[Bacterial flora in the digestive tract during diarrhea in infants upon hospital admission and at the end of hospitalization].

The stools of 23 children aged from 14 days to 18 months were analysed (qualitatively and quantitatively) for some genera of bacteria which could be the cause of diarrhea. Feces were collected at the beginning and the end of hospitalisation. The results were evaluated and referred to five age subgroups and duration of hospitalization. The presence of potentially enteropathogenic bacteria was noted in 87 per cent of children in widely varying quantities. In the most numerous group of children the same bacteria were revealed at the beginning and at the end of hospitalization. The microorganisms were most frequently present at the moment of hospitalisation in newborn children. Complete elimination during stay in hospital occurred mainly in the youngest children, who did not acquire these bacteria during their stay in hospital. Colonisation with potentially pathogenic strains in hospital affected mainly children aged 7 to 12 months. EPEC prevailed among the strains isolated in both analyses.

[Evaluation of susceptibility of methicillin resistant Staphylococcus aureus strains to vancomycin].

Susceptibility to vancomycin and teicoplanin was studied in the most frequently isolated Staphylococcus species isolated in the State Hospital Clinic Nr 1 in Warsaw in 1994. No strains resistant to vancomycin were found in two Staphylococci most frequently isolated from clinical material: S. aureus and S. epidermidis. Occurrence of this resistance is very rare and is restricted only to coagulase-negative Staphylococci such as S. xylosus, S. capitis, S. lentus and S. cohni. Resistance to teicoplanin was observed more often (1.4% in S. aureus and from 11.8% to 39.3% in 9 species of coagulase-negative Staphylococci). For the majority of methicillin-susceptible clinical strains (80%), the MBC value did not exceed 4.0 mcg/ml of vancomycin while for almost half of the strains in the population of methicillin-resistant strains MBC values ranged from 32.0 to 128 mcg/ml of vancomycin. Moreover, vancomycin-tolerant strains were found eight times more often in the studied MRSA (26.7%) population than in MSSA strains (3.3%).

[Study of the thermoresistance of Clostridium difficile spores].

Thermoresistance of C. difficile spores was investigated. C. difficile strains were isolated from different sources. As control, toxigenic VPI 10463 and nontoxigenic NIH BRIGGS 8050 C. difficile strains were used. The inhibition of growth majority of C. difficile after heating at 85 degrees C was shown. No correlation between thermoresistance of C. difficile spores, toxigenicity and source of the strains was observed.

[Selected properties of Bacteroides fragilis enterotoxigenic strains].

Seven B. fragilis strains were examined. One strain was reference, non-enterotoxigenic, representing serotype E2 according to Beerens et al. (1971). Six strains produced enterotoxin (ETBF). Four of them were isolated from human feces and two of them from swine feces. All strains were investigated morphologically, biochemically (ATB Expression, France) and by means of direct immunofluorescence (Bacteroides--IF test, Poland). Their resistance to chemotherapeutics was tested (ATB Expression, France). The MIC values of clindamycin and metronidazole were determined using E test strips (AB Biodisk, Sweden). Each strain formed capsules. The percentage of encapsulated cells was high (80-100%). Some strains produced thick capsules. Biochemical patterns were similar and typical for B. fragilis rods. One enterotoxigenic strain produced gelatinase and three ETBF strains fermented trehalose. All strains reacted in direct immunofluorescence exclusively with conjugate against serotype E of BFG. Thus, each strain showed antigenic pattern E. Drug susceptibility of all strains was similar. One enterotoxigenic strain was resistant to clindamycin. All strains were susceptible to metronidazole. These studies indicate that on the basis of morphological, biochemical and serological features (IF), enterotoxigenic B. fragilis strains cannot be distinguished from the nonenterotoxigenic one. Also, the correlation between toxigenicity and drug sensitivity of the examined strains is not observed.

[Use of cell-surface antigens of Bacteroides thetaiotaomicron in the passive hemagglutination test and in the inhibition of passive hemagglutination].

