Characterization of the IgA and subclass IgG responses to neutralizing epitopes after infection of pregnant sows with the transmissible gastroenteritis virus or the antigenically related porcine respiratory coronavirus.

In this study, we have investigated the characteristics of secreted IgA and other classes of Ig induced after vaccination of sows with transmissible gastroenteritis virus (TGEV) or the antigenically related porcine respiratory coronavirus (PRCV). Both viruses induced the secretion of neutralizing antibodies of different classes in the sows' milk, but these protected suckling piglets against TGEV to different degrees. Quantitative differences in the induction of IgA by both viruses were found among the different viral antigenic sites and subsites of glycoprotein S. In TGEV-vaccinated sows, antigenic subsite A was the best inducer of IgA, followed by antigenic site D. After vaccination with PRCV, lower levels of IgA were detected on colostrum and milk, antigenic site D and subsite Ab being the immunodominant sites. This quantitative difference in epitope recognition could explain the differences in newborn piglet protection found using Ig classes purified from the milk of sows immunized with both viruses. Apparently only IgA recognizing at least antigenic sites A and D confers good protection in vivo, whereas any Ig class recognizing only one antigenic site may neutralize the virus in cell culture. These results indicate that the formulation of a subunit vaccine against TGEV has to consider the inclusion of more than one antigenic site involved in virus neutralization.

[Clindamycin-primaquine in the treatment of Pneumocystis carinii pneumonia].

BACKGROUND: The efficacy of clindamycin-primaquine as treatment of Pneumocystis carinii pneumonia (PCP) is similar to that of trimethoprim-sulfamethoxazol and endovenous pentamidine. The untoward effects of this combination are scarce and of mild degree. METHODS: Patients presenting with microbiologically confirmed PCP (bronchoalveolar lavage or induced sputum) were treated with clindamycin-primaquine. Patients presenting PO2 of less than 60 mmHg were excluded, being therefore treated with conventional therapy. RESULTS: Sixteen patients received treatment with clindamycin-primaquine. All demonstrated clinical improvement after the fifth day of treatment. Treatment was discontinued in three patients due to cutaneous adverse reactions. One patient voluntarily discontinued treatment, being readmitted with acute respiratory failure, and died. Clinical cure was achieved in 13 patients who completed the treatment with clindamycin-primaquine. CONCLUSIONS: The combination of clindamycin-primaquine was effective as the treatment of Pneumocystis carinii pneumonia in the patients here reported. The percentage of patients who presented secondary effects was not greater than expected with other therapeutic options. The severity of the adverse events was mild and easy to control.

[Differential aspects of drug prescriptions in elderly people living in the northeast area of Granada: study of 366 individuals].

BACKGROUND: The geriatric population consumes greater numbers of medicines than other age groups. We compared drug prescription and consumption in elderly persons residing in a nursing home or living in their own home. METHODS: We studied 366 elderly men and women (older than 65 years) who lived either in nursing homes or in their own home. The variables recorded were the most frequent diseases, number and types of drugs prescribed per person, and adverse reactions of the most frequently prescribed drugs. RESULTS: The most common diseases recorded among subjects living at home were arterial hypertension and rheumatic diseases; among those in nursing homes, the most common ailment was dementia. Subjects in nursing homes were prescribed more different drugs (2.8 +/- 1/person) than individuals living on their own (1.8 +/- 1/person). The percentage of adverse reactions was highest in ambulatory patients (64%) and in patients taking benzodiazepines (56%). A large proportion of all prescriptions (74.5%) were for solid galenic forms. CONCLUSIONS: We draw attention to the greater consumption of drugs by elderly persons residing in nursing homes, as well as the prescription of benzodiazepines and analgesic-antiinflammatory drugs within old age.

Laryngeal carcinoma: sclerotic appearance of the cricoid and arytenoid cartilage--CT-pathologic correlation.

PURPOSE: To assess the significance of sclerotic-appearing cricoid and arytenoid cartilage with computed tomography (CT) in patients with laryngeal carcinoma. MATERIALS AND METHODS: CT scans obtained in 75 patients with laryngeal carcinoma were prospectively studied; laryngeal CT studies obtained in 50 patients without laryngeal carcinoma were retrospectively reviewed. RESULTS: Twenty-four of the 75 patients (32%) with laryngeal carcinoma had sclerotic cartilage. Evaluation of pathologic specimens obtained in 12 of these 24 patients showed 11 cases of sclerotic arytenoid cartilage and two cases of sclerotic cricoid cartilage. Tumor infiltration was demonstrated in six of these cases but not in the seven others. In 11 of the 12 cases with pathologic proof, however, tumor was adjacent to the perichondrium. In the 12 cases without pathologic proof, the proportion was similar. The positive predictive value of this sign for cartilaginous invasion was 46%. CONCLUSION: Although it is not a reliable sign of cartilaginous invasion, sclerotic-appearing cricoid and arytenoid cartilage in patients with laryngeal carcinoma is predictive of the tumor to this cartilage.

