[Metastatic bile duct cancer in secondary biliary cirrhosis associated with primary sclerosing cholangitis in ulcerative colitis].

By means of a case-report of a 33 years old pat., having colitis ulcerosa for 16 years with serious epitheldysplasia and developing primary sclerosing cholangitis, the typical progress of the disease with manifestation of a secondary biliary cirrhosis is shown. The course was complicated by the development of a bile-duct carcinoma. Present possibilities of diagnosis and therapy are discussed.

[Metastasizing malignant melanoma of the vagina].

We report on a 54-year-old patient with multiple cutaneous, lung and liver metastases. The primary tumour is a malignant melanoma of the vagina, who developed within one year after the last gynaecological examination. Our case report is another example for the poor prognosis of this rare vaginal tumour.

Carcinogenicity studies on natural and man-made fibres with the intraperitoneal test in rats.

Female Wistar rats were injected intraperitoneally (i.p.) with a suspension of 11 fibrous and 3 granular dusts. A dose of 0.25 mg actinolite or UICC chrysotile induced tumours of the peritoneum in more than 50% of the animals. Even 0.05 and 0.01 mg proved to be carcinogenic, although no adhesions of the abdominal organs could be observed. The findings are in conflict with the hypothesis that a scar is always the morphological precondition for the development of an asbestos-induced tumour. Actinolite injected i.p. in a solution of polyvinylpyridine-N-oxide gave a lower tumour incidence than when suspended only in saline, possibly due to inactivation of the fibre surface. Persistent glass fibres were less effective than actinolite having a similar fibre size distribution. On the other hand, relatively thick basalt fibres and ceramic fibres gave higher tumour incidences than expected. Wollastonite fibres were not carcinogenic, probably because of their low durability. Large amounts of polyvinylchloride, alpha-ferric oxide hydrate and wood dust also led only to adhesions of the abdominal organs and fibrosis; a definite carcinogenic effect was not detected.

Carcinogenicity studies on fibres, metal compounds, and some other dusts in rats.

About 50 dusts were examined on their carcinogenicity in rats mainly after intraperitoneal injection and some after intratracheal instillation. In the i.p. test, very low doses between 0.05 and 0.5 mg asbestos led to tumour incidences of about 20 to 80%. Polyvinyl-pyridine-N-oxide prolonged the tumour latency after injection of actinolite. 60 mg attapulgite from three sources with short fibre lengths were not shown to be carcinogenic but an attapulgite sample with longer fibres had a moderate effect. Relatively thick rock and ceramic fibres (median greater than 1 micron) induced tumours, but slag and wollastonite fibres did not, probably because of their better solubility. Intratracheal instillations of glass microfibres (20 X 0.5 mg) led to lung tumours in 5 of 34 rats (0 in control). The carcinogenic potency of an inorganic fibre depends on its size and persistency, and possibly also on other properties, especially on the surface. Nickel powder, nickel oxide, nickel subsulfide and cadmium sulfide were all found to be carcinogenic in the two tests. Cadmium chloride and cadmium oxide could only be administered in very low doses because of their high acute toxicity. A high amount of magnetite (15 X 15 mg i.tr.) led to an unexpected lung tumour incidence of 69%. The i.p. test in rats proved to be very sensitive for detecting the carcinogenic potency of non-acute toxic natural and man-made mineral dusts as well as metal compounds. This means that, if a high dose of one of these dusts does not induce tumours in this test, no suspicion of carcinogenic potency can be substantiated.

[Synovitis in familial Mediterranean fever].

Arthroscopy done on a 24-year-old turkish male with familial Mediterranean fever (FMF) and arthritis of the knee joint provided morphological data during the acute stage of FMF-arthritis. Main finding is a heavy granulocytic infiltration of the subsynovial stratum, similar to that seen in non-specific purulent inflammation, accompanied by marked ectasis and hyperaemia of the synovial vessels. In the microbiologically sterile synovial fluid cell counts and lactate values are found as in bacterial arthritis. The typical history and the characteristic course of the disease are indicators for the diagnosis. Serological, immunological and radiological findings are non-specific. The efficacy of prophylactic colchicine in symptomatic therapy could be verified but it is essential that the drug is taken regularly and that the patient is instructed accordingly. Investigation of other members of the family showed a high frequency of intermarriages and the presence of this autosomal-recessive inherited disease in three generations.

