[Teratoma of the stomach in a 4-day-old newborn infant].

Teratoma of the stomach is a very rare tumor. Nearly all previously reported patients were males. We report a 4 day-old male infant, who presented with a palpable abdominal mass. A preliminary diagnosis of teratoma was made by abdominal x-ray, sonography and MR imaging. Surgery was performed and diagnosis of teratoma of the stomach was confirmed by histological examination of the tumor. Because teratoma of the stomach is always a benign neoplasm surgery is curative and no further therapy is needed.

[MR study of bone marrow changes of the spine following radiotherapy].

MR examinations of bone marrow variations in the spine after radiotherapy were performed on 24 patients in the thoracic and lumbar vertebral column. The actinically affected bone marrow showed a characteristic increase of signal intensity in T1-weighted sequences in the sagittal plane, due to conversion of red marrow to fatty marrow. The dose in the well-defined radiation areas was between 28 and 70 Gray (Gy). The lowest dose, applied to the bone-marrow bordering on the defined radiation areas, where we still could find an increase of signal intensity, was below 2.6 to 5 Gy. MR imaging was performed between 6 and 9 month after radiotherapy.

Magnetic resonance imaging in children with acute hip pain.

45 children presenting with acute hip pain were prospectively evaluated with conventional radiography, radioisotope bone scan, and magnetic resonance imaging (MRI). The final diagnoses were transient synovitis (n = 17), septic arthritis (n = 2), Legg-Calve-Perthes disease (LCPD, n = 13), epiphyseal dysplasia (n = 2), other conditions (n = 4), and normal findings (n = 7). In the work-up MRI provided more morphologic information than other techniques and enlarged the diagnostic possibilities. It was the only imaging technique able to give an early indication of bone marrow involvement in systemic diseases. For the early diagnosis of LCPD, MRI was as sensitive as isotope bone scan and more precise than conventional radiography. In the follow-up of LCPD patients MRI was not able to indicate the start of revascularisation of the necrotic area, which bone scans showed reliably in six patients: but MRI provided excellent evaluation of the position, form and size of the femoral head and the surrounding soft tissues.

Magnetic resonance imaging in cervical cancer: a basis for objective classification.

Conventional clinical staging of cervical cancer is subjective because it is based on palpatory findings and inadequate because it cannot assess the single most important prognostic factor--tumor size. To determine the exactitude of in vivo MRI measurements of tumor volume, 22 patients with invasive cervical cancer were studied before surgery. The volumes obtained by MRI correlated well (r = 0.983) with those obtained by histomorphometric analysis of the surgical specimens, but only weakly with clinical stage. MRI may provide a basis for precise classification of cervical cancer and for objective comparison of surgery and radiotherapy.

Abdominal metastases of an ovarian cancer demonstrated on bone imaging.

Tc-99m HMDP uptake by abdominal metastases of an ovarian cancer was seen on a bone image. The findings were confirmed by computed tomography and by biopsy at surgery. The psammoma bodies throughout the tumor tissue appeared to be the substrate of this unusual extraosseous tracer uptake.

MR findings in methotrexate-induced CNS abnormalities.

MR of the brain was performed in eight patients (mean age, 14.9 years) with osteogenic sarcoma during or after IV treatment with high-dose methotrexate. MR detected brain abnormalities in four patients, three of whom had concomitant neurologic dysfunction. Pathologic findings demonstrated on MR were (1) chronic brain edema, demonstrable over a period of 3-14 months (proved by autopsy in one patient); (2) multifocal white matter necrosis; and (3) deep brain atrophy. MR appears to be valuable in the detection of abnormalities induced by treatment with high-dose methotrexate.

[Indications for MIBG scintigraphy in the diagnosis of neuroblastoma].

The significance and indications of MIBG scintigraphy are critically assessed. The results are compared with the results of whole-body bone scintigraphy, computed tomography (CT) and magnetic resonance tomography (MRT), and are related to values of catecholamine metabolites in 24-h urines. In our patients (10 histologically proven cases) MIBG scintigraphy turned out to be most useful in tumor follow-up. In contrast, the significance was much lower in primary tumor diagnosis and tumor staging as the exact primary diagnosis was established by other means such as CT, MRT, MDP whole-body scan, urine chemistry and bone marrow biopsy in all cases. MIBG scintigraphy in diagnostic imaging of neuroblastoma is an additive diagnostic tool and is called for in (1) tumour follow-up (progress, recurrencies, metastases); (2) primary diagnosis if the primary tumour has not been localized by means of CT or MRT; and (3) tumour staging to differentiate stage IV disease from lower stages as long as stage IV disease has not been established by bone-marrow biopsy or MDP whole-body scan.

Hemodynamic effects of nifedipine and oxygen in children with pulmonary hypertension.