The antigens were prepared from three B. thetaiotaomicron strains of different origin (NCTC 10582--reference strain, 312/85--isolated from a pancreas fistula drain of a patient, and 9/18--derived from normal human intestinal microflora). Lipopolysaccharides were extracted by the hot phenol-water method and purified by nuclease tretment. Degradation of lipopolysaccharides was achieved by mild acid hydrolysis obtaining polysaccharide (PS) and lipid (LA) fractions. Capsular polysaccharide (CPS) was extracted from the clinical strain 312/85 producing thick capsules. Antibacterial sera were prepared by immunization of rabbits with formolized suspensions of investigated strains. HA and IHA were performed with all antigens and immune sera. Examined cell-surface antigens (LPSs and CPS) were capable of coating formolized sheep erythrocytes. The titres obtained in the passive hemagglutination test in homologous reactions were 160-320. Polysaccharide fractions (PS) prepared by means of mild acid hydrolysis of LPSs were unable to coat formolized sheep red blood cells. The activity of sensitization of sheep erythrocytes was revealed by lipid fractions of LPSs. All preparations were active as inhibitors in the inhibition of passive hemagglutination. The strongest inhibitors in homologous systems were polysaccharide fractions of LPSs and capsular polysaccharide (the concentration of inhuibitor 8-15 micrograms/ml). The results of performed serological tests indicated the antigenic similarity of standard B. thetaiotaomicron strain NCTC 10582 and clinical strain 312/85. The strain 9/18 from normal human intestinal microflora showed distinct antigenicity. High-molecular cell-surface B. thetaiotaomicron antigens containing carbohydrates (LPS and CPS) can be applied in the passive hemagglutination test and in the inhibition of passive hemagglutination.

Detection of Bacteroides fragilis endotoxin in amniotic fluid by counterimmunoelectrophoresis.

The ability of counter immunoelectrophoresis (CIE) to detect Bacteroides fragilis endotoxin in amniotic fluid in small concentrations was evaluated. A method was developed which, in combination with ultrafiltration, permits detection of B. fragilis endotoxin in amniotic fluid in a concentration of 40 ng/ml or more. The sensitivity threshold was reduced to 2 ng/ml by using a highly reactive IgG-fraction isolated from rabbit anti-B. fragilis IPL E 323 antiserum.

The toxicity of antigens extracted from strains of Bacteroides vulgatus from different origins to chicken embryos.

The toxicity of purified lipopolysaccharides (LPS) and capsular polysaccharides (CPS) extracted from 3 strains of Bacteroides vulgatus to 11 day old chicken embryos was examined. The three B. vulgatus strains were isolated from different sources. The CELD50 (chicken embryos lethal dose 50%) of these preparations was determined. All antigens were shown to be lethal to chick embryos. The toxicity of LPS of the strains isolated from clinical sources was higher than that of the capsular antigens. The LPS and CPS of one strain isolated from normal gut-flora was less toxic than other antigens.

Endotoxin-induced fetal growth retardation in the pregnant guinea pig.

OBJECTIVES: Our purpose was to test the hypothesis that bacterial endotoxin may reduce fetal growth and to assess some of the pathophysiologic mechanisms of such an effect. STUDY DESIGN: Two randomly selected groups of nine guinea pigs at 30 days' gestation were treated with a solution of endotoxin isolated from Bacteroides fragilis or with solvent alone. Antibody titers, glucose, triglycerides, and 6-keto-prostaglandin F1 alpha were determined in maternal or fetal blood samples. Fetal weight was determined at 61 days' gestation. RESULTS: Endotoxin-treated guinea pigs showed positive antiendotoxin antibody titers, reduced weight gain, and significantly higher serum levels of triglycerides and 6-keto-prostaglandin F1 alpha, but not of glucose, than did sham-treated controls. Fetuses of endotoxin-treated animals had significantly lower birth weights and serum glucose concentrations and significantly higher triglyceride levels than did control fetuses. CONCLUSIONS: Bacteroides fragilis endotoxin causes fetal growth retardation in the pregnant guinea pig, which may be due to alterations in carbohydrate and fat metabolism mediated by cytokine action.

Comparison of Delmée and Polish serogroup-specific Clostridium difficile strains.

In this study we compared Delmée and Polish serogroup-specific Clostridium difficile strains by slide agglutination, observation of flagella and SDS-PAGE protein patterns. Among Delmée serogroup specific strains we observed flagella in groups A, D and K. Among Polish serogroup-specific Clostridium difficile strains we did not observe flagella. The Polish serogroup-specific strains gave different protein patterns compared to Delmée strains. SDS-PAGE protein pattern analyses of Polish serogroup-specific strains divide these strains into 3 groups: 71, 88 and third embracing 18, 27, 70, 72, and 89.