Adult

[Comparative morphology of lymphoepithelial carcinoma in primary tumor, metastasis, and xenograft (author's transl)].

Tissue of lymphoepithelial tumor metastasis was transplanted subcutaneously to thymus-aplastic "nude" mice. After 75 days a pea-size tumor was developing which again was transplanted to further murine passages. During the second passage the tumor was growing faster and material was taken for light and electron microscopy. The results were compared with the datas obtained from the primary nasopharyngeal carcinoma and metastasis. The tumor cells of the metastasis and of the xenograft showed all morphological criteria of a non-keratinizing squamous epithelial carcinoma of undifferentiated type. The ultrastructural criteria of the human NPC-tumor cells were all present including cytoplasmic deposits of Corona viruses and Herpes virus-like particles within the nuclei and the cytoplasm.

["Plica disease" (synovial folds) of the knee-joint: arthroscopic and histological findings, with suggestions for treatment (author's transl)].

A mediopatellar plica (synovial fold) of the knee-joint may develop without recognisable cause in adolescents or young adults, predominantly females. It leads to pain on pressure over the medial knee compartment, sudden or "springing" intraarticular movements and pseudolocking of the joint. Similar plicae occur after traumatic joint contusion, with meniscus disease, or more rarely with arthrosis deformans. Histologically they are characterized by band-like fibrosed evaginations of the synovial membrane and of the synovial fat and connective tissue into the joint spaces. The following therapeutic suggestions, based on the personal experience of 15 cases, are made in the knowledge that significant inflammatory or proliferative arthritic changes can be excluded: the plica can be cut through under arthroscopy; chondromalacial defects, directly or indirectly caused by plical rubbing, of the medial femoral condyle and the medial patella can be removed, also under arthroscopic control, with an electric razor. Arthrotomy is no longer needed in most cases.

Light and electron microscopic investigations of nasopharyngeal carcinomas with regard to the viral etiology of these tumors.

Five carcinomata of the nasopharynx (four lymphoepithelial carcinomata of the Regaud type and one squamous cell carcinoma) were examined light and electron microscopically. In addition to the familiar histological and cytological features of these tumors, and because of an increased antibody titer against Epstein-Barr virus in all five patients, all those cytoplasmic and nuclear inclusions were examined which could be interpreted as indicative of a virus contact. The following structures were found: 1. Particles and microtubules which correspond in diameter, shape, and location to Corona viruses. 2. Particles surrounded by a double membrane and resembling in form and diameter Oncorna viruses. 3. Tubulo-reticular, coil-shaped cytoplasmic inclusions interpreted as an unspecific reaction of the host cell to viral attack. 4. Spherical nuclear bodies, which are frequently observed in tumors and in viral infections. 5. Intranuclear particles which correspond in diameter, structure, and distribution to viruses of the herpes type such as have been described in cell cultures of Burkitt lymphoma and nasopharyngeal carcinoma. The fifth group particularly was discussed in detail with regard to differentiation between those particles and other structures which could simulate a virus structure. Together with the appearance of increased ribosomes and of particular chromatin distribution within the tumor cell nuclei, the particles we discussed have been interpreted as morphological indications of a virus etiology of the examined tumors.

[Viruses, virus-like and virus-related structures in nasopharyngeal carcinoma (author's transl)].

Five cases of nasopharyngeal carcinoma with typically elevated EB-virus-antigen-titer were studied by light- and electron microscopic methods. Within the cytoplasm corona viruses in different forms, regular capsid-like particles with a diameter of 60 nm, tubuloreticular aggregates, and pathologic alterations of the membranes of the endoplasmatic reticulum could be found. The nuclei of the tumour cells often showed particles within the nucleoplasm with an average diameter of 100 nm containing a central core; the appearance of the particles is similar to that of herpes viruses as shown in the literature concerning EB-viruses. Atypic mitoses are common as well as free nucleoplasma-like condensates within the ground cytoplasm. Multiple doubling of the membranes of the endoplasmatic reticulum are characteristic for the tumor cell cytoplasm. Myelin figures are often situated within the enlarged tubules of the E. R., within mitochondria and within the nuclei. The results are discussed concerning the possible viral etiology of nasopharyngeal carcinoma. We do not believe that the particles found within the nucleoplasm have a relation to nuclear pores. The arguments are discussed.

[Eye and lymph drainage].

