OBJECTIVE: To verify whether a prolonged (therapeutic) treatment with ranitidine or famotidine may affect blood alcohol concentrations (BAC) in patients with duodenal ulcer (DU). METHODS: Sixteen male patients with DU were studied. Subjects with Helicobacter pylori-associated DU did not enter the study. Patients randomly received either 300 mg ranitidine (n = 8) or 40 mg famotidine (n = 8) at bedtime for 2 months. They had a standard lunch (1:00 PM), and 0.3 g/kg of alcohol was given 15 min after the meal. BAC were measured by head-space gas chromatography up to 150 min. RESULTS: The rate of GFPM was checked in all patients before they entered the study; we found it to be 53%, by comparing the area under the curve (AUC) of BAC after either intravenous or oral (po) administration of 0.3 g/kg ethanol. Ranitidine did not significantly modify either the mean AUCpo (5.8 +/- 1.8 vs. 6.2 +/- 1.4 mM/h, before vs. after treatment) or the peak BAC (4.6 +/- 1.1 vs. 5.3 +/- 1.7 mM) after 2 months of treatment. Famotidine failed to affect BAC in the second group of patients (AUCpo 5.0 +/- 1.4 vs. 5.6 +/- 1.7 mM/h, peak BAC 4.0 +/- 1.7 vs. 4.3 +/- 1.8 mM; before vs. after treatment). CONCLUSIONS: These findings suggest that, in males with DU, prolonged treatment with ranitidine or famotidine had no effect on BAC after administration of a small dose of postprandial alcohol.

The authors in explaining their limited clinical experience relative to one single case, describe the peculiarity of Takayasu's disease and its rare incidence in western populations. The case under observation because of its peculiar rarity had been underestimated and treated in a completely inappropriate manner. Important instrument in its diagnosis is the High Resolution echography which in determining the diagnosis favors and speeds up the diagnostic therapeutic iter.
In this paper the Authors report their experience on diagnostic and surgical procedures with respect to aneurysms of the visceral district. The clinical review during the last ten years of surgical activity in the Istituto di Clinica Chirurgica (Università di Ferrara) regards 13 patients with different visceral aneurysms. The study reports the different percentage of localization, the diagnostic approach, the surgical procedure performed and discussion of results obtained. Patients at risk for this disease are pointed out.
The Authors report on the use of non-invasive techniques in the preliminary evaluation of revascularization of the femoral-popliteal axis. The study of this pathology, for which surgery has become less and less frequent, being limited to the more serious cases, has greatly benefited by the use of H/R echography and of Doppler. These techniques, which remain complementary to angiography, have proved very important and reliable for their capacity to visualize the arterial lumen, an important element for the pre- and post surgical evaluation of vascular recanalization and of the relationship of continuity and continuity with the nearby structures.
An anomalous (because of the advanced age of its onset) case of isthmic coarctation of the aorta in preductal site observed at the persistence of hypertensive symptomatology associated with cephalgic and lipothymic attacks is reported. Instrumental examinations confirmed the clinical suspicion and resective-reconstructive surgical treatment led to complete resolution of both cause and effect.

A case of retroesophageal right subclavian artery, occasionally observed in a patient submitted to diagnostic investigation and surgical treatment for bilateral steno-obstructive involvement of the carotid district, is described.
Personal experience on the intraoperative use of HR echography for the evaluation of the immediate surgical results in carotid bifurcation T.E.A. is reported. It is already assessed that early failure in vascular surgery is due to technically incorrect procedures.
Following a short introduction on the physiopathology of the phlebo-lymphatic system, the results of a study carried out on 114 patients suffering from chronic venous insufficiency (CVI) treated at the Department of General Clinical Surgery and Surgical Therapy of the University of Ferrara are reported. These patients were subdivided into two random groups and treated with common phlebotonic drugs and aminaftone, respectively. Both groups were also subjected to those physical and medical measures that are usually adopted for the treatment of such patients. The therapeutic effects were assessed by comparing symptomatology before and after 90 days of treatment, both subjectively (patient's assessment of the symptoms by means of a scoring system) and with objective methods (measurements and echo-Doppler examinations). The differences between the two groups were statistically very significant in favour of patients who had taken aminaftone. Excellent results were obtained in the treatment of CVI as well as in lymphatic-related pathology (lymphedema).

The role of thyroid hormone in the pulmonary repair process following chloride-induced acute injury, was assessed in the present study. Thyroidectomized (Thyx), male Sprague-Dawley rats were exposed by inhalation to cadmium chloride aerosol (CdCl2, 10 mg/m3). Rats were sacrificed 1 hr after [3H]thymidine (3H-T) injection at intervals up to 10 days after exposure. Thyroidectomy, followed by CdCl2, produced earlier and more severe acute injury in the form of alveolar hemorrhage edema and hyaline membrane formation, than CdCl2 alone. However, Type 2 cell hyperplasia was markedly reduced in this group of rats compared with CdCl2 controls. Uptake of 3H-T by Thyx-CdCl2 lung tissue was only 40% of that measured in CdCl2 controls. Autoradiographic studies indicated that Type 2 cell labeling was less than 66% of controls up to 3 days after exposure. Cells obtained by lung lavage of Thyx-CdCl2 rats were reduced in number up to 60% with respect to controls, during the first week after exposure. Additionally, the activities of lung antioxidant enzymes (glucose-6-phosphate dehydrogenase, superoxide dismutase, and glutathione peroxidase were significantly inhibited (45-55%) throughout the experiment in Thyx-CdCl2 animals compared with normal rats. In summary, thyroidectomy impairs the repair response in CdCl2 lung damage by enhancing Type 2 cell damage, reducing Type 2 cell proliferation, altering alveolar macrophage populations, and depressing antioxidant defense systems.
The Authors report about their experience of 27 cases of Total Enteral Feeding. The role of the nitrogen balance inversion, from negative to positive, as an expression of protein anabolism, is specially emphasized. This parameter proves more suitable than the serum albumin level increase, of low statistical significance.
