[Pyridimocarbamate (prodectin)-induced acute liver damage, cirrhosis and primary liver cancer--accidental finding?].

Authors have verified during the postmortem of a 60-year-old woman, in addition to clinically manifested vascularly and parenchymally decompensated cirrhosis a small hepatocellular carcinoma with signs of vascularization. The patient received since four years pyridinolcarbamate (Prodectin) because of obliterating peripheral arteriosclerosis. During treatment, icterus occurred the drug-induced nature of which was proved by the provocation-test. Owing to the known hepatotoxic effect of pyridinolcarbamate the question must be raised concerning a possible causal relationship between the acute hepatic damage having occurred four years ago and the cirrhosis and cancer observed in the autopsy material. The reporting of this case was aimed at drawing attention to such a possibility.

Comparison of intestinal toxic effects of platinum complexes: cisplatin (CDDP), carboplatin (CBDCA), and iproplatin (CHIP).

The biochemical background of the intestinal side effects of cis-diammine-1,1-cyclobutane dicarboxylate platinum (II) (CBDCA) and cis-diisopropylamine-trans-dihydroxy-dichloro platinum (IV) (CHIP) was compared with those of cis-diamminedichloroplatinum (II) (CDDP). Biochemical investigations were carried out on mucosal cells isolated by a combined chemical-mechanical method from the total length of the small intestine. After treatment with single, equitoxic doses of Pt analogues, the activities of thymidine kinase (TK) EC 2.7.1.21, sucrase (SUC) EC 3.2.1.26, maltase (MAL) EC 3.2.1.20, and protein content showed dose-dependent decreases, whereas the activity of alkaline phosphatase (AP) EC 3.2.1.20 increased slightly. The nadir of enzyme activity changes occurred 24-48 h after treatment. For the regeneration of the mucosa more than 96 h was necessary. Of the platinum analogues studied, CHIP proved to be the most toxic to the small intestine. While the highest doses of CDDP and CBDCA (0.66 x LD50) caused significant but less than 50% decreases in TK, SUC, MAL, and protein content (PROT), the CHIP doses needed for 50% reduction were between 0.44-0.66 x LD50.

[Scanning electron microscopy studies of the surface structure of benign and differentiated, tumorous papillary urothelial lesions].

Surgical biopsy specimens from the urinary bladder revealing normal histological structures in 2 cases, inflammatory lesions in 4 cases, papillomas--according to WHO classification criteria--in 7 cases, and differentiated non-invasive papillary transitional cell carcinomas in 7 cases, were investigated by means of scanning electron microscopy, with regard to diagnostic surface characteristics. The preparing method using paraffinembedded tissue blocks for the retrospective study proved to be suitable for the visualization of the different surface structures known from the literature. Some of the observed surface alterations seemed to be associated with both inflammatory and neoplastic urothelial lesions. A change of the surface structure is, however, of limited diagnostic relevance, since only the quantitative distribution pattern of single characteristics points to a certain entity. Especially the appearance of pleomorphic microvilli failed to be a hallmark of malignant growth. The role of REM among other methods in the diagnostics of urothelial lesions is discussed.

Column liquid chromatography of methotrexate and its metabolites using a post-column photochemical reactor and fluorescence detection.

On irradiation with short-wavelength UV light in the presence of hydrogen peroxide, methotrexate and its metabolites 7-hydroxymethotrexate and 2,4-diamino-N10-methylpteroic acid are cleaved into highly fluorescent products. This reaction can be used for the sensitive and selective detection of the compounds in biological fluids, following reversed-phase high-performance liquid chromatographic separation. Study of the effect of the mobile phase composition and irradiation time on fluorescence signal intensity showed that a residence time of ca. 3 s in the on-line photochemical reactor was best. The detection limit for methotrexate was 0.4 ng, for 7-hydroxymethotrexate 1.0 ng and for 2,4-diamino-N10-methylpteroic acid 0.6 ng. The addition of dimethylformamide to the mobile phase enhanced the selectivity of separation.

[Stromal tumor of the testis with Leydig cell components. Light and electron microscopy analysis and fluorescence microscopy demonstration of F-actin].

