils, positive cells were also seen in the basal layer of the surface epithelium. These results support the hypothesis that ProT alpha expression is restricted to actively proliferating cells. Immunoelectron microscopy revealed that ProT alpha was located in the nucleus, mainly in the border between euchromatin and heterochromatin.

[Left atrial thrombus. Its evolution with oral anticoagulation].

UNLABELLED: Left ventricular thrombi were found in 25 patients by two-dimensional echocardiography. All patients were in atrial fibrillation, 16 had mitral or mitroaortic valve diseases and nine mitral or mitroaortic bioprosthetic valves. Nine patients (36%) had history of arterial embolism. At diagnostic time, oral anticoagulation with coumadin was instaured in all the patients. Periodic clinic and echocardiography follow-up was performed. Left atrial thrombi either disappeared (18 patients [72%]) or reduced their size (one patient). Six patients with mitral stenosis were considered as candidates to percutaneous mitral valvuloplasty, which was performed in 4 patients whose thrombi disappeared with anticoagulation therapy in 6 months. During the follow-up one patient had cerebral embolism without sequelae. CONCLUSIONS: 1st. Patients with left atrial thrombi have high risk for arterial embolism. 2nd. Left atrial thrombi disappear in a high proportion after prescribing oral anticoagulation, which has some important therapeutic implications.

Prothymosin alpha expression is associated to cell division in rat testis.

Using immunohistochemical methods, we have investigated the cellular distribution of prothymosin alpha (ProT) in adult rat testis. A policlonal antibody raised against thymosin alpha 1 conjugated to keyhole limpet hemocyanin was used. ProT immunoreactivity was observed in the cytoplasm and nucleus of spermatogonia and primary spermatocytes in initial phases of the first meiotic division, preleptotene, leptotene and zygotene. However, in pachytene phase they already showed a weak or negative staining. On the other hand, secondary spermatocytes, spermatids, spermatozoa and Sertoli cells were not stained. Based on this fact we suggest that ProT is present in the proliferative cycle in the final steps of G1 phase, throughout the S and G2 phases and in initial steps of the prophase.

[Right-sided infectious endocarditis. Experience with a series of 35 patients].

From 1978, 35 patients with right-sided endocarditis were treated at our hospital. There were 25 male and 10 female patients, with ages ranging from 14 to 77 years. The cause was intravenous drug abuse in 27 cases. Positive blood cultures were obtained in 29 cases, isolating staphylococcal organisms in 26 of them. Two-dimensional echocardiography was performed in 30 patients, confirming the diagnosis in 27 of them (90%). Vegetations were found in 25 patients and perivalvular abscess was seen in 4 patients. The management was medical only in 32 patients. Three patients were operated on because of failure to control pyrexia and heart failure, performing total tricuspid valvectomy in two, and only partial in the third one. All the 3 patients had perivalvular tricuspid abscess. There were 2 hospital deaths (5.7%), because of septic shock in drug abusers, one of them after a tricuspid valvectomy. In our experience, right-sided endocarditis is mainly associated with drug abusers and staphylococcal organisms. Two-dimensional echocardiography plays an important role in the diagnosis of this entity. In our series the size of vegetations does not play a role in selection of surgical candidates. The presence of perivalvular abscess was the only predictive factor for surgery. Only a few patients do not respond to medical therapy. In those cases the elective surgical procedure in our criterion and in our patients is tricuspid valvectomy without valve replacement, or if possible, the use of reconstructive or reparative approaches.
