[Desmoid fibroma of bone. An histoenzymological and ultrastructural study of the mandible in one case (author's transl)].

A case of post-traumatic desmoid tumor of the mandible is reported in a 4-year old girl. This tumor was considered typical clinically and by light microscopy. Four years later, the patient was symptomless, without evidence of recurrence. Histoenzymology showed a high level of leucine aminopeptidase activity. On electron microscopy, a part from a few primitive mesenchymal cells, abnormal fibroblasts represented the principal cellular component. These cells showed evidence of intracytoplasmic collagen fibers synthesis and of myoid differentiation (numerous filaments and endocytosis vacuoles). Myofibroblasts, with a microfibrillary network showing areas of dense osmiophilic condensation in the vicinity of cell membrane were also present. This atypical ultrastructural feature suggests, in this case, the non-neoplastic nature of this lesion. It seems to reflect an abnormal reaction to a traumatic aggression, characterized by proliferation of myofibroblasts (which normally disappear in the late stages of healing) and to have, thus, the signification of pathologic scar.

[Intraoral minor salivary gland tumors. Report of 296 cases (author's transl)].

296 intraoral minor salivary gland tumors were observed among 702 salivary tumors. The study emphasizes a frequent palatine location, observed in more than 50 per cent of theses tumors. Pleiomorphic adenomas (or so called mixed tumors), are frequently encountered; but it is noteworthy that the proportion of muco-epidermoid tumors or adenoid cystic carcinoma is higher among theses tumors than among major salivary glands; this fact explain a poor overall prognosis of minor salivary gland tumors.

Adenoma

[Inflammatory granuloma of the face with a malignant course. Atypical Hodgkin's disease (author's transl)].

A case of Hodgkin's disease beginning in the skin of the face with local lymph node involvement is reported. This was a lymphogranulomatous form without characteristic Reed-Sternberg cells in skin biopsy specimens, rendering the diagnosis difficult. There was a combination of granulomatous inflammation, plaques of necrosis and lesions of fibrinoid necrosis in the vessel walls. This form entered the classification of clinical Stage IV, with its associated serious prognosis.

[Aneurysmal cysts located in the maxilla or mandible (author's transl)].

Aneurysmal cysts of the mandible and maxilla are rare conditions, though not exceptional, which are seen in particular in the child and adolescent. A part from facial asymmetry and tumefaction, it is rare to find other clinical manifestations. No neurological signs were seen in the present study. An aneurysmal cyst gives a radiological appearance of non-characteristic lysis compatible with a large range of diagnoses. Histological study permits early diagnosis but must always involve the whole specimen with careful examination to detect any other associated lesion. The pathogenesis of the condition remains obscure and, at the extreme, it may be questioned whether the aneurysmal cyst is a separate entity or a particular form of chronic osteopathy.

Adolescent

[Chromatography of urinary amino-acid in bismuth encephalopathy (author's transl)].

The urinary excretion of amino-acid have been determined by ionic exchange chromatography for 10 cases of bismuth encephalopathy and for 15 normal controls. The ninhydrine positive constituant reported by Burns in 5 cases of encephalopathy has not been detected. We are thinking it may be related to the use of a rich amino-acid solution during perfusions. We did not find any significant difference between the urinary excretion of amino-acid average value to the two groups. We found an increase excretion when severe encephalopathy were observed. Separate analysis of individual amino-acid excretion showed the following results: i) average percent of tyrosine excretion is lower in controls as in encephalopathies; ii) among the different ratios tested only SER + THR)/TYR and VAL/TYR are different in the two groups.

[Surgical treatment of loss of substance of the lower lip].

Repair of loss of substance of the lower lip has been the object of numerous studies. Each author has used his own ingenuity to describe a new type of flap aimed at restoring a particular form of loss of substance. The techniques suggested here are capable of solving a majority of problems on the basis of simple and original solutions. These flaps make it possible to resolve the problems posed by most cases of loss of substance of the lower lip. The choice of these plastic procedures permits restoration which is at one and the same time functional and anesthetic.

Dermatologic Surgical Procedures

[Functional anatomy and terminal tubules of Stensen's duct].

The dynamic anatomy of STENSEN's duct is somewhat different from what is usually accepted. GUERRIER, COURBIER and FRANCHEBOIS had indeed admitted that the buccinator msucle acted as a passive sphincter, without however mentioning the presence of terminal siphons and canalar valvules and without drawing particular attention to their dynamic relationship. This duct should not be considered inert, as part of a theoretical bayonet-like pathway which is more topographical than functional: the buccinator muscle and STENSEN's duct with its valvules and terminal siphons should be considered together as forming the real salivation apparatus.