Up to now lymphatics in the eye could not be pointed out. An ocular lymph drainage is denied. Földi succeeded in producing experimentally the syndrome of "lymphostatic encephalopathy and ophthalmopathy" by operative blockade of the cervical lymphatics in animals. In the first part of the present paper a historical view considering the subject "Eye and lymphatic system" is given. In the second part it is entered into the particulars of own experimental studies. As to our own investigations, rabbits have been injected 99mTc-sulfur-colloid, 99mTc-microcolloid, 99mTc-Albumin and 198Au-colloid into the retrobulbar space, anterior chamber, vitreous body and subconjuctival space of one eye. Measurements of the activity's distribution have been made in vivo with an Anger type camera (pho-Gamma-IV Hp, Searle Nuclear Chicago) and in vitro after section with a sodium iodine crystal well counter (Clinimat-200, Picker). In some animals the investigation has been combined with a bilateral dissection of the cervical lymph nodes. After injection in the retrobulbar space a significant concentration of the activity could be observed for the most part in the equilateral Lymphonodulus cervicalis profundus. By the cervical lymph blockade the removal of lymphoctopic substances from the retrobulbar space was largely inhibited. After injection in the anterior chamber a significant concentration could be observed for the most part in the equilateral Lymphonodulus cervicalis superficialis. After intravitreal injection a drainage to the bilateral deep cervical lymph nodes could be observed. After injection into the subconjunctival space a significant accumulation of activity could be registered in the equilateral Lymphonoduli mandibulares and cervicales superficiales. The data substantiate a segmental lymph drainage from the eye: vitreous body and retrobulbar space for the most part into the Lymphonoduli cervicales profundi, anterior chamber and subconjunctival space for the most part into the Lymphonoduli cervicales superficiales been studied. In albino-rabbits indiaink has been injected into the vitreous body and afterwards its drainage has been controlled histologically (frozen sections). In some animals the investigation has been combined with a bilateral dissection of the cervical lymph nodes. The lymph node dissection causes a massive deceleration of the India-ink's outflow; the perivascular connective tissue shows a clear edematous relaxation. The histological evidence suggests that 1. the vitreous body 2. the interstice between the glia cells of the papilla 3. the pial tissue between the neurone bundles of the optic nerve 4. the perivascular space around the central vessels of the retina 5. the subarachnoid sheath of the optic nerve may be interpreted as prelymphatic pathway.

Animals

[The unaffected primary rejection of xenogeneic kidney transplants in the closely related fox-dog species system].

The present study reports of three kinds of experiments of unaffected primary rejection of xenogenous kidney transplanats in the close-related fox-dog species system. The issue is whether there is a relation between the amount of grafted parenchyma and the immune induced potency, that is whether the course of rejection of transplanted single kidneys (group I a) differs from the course after en-bloc transplantation of both kidneys (group I b). In group II alterations of blood chemism and behavior of humoral antibodies are followed in dogs to which a fox kidney was transplanted, while keeping their own functioning kidneys. This experiment is to give information whether the uremic syndrome influences the development of humoral immunity, and what changes of blood chemism may primarily be related to destruction of the graft, under the condition of absent uremia. Untreated graft recipients survived for 5,4 +/- 0,49 days (n = 5) when single kidneys were transplanted (group I a), and 5,2 +/- 0,75 days (n = 5) when both kidneys were grafted en-bloc (group I b). As to the rejecting reactions, both groups are almost equal: the increasing functional failure causes a fast increase of creatinine and urea nitrogen; alkaline phosphatase and LDH show distinct alterations, related to the progress of the graft's destruction. Decrease of albumin level and loss of cholinesterase activity indicate an impaired hepatic function as reaction to uremic intoxication. Gamma-globulins and leucocytes show alterations that can be related to non-specific inflammatory reactions. The immunologically specific initial lymphopenia suggests that after revascularization these cells migrate to the graft, and later react with antigenic structures of vascular endothelium and still later with those of the organ cells. Cytotoxic antibodies appear on the 4th postoperative day in increasing amount. Post mortem histologic examination shows round cell infiltrates in the vastly necrotic renal parenchyma. When the recipient's kidneys are kept in situ and a fox kidney is transplanted (group II) uremia is avoided and the animals survive. During the 30-days period of observation, that is longer than the term of rejection, the titer of cytotoxic antibodies remains stable or tends to increase. LDH and alkaline phosphatase show characteristic changes that are considered sequels from destructed transplantate. The experiments show, aside from certain reservations, that the donor-host combination fox-dog is suitable to serve as preclinic model for human transplantation using xenogenous donors of organs, i. e. anthropoid primates.

