[Continuous EEG monitoring in carotid surgery].

From March 1980 to March 1987, at the Department of Vascular Surgery of the University of Padua, 217 patients underwent 252 carotid revascularizations with routine use of an intraluminal shunt (IS) for symptomatic (70%) or asymptomatic (30%) internal carotid artery (ICA) atherosclerotic stenosis. All carotid endarterectomies (CEs) were routinely performed with patch graft angioplasty to prevent restenosis. In the immediate post-operative period, no patient presented permanent or transient neurological deficits; no patient died. In the early post-operative course there was a 0.9% (2 patients) stroke rate and 0.4% (1 patient) mortality rate. These results suggest that the selective use of the IS on the basis of EEG changes is able to reduce the perioperative morbidity and mortality to nearly zero.

An unusual kind of muscular and skeletal involvement in a heroin addict. A case report.

After a brief review of the infectious complications that frequently occur in heroin users, the authors report a case of chronic osteomyelitis affecting the ulnar diaphysis bilaterally. Musculo-cutaneous dystrophic lesions were also present and resulted in forced flexion of the distal phalanges of fingers 2-5. The most likely pathogenetic development discussed.

[Congenital tricuspid insufficiency due to valvular dysplasia].

A case of congenital tricuspid incompetence in a 39 year old woman is reported. The patient, who was in suffering congestive heart failure, had no history of trauma, myocardial infarction, rheumatic fever, infective endocarditis. Right ventriculography and right atrial pressure recording both showed signs of severe tricuspid insufficiency. Echocardiography showed dilatation of the right ventricle and atrium, and the lack of normal systolic coaptation of anterior and septal tricuspid leaflets, while contrast echocardiography gave direct evidence of regurgitation. Valvular replacement with a bioprosthesis was carried out. The diagnosis of congenital tricuspid incompetence was confirmed at operation. The patient is asymptomatic at one year follow up.

The mechanical properties of "cement-antibiotic" mixtures.

The authors investigated the mechanical properties of "CMW cement-antibiotic" mixtures compared with those of the standard preparation. They carried out comparative tests of tensile strength, hardness and flexion (flexion breaking load, percentage flexion, modulus of elasticity to flexion). The antibiotics used were: streptomycin, ampicillin, cefalotin, carbenicillin, rifampicin, kanamycin, gentamycin and tobramycin. The mechanical tests performed showed that the percentage reductions in the strength of the mixtures varied considerably depending on the type of antibiotic used. The reductions produced by antibiotics used in aqueous solution (gentamycin and tobramycin) were excessive. The results obtained with the other mixtures were much better; in particular, some of the values obtained with the CMW cement-cefalotin and CMW cement-streptomycin mixtures were very close to those for the standard preparation. The combination of rifampicin with CMW cement, on the other hand, prevented complete polymerization of the cement.