A testicular gonadal stromal tumor with Leydig cells and mesenchymal cells resembling to Sertoli cells is presented. At the ultrastructural level most of the tumor cells could only be classified as endocrine differentiated cells. Because of their uncharacteristic organelles no Sertoli cells could be demonstrated by means of electron microscopy. The labelling of fibrillary actin underlines the cellular heterogeneity and the different cellular functional state of the tumor cells. These findings are interpreted as an evidence of a multidirectional differentiation potential of the gonadal stroma mesenchyme. These observations support the classification of these neoplasia as gonadal stromal tumors particularly in light of the clearly visible differentiation. Problems of evaluating the malignant potential of gonadal stromal tumors are also discussed.

Testicular germ cell tumours. Current problems of histogenesis and classification.

Testicular germ cell tumours, owing to their variety in biological behaviour and morphological appearance, claim a place of their own in clinical oncology and tumour research. Much of the histogenesis has remained unexplained, as reflected by the different systems of pathological classification. This report sums up, on basis of the literature, the current pathologic views on the question of testicular tumour genesis. Data obtained from immune histochemical examinations, animal experiments, ultrastructure studies, together with clinical observations, suggest that differentiation of the carcinomatous stem cells are apt to produce forms, transitional between seminomatous and non-seminomatous types of tumour. The possibility to set up a uniform, clinically appropriate nomenclature depends on the progress in histogenetic knowledge.

[Value of ultrasonography in the diagnosis of bladder tumors].

The authors have compared the value of transabdominal, transrectal and intravesical ultrasonography in 100 patients with carcinoma of the bladder. They have also compared the pathological stage, determined at operation or post mortem, with the "ultrasound" stage. Transabdominal examination resulted in agreement in 61% cases, transrectal 69% and intravesical in 92%. In the authors' opinion, transabdominal ultrasonography is suitable for general orientation. It can determine the size and position of the tumour. Transrectal examination is particularly valuable if, for any reason, it is impossible to perform cystoscopy. It is also valuable for the examination of tumours localised at the bladder base. Intravesical sonography demonstrated the tumour in every case. During this examination, changes in the elasticity and distensibility of the bladder due to tumour and changes in the bladder volume can be determined. Intravesical ultrasonography carried out at the same time as cystoscopy is a rapid and highly practical procedure, which ideally supplements cystoscopy. At present it is the best method for demonstrating infiltration of the bladder wall.

Intravesical ultrasonography as an aid to study the size of bladder cancers.

On clinical examination of 50 patients with bladder cancer the authors performed intravesical ultrasonography. By this method they never failed to detect the tumours. The stage of the pathological material obtained by operation or at autopsy was compared with that obtained by the intravesical ultrasonography. In 88% of the cases the two classifications gave identical results. By ultrasonography the pathologic stage was overestimated in 4, and underestimated in 2 cases. Intravesical ultrasonography can be combined with cystoscopy. It is a fast procedure and an ideal supplement to cystoscopy. The combination of the two methods permits the localization of intravesical tumours as well as the estimation of invasion and of the degree of infiltration.

[Effect of high-frequency ventilation on intracranial pressure].

The influence of respiratory frequency, tidal volume, inspiratory flow and ratio of inspiratory/expiratory time on ventilator-related intracranial pressure (ICP) fluctuations was studied in six patients with severe brain trauma. ICP fluctuations were found to be markedly reduced at frequencies of 20/min and usually eliminated at 30/min. We found an exponential correlation between ICP fluctuations and respiratory frequency, but there was no correlation between tidal volume and ICP. Central venous pressure amplitudes were found to be in linear correlation with respiratory frequency and tidal volumes as well. The amplitude of respiratory ICP fluctuations correlates with the length of expiratory time. Our findings demonstrate that artificial ventilation without ventilator-related fluctuations in ICP ("brain-protective" ventilation) may be performed by conventional volume-constant, time-cycled ventilators. To assess the therapeutic relevance of eliminating respirator-related fluctuations of the ICP course in brain-injured patients, we suggest that frequencies of 25-30/min and tidal volumes of 6-9 ml/kg body weight should be used.

Relative incidence of primary liver cancer in cirrhosis in Austria. Etiological considerations.

