Value and role of percutaneous regional node aspiration cytology in the management of penile carcinoma.

The determination of the status of regional draining lymph nodes is crucial in the prognosis and treatment of penile carcinoma. Tumor staging is inaccurate without histologic examination of the nodes, but the high attendant morbidity rate of lymphadenectomy mandates careful case selection rather than routine prophylactic dissection. Since the penile and pedal lymphography opacifies all lymph nodes involved by penile carcinoma, fine needle aspiration cytology of regional nodes is an innocuous, accurate, easy, and inexpensive diagnostic procedure and alternative to surgical staging in determining the nodal extension of neoplasm. Positive cytology enables rapid, early and even curative lymphadenectomy in patients with clinically negative but microscopically positive nodes. Since radiopaque contrast medium opacifies the nodes for 6-9 months, repeated fine needle aspiration cytology may be used for accurate, careful and regular follow-up examination of patients with penile carcinoma.

'True' sarcomatoid carcinoma of the renal pelvis. First case report with immunocytochemical study.

A case of sarcomatoid carcinoma of the renal pelvis is reported. The neoplasm showed polypoid configuration and was composed exclusively of plump, spindle and pleomorphic cells. Light microscopy did not reveal any epithelial differentiation of the neoplastic cells. Immunoperoxidase staining for keratins and for epithelial membrane antigen was strongly positive in the spindle elements and showed the epithelial nature of the proliferation. The present case suggests that pleomorphic tumors, when occurring in visceral organs, should be carefully sampled and immunocytochemical markers for epithelial and sarcomatous differentiation studied, before diagnosis of carcinosarcoma or sarcoma may be accepted.

Squamous cell carcinoma with sarcoma-like stroma of the female genital tract. Clinicopathologic study of four cases.

Four cases of squamous cell carcinoma with sarcoma-like stroma located in the vulva (1), vagina (2) and cervix (1) of postmenopausal women are presented. The gross and microscopic features are very similar to those of similarly named tumors occurring in the upper respiratory and digestive tract and in the skin. Light microscopic, electron microscopic, and immunohistochemical examination provided convincing evidence that these tumors are composed solely of squamous cell carcinoma, which has undergone a spindle cell sarcoma-like transformation in the deeper portions. Follow-up revealed an aggressive clinical course in three of the four patients, who died of their tumor between 2 and 45 months after presentation. At the time of death, two of the patients had widespread metastases and the other had massive local recurrence.

Accuracy of transcutaneous aspiration biopsy in the definitive assessment of nodal involvement in prostatic carcinoma.

The inaccuracy of lymphangiography in predicting nodal involvement and the complications of pelvic lymphadenectomy prompted us to evaluate other methods of staging prostatic cancer. On the basis of the results obtained in 24 patients with carcinoma of the prostate, the authors suggest transcutaneous fluoroscopy-guided fine needle aspiration biopsy as a satisfactory alternative to lymphadenectomy. In 95-8% of patients pelvic lymphadenectomy confirmed the cytological findings of transcutaneous aspiration biopsy of pelvic lymph nodes using a "long-bevelled, side-holed" thin needle. It is considered essential to perform aspiration biopsy on most of the pelvic lymph node chains opacified, regardless of lymphographic diagnosis.

The value and role of percutaneous pelvic lymph node aspiration biopsy in definitive staging of prostatic and bladder carcinoma.

Percutaneous fluoroscopy-guided aspiration biopsy of the pelvic nodes was performed for staging in 136 nodal chains of 26 patients with clinically localized carcinoma of the prostate and in 14 patients with bladder cancer. The fine-needle aspiration biopsy had an overall accuracy of 97.5%, sensitivity of 94.11% and specificity of 100%. In the management of prostatic cancer, the positive aspiration biopsy may be considered a definitive diagnostic means for determining tumor stage. The negative aspirations may be accepted as definitive diagnostic staging procedure too, since the sensitivity of the method is of 93.3% in patients with prostatic carcinoma. As the presence or absence of nodal metastasis cannot be predicted accurately on the basis of the T or P category of the primary tumor in patients with bladder carcinoma, the fine-needle aspiration biopsy may have a prominent role in accurate clinical stage determination, for a rational application of the treatment.

Primary malignant mixed mesodermal tumor of the gallbladder. Report of a case and critical review of diagnostic criteria.

A rare case of primary malignant mixed mesodermal tumor of the gallbladder arising in a 75 year old woman is reported. The previously published cases of similar tumors were reviewed in order to outline the histological features and the histogenesis. In diagnosing a malignant mixed mesodermal tumor of the gallbladder it was imperative that we excluded malignant neoplasms with multiple histological patterns. The diffuse and close intermingling of the epithelial and mesenchymal component ruled out a collision tumor. The high mitotic rate, the typical reticulin pattern and the obviously malignant osteoblasts excluded a spindle cell carcinoma with osseous metaplasia. The authors conclude that this is the first case of malignant mixed mesodermal tumor with evident osteosarcomatous areas, described in the gallbladder.

["Spontaneous" esophageal perforation. A rare emergency for surgical treatment].

Spontaneous esophageal perforation is a rare but not-exceptional occurrence. Its polymorphous symptomatology often prevents the early diagnosis. Surgery is the therapy of choice. The treatment is variable according to early or late diagnosis, severity of the lesion and associated conditions such as mediastinal sepsis, hydropneumothorax and loss of water, electrolytes and proteins. The mortality rate among the untreated patients is highest when compared with the treated ones (90-100% versus 28-65%). A conservative approach is justified only in the case of bad general conditions, contraindicating thoracotomy. A wide review of the surgical managements is given.

[Diagnostic value and therapeutic limits of endoscopic polypectomy of the stomach].

Digestive endoscopy represents the current therapy of the gastric polyps. These lesions are endoscopically found in an unexpected number of patients, because of the frequent failure of radiology by itself. Furthermore endoscopic radical polypectomy is mostly possible for the inflammatory and adenomatous polyps. Otherwise endoscopy favours the early detection of malignant changes and coexisting per-cancerous gastropathy. A wide review of the literature is given.

[Breast metastasis as the initial clinical symptom of renal adenocarcinoma].

Renal adenocarcinoma shows an unpredictable clinical course. At autopsy, metastases are present in as many as 95 percent of patients with renal adenocarcinoma. The percentage of metastases from renal adenocarcinoma is greater than would be expected from the frequency of this carcinoma among all carcinomas. The breast is an exceptional site of metastasis, that is not described in literature. In the present report the metastasis in the breast represented the early symptom of renal adenocarcinoma.