[Biological activity of Bacteroides fragilis].

The investigation was performed with 9 strains of B. fragilis isolated from pregnant women in their 38th week of pregnancy, strain IPLE 323 obtained from France, strains BE17 and BE61 obtained from Netherlands and a strain 210 isolated from a case of appendicitis. The strains were identified by indirect immunofluorescence method and by API 32 ATB test. Ability to adhere to epithelium of oral cavity and vaginal mucosa and sheep and human erythrocytes was investigated. Susceptibility to antibiotics was also measured. It was found that strains isolated from vagina easier adhere to epithelial cells of vagina than to cheek epithelium and erythrocytes.

[Susceptibility of Clostridium difficile strains to teicoplanin and ramoplanin].

The in vitro activity of teicoplanin and ramoplanin against 113 Clostridium difficile strains isolated from different origins was determined. The minimum inhibitory concentrations (MICs) of antibiotics were determined by the agar dilution method. MIC of teicoplanin and ramoplanin were twofold lower for Clostridium difficile strains isolated from patient with pseudomembranous colitis. No correlation between the origin of the strains toxigenicity and susceptibility to teicoplanin and ramoplanin was observed. Our result indicate that teicoplanin and ramoplanin are potentially effective alternative antibiotics for treatment of infections caused by Clostridium difficile.

Human T cell adhesion to endothelium stimulated by membrane components extracted from strains of Bacteroides vulgatus (member of B. fragilis group).

Bacteroides fragilis is reported to be less virulent than other Gram negative rods, existing in normal gut flora (e.g. Enterobacteriaceae). Lymphocytic infiltration, a typical finding in chronic infection, depends on cell extravasation and trafficking towards a site of antigen, a process that is regulated by endothelial expression of adhesion molecules. We have studied the ability of lipopolysaccharides and capsular polysaccharides of B. vulgatus group to induce human T cell adhesion to cultured endothelial cells. Both components of the bacterium caused markedly lesser T cell adhesion than LPS extracted from E. coli. Our data suggest that lower virulence of members of B. vulgatus group might be also dependent on its decreased ability to induce lymphocyte attachment to endothelium and subsequent tissue infiltration.

[Occurrence of Clostridium difficile in the digestive system of dogs].

This study was aimed at seeking strains of Clostridium difficile in feces and investigation of influence of antibiotics application on frequency of isolation and detection of toxing in vivo produced by this microorganism. Samples of feces were obtained from experimental dogs consisting of two groups. To groups I belonged 150 healthy dogs. Group II consisted of samples of feces received from four dogs before and after application of various antibiotics. Clostridium difficile was not isolated from group I dogs. From dogs belonging to group II, 28 strains were isolated. Production of toxing and classification to serological groups of the isolated strains were performed. Presence of this microorganism in feces of dogs is evident only after multiple application of antibiotics. Results of these studies suggest that dogs may constitute a reservoir of Clostridium difficile.

[Results of treatment with ofloxacin for inflammatory conditions in male reproductive organs].

The results are presented of the clinical and microbiological studies of the patients of the Andrological Outpatient Clinic in Warsaw, who were examined for explaining the causes of marital infertility or for elucidation of the cause of testicular pain. Thirty-two men aged 21 to 42 years were chosen, in whom semen investigation demonstrated presence of leucocytes in numbers exceeding 10 per field of vision and with clinical signs of inflammatory process in the male reproductive organs (10 patients) and without clinical signs (22 patients). The patients in whom semen cultures demonstrated presence of bacteria sensitive to ofloxacin were treated with the preparation Tarivid Hoechst for 10-20 days in daily doses of 2 x 200 mg. The clinical signs of inflammation of the reproductive organs regressed in 100% of cases, and in 64% of cases no leukospermia was found after the treatment.

Comparative studies of serotype-specific Clostridium difficile strains.

The following properties of serotype-specific Clostridium difficile strains were studied: toxigenicity, encapsulation, susceptibility to certain antibiotics, biochemical properties, enzymatic activity. No correlation between toxin titer and frequency of capsule production as well as serogroup affiliation and sensitivity to antibiotics was observed. The strain representative of serogroup C attracts attention because of its distinct properties.