[Anatomical distribution of a CCK-oktapeptide like substance in the human brain (author's transl)].

Using Gastrin 14-17 as immunogen it was possible to raise an antiserum, which showed a high sensitivity for the C-terminal octapeptide of CCK in a radioimmunoassay with 125J-gastrin as tracerhormon. High immuno-reactivity could be demonstrated in the grey and white matter of the human cortex as well as in the putamen and nucleus caudatus using this assay. By comparing the unknown brain substance with different peptides in different physico-chemical systems, the brain substance was shown to be homogenous and being similar to CCK-8.

[Histological and morphometric investigations of knee joint synovial membrane concerning diabetic arthropathy (author's transl)].

Joint capsular tissue from both knee joints have been investigated light microscopically and morphometrically in 40 cases with maturity onset of diabetes. The tissue specimen were taken from autopsy cases (27 female, 13 male). The following alterations of synovial tissue can be regarded as typically in diabetes mellitus: 1. An increased formation of synovial villi without proliferation of synoviocytes and without inflammatory reaction. 2. Increased hyalinosis and fibrosis of the synovial villi as well as fibrosis of the flat synovial membrane. The alterations were far developed in more than 50% of all cases with diabetes.

Aged

[Spontaneous hemorrhages into dilated conjunctival lymphatics (author's transl)].

The means of own observations the clinical picture of spontaneous hemorrhages into the conjunctival lymphatics is illustrated. The reasons of this phenomenon are discussed with regard to the histomorphological findings. Hitherto the spontaneous hemorrhages into the conjunctival lymphatics do not seem to have any pathogenic effect.

[Morphometric studies on lymphatic vessels of the axillary region].

Axillary lymph vessels of 94 autopsy cases (42 female, 52 male) have been investigated histologically in serial slices. Total diameter of the lymph vessels, vascular wall diameter, diameter of the medial connective tissue, finally that one of the artificial tissue spaces were measured in almost 8000 single measurements. The mean values of counts for the different vascular wall components were assessed statistically with the aid of a computer. By reason of the enormous individual variation of the number and the size in lymph vessel system a direct comparison of the measurement data from the different wall components was impossible. Therefore, the ratio of connective tissue to smooth muscle within the vascular media was regarded as a significant parameter for the development of a lymphangiosclerosis. The connective tissue/smooth muscle gradient increased from decade to decade with a slighter progression in groups with higher age A significant increase of lymphangi osclerosis was noted in cases with right heart insufficiency in correlation to the degree of heart insuffiency. Severe lymphangiosclerotic alterations were developed after radical mastectomy and radiotherapy of the axilla, after radiotherapy of other malignant tumors, and in cases with heart failures or vascular dysplasia. Morphometric investigations were not possible in cases with floride lymphangitis and in one case of general vascular amyloidosis.

Electron microscopic findings in four cases of nasopharyngeal fibroma.

Light and electron microscopic investigations of four cases of juvenile nasopharyngeal fibroma revealed characteristic structures; a fibrous stroma, an inclination to hyalinisation and formation of scar like tissue, a lacunar thin walled vascular component, large numbers of mast cells and of fibroblasts. The tumor fibroblasts contained different nuclear bodies and particles. There existed five different types of more or less complex spherical bodies. The previously described nuclear electron dense particles with an electron lucent halo could be divided into four groups measuring 60, 90, 150, and 300 nm in diameter. In addition to the previously described ultrastructural properties of the tumors, the nuclei of the tumor fibroblasts were found to contain virus like particles. These particles were less electron dense, measuring 40 to 55 nm in diameter and arranged in groups throughout the nucleoplasm; they were different from chromatin condensations and from perichromatin granules. The structure and the size of the smallest particles was not comparable with the other nuclear inclusions.

The synovial changes in post-traumatic synovitis and osteoarthritis.

The histopathologic appearances of post-traumatic synovitis and synovitis of osteoarthritis cover a limited range of changes and have features in common. Electron microscopy yields information complementary to that given by light microscopy. With these techniques, based on sufficient material, histological analysis linked with clinical observation will permit some degree of differentiation between the two conditions.