Fourteen patients, 2 to 20 years old were investigated. Two had primary pulmonary hypertension, 11 had congenital heart disease and post-tricuspid shunts, and 1, a 20-year-old patient, was investigated after he had undergone surgical correction of truncus arteriosus I. Pulmonary arterial pressure, pulmonary flow index, peripheral systolic blood pressure and heart rate were measured before, and several times after intrapulmonary injection into the pulmonary artery of 0.5 microgram nifedipine/kg. Six patients were given an additional dose of 1 microgram nifedipine per kilogram into the pulmonary artery and hemodynamic measurements were repeated. In eight children, receiving 100% oxygen via a breathing mask, nifedipine effects were compared with oxygen effects. After 10 minutes under oxygen, the same hemodynamics were determined as after nifedipine. In addition, in four of these children aortic pressure and arterial oxygen saturation were also measured. Maximal effects occurred within 4 minutes. 0.5 micrograms nifedipine per kilogram caused a slight reduction in mean pulmonary arterial pressure (p less than 0.05), as well as increase in pulmonary flow index (p less than 0.005). However, no significant change in heart rate or in systolic blood pressure was observed. 1 microgram nifedipine per kilogram IP had almost the same effects. No adverse side effects occurred, besides mild headaches in one child. A comparison of nifedipine injected into the pulmonary artery with oxygen breathing in congenital heart disease combined with pulmonary hypertension, is reported for the first time. Nifedipine had a more pronounced and beneficial effect with a selective action on the pulmonary vascular bed.

Magnetic resonance imaging in clinical cervical cancer: pretherapeutic tumour volumetry.

MRI can define the spread, size, and volume of clinical cervical cancers. Appropriate pulse sequences and slice thicknesses are necessary. Twenty-five patients underwent MRI tumour volumetry before radical hysterectomy. The volume obtained by MRI was compared with that obtained from the histological giant sections; the volumes agreed at a statistically significant correlation coefficient of 0.983. The volumes obtained by MRI of 13 unfixed surgical specimens correlated with their histological volumes with a statistically significant coefficient of 0.894. Tumour volumes were compared with the respective clinical stages. Clinical stage did not correlate with tumour volume. Three very large tumours were in clinical Stage Ib. Tumour size is a major prognostic factor, can be measured easily, and, as the basis for classification, is superior to FIGO staging. MRI can measure tumour volume before treatment.

[Differentiation of cicatricial fibrosis and recurrence of gynecological tumors by MRI].

In a retrospective analysis the relative signal intensities in recurrent tumours of gynaecological origin (n = 16) and in areas of pelvic fibrosis (n = 24) were evaluated. MRI was able to assess biopsy-proven recurrences in 15/16 patients. In one female MRI was false-negative (submucosal tumour spreading), in another case false-positive (reactive lymphnode hyperplasia). Early fibroses (1-12 months following tumour treatment) were of higher signal intensity that late fibroses (time interval since treatment more than 12 months). Differentiation of recurrent disease from post-treatment fibrosis was optimized by long TR (2500 msec), long TE (80-100 msec) spinechopulse sequences.

Diffusion characteristics dissociate ouabain binding from inotropic effect in guinea-pig myocardium.

1. [3H]-ouabain uptake in resting guinea-pig papillary muscles depended directly on incubation time and inversely with muscle radius. The equivalence of both parameters support the relevance of diffusion. A particular mechanism of receptor-controlled diffusion was implicated by the saturation of initial rates of uptake with increasing ouabain concentrations. Saturation of initial uptake indicates an inhomogeneity of receptor occupancy with ouabain-equilibrated receptors in the superficial areas of the preparation and free receptors in the muscle core. 2. For comparison [3H]-ouabain diffusion was evaluated in a non-cellular preparation i.e. glass fibre filters soaked with modified Krebs-Henseleit solution. Diffusion was approximately 3 orders of magnitude faster when compared with the papillary muscles. 3. The time course of the inotropic effect of ouabain on rested-state contractions dissociated from the tissue content of ouabain with an increased steroid concentration. This dissociation appears to be associated with the inhomogeneity of receptor occupancy, since equal amounts of the steroid correspond to different concentration profiles in the tissue when the bath concentration is changed. Functional coupling between different muscle areas may have modulated the influence of inhomogeneous receptor occupancy.

Tumor recurrence versus fibrosis in the female pelvis: differentiation with MR imaging at 1.5 T.

Twenty-two women with previous malignancies of the pelvis were examined with magnetic resonance (MR) imaging. In 21 of 22 patients, the MR imaging findings were confirmed with laparotomy on transvaginal biopsy. Twelve of the 22 patients had recurrent tumors; ten had a localized fibrotic mass, and two were found to have coexistent local fibrotic masses and distant recurrence. In 11 of the 12 patients with recurrence, MR imaging demonstrated the recurrent tumor as an area of increased signal intensity on long repetition time, long echo time spin-echo pulse sequences. Signal intensity of localized fibrosis varied with the time since initial treatment. Separate signal intensity measurements for T1- and T2-weighted images were obtained in regions of interest in recurrent tumors, early fibrosis (1-6 months after first treatment), and late fibrosis (more than 12 months). Ratios of signal intensity of muscle to that of fat were calculated, and a statistical analysis (Student t test) was performed. On heavily T2-weighted pulse sequences, the differences in signal intensity between late fibrosis and recurrent tumors were statistically significant (P less than .001). MR imaging is useful in identifying recurrent pelvic neoplasm and distinguishing it from posttreatment fibrosis.