The incidence of primary liver cancer (PLC) was assessed in 652 patients (429 male, 223 female) with liver cirrhosis. By the end of the study 416 (64%) of all patients had died. The autopsy rate was 65%. PLC developed in 73 patients. Histologically, 67 cases were classified as hepatocellular (HCC), one as mixed hepatocellular-cholangiocellular and five as cholangiocellular carcinoma. In 30 cases, HCC was confirmed within a year of diagnosis of cirrhosis, and in the remaining 37 cases HCC developed 2-13 years later. At autopsy, the frequency of HCC was almost twice as high in males (27%) as in females (15%) (p less than 0.025). This difference occurred in alcoholic and posthepatitic cirrhosis but not in cryptogenetic cirrhosis. HBsAg was detected in 19.0% of male and 9.2% of female patients (p less than 0.01). Comparison of patients with and without HCC revealed no significant differences in the prevalence of HBsAg. These data indicate that HCC is common in Austrian patients with cirrhosis, representing 60% of all malignant tumors in this group. In view of the high HBsAg carrier rate and the prevalence of chronic alcoholism in patients with cirrhosis, it is suggested that the two factors together lead to an increased risk of cirrhosis followed by an increased incidence of HCC.

[Prognosis of liver cirrhosis: results of a 14-year-long clinical follow-up study].

The incidence of primary liver cancer (PLC) was assessed in 652 patients (429 male, 223 female) with liver cirrhosis. By the end of the study 64% of all patients had died. The autopsy rate was 65%. 138 patients (49%) died after acute gastrointestinal hemorrhage. PLC developed in 73 patients (11.3%). Histologically 67 cases were classified as hepatocellular, one as mixed hepatocellular-cholangiocellular, and 5 as cholangiocellular carcinoma. In 32 cases PLC was confirmed within a year of diagnosis of cirrhosis, while in the remaining 41 cases PLC developed 2 to 13 years later. At autopsy the frequency of PLC was twice as high in males (30%) as in females (15%) (p less than 0.025). This difference occurred in alcoholic and posthepatic cirrhosis but not in cryptogenetic cirrhosis. HBsAg was detected in 18.5% of male and 8.9% of female patients (p less than 0.01). Comparison of patients with and without PLC revealed no significant differences in the prevalence of HBsAg. These data indicate that PLC is common in Austrian patients with cirrhosis and represents 64% of all malignant tumors in this group. In view of the high HBsAg carrier rate and the prevalence of chronic alcoholism in patients with cirrhosis, it is suggested that both factors together lead to an increased risk of cirrhosis followed by an increased incidence of PLC. Survival curves show that that the prognosis of severe liver cirrhosis is the same as that of patients with malignant diseases. Therefore, it is important to detect these patients at an early stage of the disease.

["Fibrous long-spacing collagen" in seminomas].

In the lymphocytically infiltrated stroma of 2 out of 5 classic seminomas normal collagen, an amorphous substance and a collagen with atypical periodicity, so called fibrous long spacing collagen, were found. In one case, the fibrous long spacing collagen was in contact with the basal membrane of tumor cells. The origin and the importance of fibrous long spacing collagen are discussed.

[Determination of cis-diamminedichloroplatinum compound toxicity in pigs from the viewpoint of pathomorphology].

The study is a part of the clinical testing of the cytostatic preparation PLATIDIAM --Lachema with cis-diammine-dichloroplatinum complex (cis/DDP) as an active ingredient. The organs and tissues of 18 test pigs weighing 31 to 55 kg were subjected to the patho-morphological examination. The animals had been given experimentally the test cytostatic preparation Platidiam at a rate of 2.5 and 4.0 mg cis-DDP per kg 1. w. with and without hydration of their organisms. Severe changes were recorded in the kidneys of the pigs given the toxic doses of cis-DDP (4 mg/kg l. w.). The most frequent finding was that of cylindrical protein blocks in renal tubules combined with hyalinization of tubule cells. Optical microscopy of marrow showed vacuolization of epithelium and peeling off of epithelium layers. In some cases the dystrophic changes in renal tubules even led to necrotization of epithelium. No such changes were observed in the remaining animals given the therapeutic doses (2.5 mg/kg l. w.) nor in the controls. The findings on other organs cannot be associated with the toxicity of cis/DDP.
