[Liver tumors and oral contraceptives. Report of a case and review of the liverature].

On laparotomy in a 33-year-old woman presenting with signs of chronic cholecystitis and cholelithiasis, a tumor (diameter 4.5 cm) of the right lobe of the liver was found and resected. It proved to be a hamartoma (focal nodular hyperplasia). At the age of 20 years the patient had been treated by radiation and chemotherapy for stage I Hodgkin's disease. Oral contraceptives had been prescribed for a period of 2 years. In the literature approximately 70 cases of chielfy benign liver tumors have been reported in women taking oral contraceptives. More than one third of these cases came to light because of life-threatening intraabdominal bleeding due to rupture of the tumor. A causative relationship between hepatic tumors and the use of oral contraceptives is suggested. As yet, however, there is no conclusive evidence to support this suggestion.

[Postoperative haemolytic jaundice due to deficiency of G-6-PD (author's transl)].

A case of postoperative haemolytic and cholastatic jaundice after cholecystectomy with no G-6-PD activity is reported. Enflurane is supposed to be the cause in this postoperative jaundice. Considering the fact that Glucose-6-Phosphate-Dehydrogenase deficiency is quite common depending on the patients' origin, this cause must be taken into account and the G-6-PD activity of the erythrocyts should be determined.

[Polycystic liver. Report of one case and therapeutic discussion (author's transl)].

The authors report a new case of polycystic liver associated with polycystic kidney. The functional disorder due to distension of the cysts led to surgery. The impossibility of carrying out removal of the liver, owing to the diffuse character of these lesions, led the authors to choose the method described by Tien Yu Lin in 1968. This seems to be indicated in a few cases owing to the remarkable functional results.

[Serial puncture of lymphnodes. Simultaneous occurrence of chronic lymphatic leukemia and malignant lymphoma with cytologic picture of Hodgkin's disease (author's transl)].

A case of the rare coexistence of chronic lymphatic leukemia and a malignant lymphoma with cytological picture resembling that of Hodgkin's disease is described. The simultaneous occurrence makes the transition from one disease to the other rather doubtful. The author's conclusion is therefore, that it is a coinciding, simultaneous incidence of two different malignant diseases of the lymphatic system. The reader's attention is also drawn to the valuable diagnostic method of serial puncture of lymphnodes.

[Anticoagulant efficacy of Sintrom and blood concentration of Ludiomil on simultaneous administration of the two preparations].

As long-term anticoagulant therapy with coumarin derivates like Sintrom becomes more and more important the question arises whether there is an interaction with a new drug like Ludiomil. An open, noncomparative trial was carried out with 20 patients receiving Sintrom (overall mean :3.0 mg/d, range :0 to 10 mg/d) for a period of 4 weeks, and in addition, during the second and third week Ludiomil (50 mg t. i. d.) orally. As a measurement of the efficacy of the antiocoagulant therapy, the correlation coefficients between the doses of Sintrom and the prothrombin value of the nex day were calculated for each patient and each week separately. An analysis of variance did not show any statistically significant difference between the correlation coefficients of the weeks with and without Ludiomil therapy, i.e. there is no change in the efficacy of the anticoagulant therapy. During the Ludiomil therapy the control of the anticoagulant therapy was slightly better, i.e. the fluctuations of the prothrombin values less, but statistically these differences were not significant. The plasma levels of Ludiomil in these patients were similar to thse found previously in both depressed and healthy volunteers.

Acenocoumarol↗

A double-blind clinical trial to determine if an interaction exists between diclofenac sodium and the oral anticoagulant acenocoumarol (nicoumalone).

A double-blind crossover trial between diclofenac sodium and placebo was carried out in 32 hospitalized patients who were thought to be stabilized on concurrent anticoagulant therapy with acenocoumarol. The object of the trial was to investigate any possible interaction between diclofenac and anticoagulant by monitoring prothrombin times daily through the four week period. No statistically significant difference between placebo and diclofenac could be shown and some problems of accurately monitoring prothrombin times are discussed.
