[Parasitic helminths infection in Coho salmón, Oncorhynchus kisutch, during their return to Simpson river, Chile].

Between may and july 1994, 17 adult returning salmons, Oncorhynchus kisutch, were collected in the River Simpson, Chile. All fishes showed infection by plerocercoids of Diphyllobothrium sp. in different locations: stomach, spleen, liver, mesenteries and gonads. Infection with larval cestodes of an unidentified species of Phillobothriidae was determined in the intestine of seven (41.2%) salmons and its prevalences of infection showed significant differences between female and male salmons. The 94.4% of total plerocercoids of Diphyllobothrium were isolated from the stomach wall. Prevalence and mean intensity of infection by Diphyllobothrium sp. did not show significant differences between fishes of different sex.

[Fenoterol-ipratropium nebulization in infants with obstructive bronchial syndrome].

A double blind prospective study was done in 120 infants with acute obstructive bronchial disease. One half of them received combined fenoterol-ipratropium solution by inhalation in three different dose schedules while the other half was given inhalation normal saline solution for comparative purposes. Statistical analysis showed a significant difference in favor of fenoterol-ipratropium treatment over controls as to severity of bronchial obstruction, days of recovery and the need of additional bronchodilator drugs. However, no significant differences in the response of these parameters were noted among patients receiving different dose schedules of fenoterol-ipratropium. The combination of fenoterol-ipratropium showed a beneficial bronchodilator effect in patients with acute obstructive bronchial disease.

Secretory IgA deficiency in pediatric patients: clinical and laboratory follow-up.

This is a prospective three-year (1985-1988) clinical and laboratory follow-up study of 43 pediatric patients without any measurable secretory IgA, intending to describe the their natural course. We also intended to detect evidence which may allow to predict their future outcome, whether they would become asymptomatic or end up developing chronic disease. A direct statistical correlation was found between those patients who normalize their secretory IgA levels and their course in an asymptomatic state. Thus, if a child does not have any sIgA at all, the relative risk to get sick is 0.86 (86%), while in those having sIgA within normal ranges, the relative risk of disease decreases to 0.46 (46%), representing a p value less than or equal to 0.05. Among the analyzed variables and their influence on the fact that a patient may or may not synthesize sIgA, none of them showed a predictive value for bronchial asthma by itself. However, when sIgA and total serum IgE levels, were analyzed together (being the two variables demonstrating to influence), the probability of becoming asthmatic is much greater in the ones with elevated total serum IgE and absent sIgA. This group is significantly different from the one with normal IgE and sIgA levels (p less than or equal to 0.001). According to our experience in evaluating and controlling a pediatric patient with repeated episodes of bronchial obstruction and lacking sIgA, an immediate strict environmental control should be established in order to avoid all possible contacts with allergens.(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of secretory IgA childhood respiratory diseases.

Secretory IgA salivary concentration was studied, in order to investigate its influence as a possible causative role in groups of children affected by different respiratory diseases. Allergic bronchial asthma (n = 37), recurrent pneumonias (n = 11), recurrent upper airway infectious (n = 12) and a control group of healthy children (n = 25) were included. The sIgA mean concentration plus S.D. were: 26.0 +/- 11.5, 15.5 +/- 16.7, 21.2 +/- 15.6, 16.2 +/- 12.7 and 30.8 +/- 10.1, respectively. A statistically significant association between decreased or absent levels of sIgA and disease was found, but no correlation between alterations in sIgA concentrations and any class of serum immunoglobulin levels, or with any other analyzed variables could be established. Among the group of diseases studied, a statistically significant difference (p less than 0.01) was found between infectious asthma and controls, recurrent upper airway infections and controls, and between allergic and infectious asthma. These findings support the conclusion that a decrease or absence of sIgA is an important factor to be considered in infectious asthma, as well as in recurrent upper airway infectious diseases affecting children.

Ten-year follow-up in pediatric patients with allergic bronchial asthma: evaluation of specific immunotherapy.

Specific immunotherapy as a treatment for bronchial asthma has been a controversial field. A collaborative study was designed in an attempt to further define the possible contribution of specific immunotherapy in a pediatric population. One hundred and sixty-six patients were treated with immunotherapy and 248 received no immunotherapy. The results were compared during 10 years of follow-up in both groups. There was a significant decrease in the number of acute crisis in the treated group (p < 0.05). However, no differences were seen in the number of hospital admissions or in the quality of life between the treated and untreated groups. The treated group required significantly fewer drugs (p < 0.05). Thus, it is probable that the untreated group shows the same clinical indexes as the treated group due to their increased use of medication. Specific immunotherapy represents an effective treatment that changes the natural course of allergic bronchial asthma in pediatric patients.

Correlation between the clinical classification of bronchial asthma severity and the methacholine test in children.

The purpose of this study was to validate the Methacholine Bronchial Challenge Test in its different degrees and to compare them with the different clinical classifications of bronchial asthma. Fifty-four child asthmatic patients over 6 years of age from the Respiratory Outpatient Clinic at La Union Hospital (Chile) were submitted to the clinical classification of bronchial asthma and later to the Methacholine Bronchial Challenge Test, which classified the children as mildly, moderately or severely hyperreactive to the test. In mildly asthmatic children, the Methacholine Test showed both a high sensitivity (100%) and specificity (92.3%). In moderately asthmatic children, the sensitivity decreased to 84.5%, whilst the specificity was maintained (92.3%). In severely asthmatic children, however, the sensitivity decreased to 53.5%, whilst the specificity increased 97.2%. The latter could be explained as a consequence of the previous exposure of this group of children to either inhaled or oral corticoids. According to our experience, the Methacholine Test is not only useful for the diagnosis of bronchial hyperreactivity in dubious cases, but also for contributing together with the clinical findings to the adequate classification of these patients.

[Infection by intestinal protozoa and helminths in schoolchildren from riverside sectors, with different fecal contamination levels, of Valdivia River, Chile].

During 1993, the prevalences of infection by intestinal protozoa and helminths (IPH) in the riverside schoolchildren population of two sectors of Valdivia river, with different levels of fecal contamination, were compared in relation to their contact with river water (swimming, bathing) and basic sanitation elements (BSE) of their houses: feces and garbage disposal, and water supply. Populations of children from sectors A (n = 418), with significatively greater (SG) total coliform most probable number (MPN), and sector B (n = 400), were examined. PAFS test was used for the stool examination of one sample, from each child. The global prevalence of infection by IPH was similar in A (74.8%) and B (72.5%) sectors. From 10 identified IPH, only Entamoeba histolytica and Hymenolepis nana showed prevalences of infection SG in sector A, than in sector B. The contact of the children with the water of the river showed association with SG prevalences for E. histolytica, Entamoeba coli and Endolimax nana in sector B and for E. histolytica in sector A. No sanitary condition for the different BSE contributed to SG prevalences for E. histolytica, Ascaris lumbricoides and Trichuris trichiura in both sectors, and for E. coli in sector B. The contact with the river water and the different BSE contributed to prevalences SG in 5 and 3 IPH in sectors B and A, respectively. The results suggest that a project in study for the development of a treatment plant of wastewater in sector A, could contribute to control infection by IPH. But, this measure must be associated with improvement of the BSE, health education, and application of chemotherapy measures for geohelminthosis in both sectors.