[Intrarenal reflux and kidney damage in essential vesicoureteral reflux].

We had reviewed our series of 59 patients with intrarenal reflux selected out of 596 diagnosed of vesicoureteral reflux. The number of renal units affected of intrarenal reflux was 74. We divided our patients into two groups, in an attempt to clarify the relationship between renal damage and urinary infection; a group include there was prenatal diagnosis of urinary tract dilatation and another group diagnosed for bacterial infection of urine after birth. Reflux nephropathy were present in 27 renal units; 9 from 30 units (30%) in the prenatally diagnosed group, and 18 from 44 units (41%) in the other group. Reflux nephropathy was diagnosed by intravenous pyelogram and/or radionuclide renogram. We concluded that intrarenal reflux (IRR) is relatively frequent in our population and that 30% of patients prenatally diagnosed had renal damage without urinary infection, landing support to the previously published existence of a prenatal origin for some newborn.

Detection of creatine kinase isoenzymes as tumoral markers of rhabdomyosarcoma.

We analyzed the expression profile of isoenzymatic fractions of creatine phosphokinase (EC 2.7.3.2) isotypes MM, MB and BB in three cell lines derived from embryonic rhabdomyosarcomas and a normal counterpart cell line. Electrophoretic data showed that the BB fraction was consistently expressed de novo, in contrast with its counterpart in normal tissue. The BB fraction may serve as new tumoral marker for the diagnosis of rhabdomyosarcoma. In addition, the appearance of macrocreatine kinase type-1 in this type of neoplasm may serve to reinforce the diagnosis when rhabdomyosarcoma is suspected.

[Diurnal variations in circulating leukocytes and subsets: relation to plasma cortisol and ACTH].

Surface cellular antigens of leukocytes, lymphocytes and corresponding subpopulations have been analysed by using monoclonal antibodies marked with fluorescein (PITC), parallel to those marked with phycoerythrin (PE). Cortisol and ACTH plasmatics have also been determined through RIA, on two samples at 8am. and 8pm. During this twelve hour evolution, a highly significant dependence of the leukocytes, T, T4 and T8 lymphocytes on the circulating ACTH has also been found. In general during the experiment time leukocytes, lymphocytes and subpopulations, have experimented an increase which is significantly related to the pituitary hormone secretion. The existence of this significant correlation establishes the presence of a possible mechanism that connects the cellular immunity to determined hypothalamus hormones.

[Prognostic significance of age in resectable cancer of the stomach].

From January 1, 1966 to December 31, 1991, 137 patients with carcinoma of the stomach underwent standard radical gastrectomy at Belen Hospital, Trujillo, Perú. The patients were classified into two groups--those with age < or = 40 years (n = 19) and those with age > 40 years (n = 118)--and we analyzed herein comparatively clinicopathologic features and five year survival rates using the Mantel Haenszel test. In patients aged 40 years and younger there was a significant increased in the number of patients with carcinomas present in the upper two-thirds of stomach (p < 0.01) and in undifferentiated type adenocarcinoma in histology (p < 0.01) compared with patients more than 40 years old. There were no statistically significant differences between these two groups of age with regard to sex, abdominal mass, size of tumor, type of cancer (early vs advanced carcinoma), Borrmann's criteria, depth of invasion (T), nodal involvement (N), clinical stage (UICC, 1987), type of operation, curability, operative death and five year actuarial survival (8.0 percent vs 6.0 percent, respectively) (p > 0.05). In this study, carcinomas in young people were found to have a higher incidence of undifferentiated forms pathologically and the majority occurred in the body and after gastric resection were similar in both groups of patients.

[Predisposing factors, clinical picture and mortality in volvulus of the small intestine].

This retrospective study evaluated predisposing factors, clinical picture and the methods of treatment related to morbidity and mortality of 19 small bowel volvulus (SBV) who underwent operation at Belen Hospital (Trujillo-Peru) during the last 26 years (1966-1992). The SBV was 1.6% of all cases of intestinal obstruction in this period and 10.8% of all intestinal volvulus. The median age was of 43 +/- 20.5 years (range, 6 to 78 years) and the majority of them were between 41 and 60 years. Sixteen cases (84.2%) were men from Indian and Spanish extraction and most of them were farmers and came from the Sierra of the Department of La Libertad. Two cases (10.5%) had non-related antecedents previous surgery. In six patients (31.6%) the volvulus was less than seven day's duration and in thirty (68.4%) it was more eight day's duration with previous attacks of obstruction (median: 19.3 days, range: 17 hours to 94 days). Pain, vomiting and distention were present in almost all of these cases. The most frequent abdominal finding was distention. The location of the volvulus was: ileum, 12 cases (63.2%), root of mesentery, 4 cases (21%) and jejunum, 3 cases (15.8%). Gangrenous bowel was present in six patients (31.5) and gangrenous intestine with perforation in two cases (10.5%) who underwent resection of the involved segment with primary anastomosis. In this group one patient (5.2%) died of sepsis and the wound infection rate was of 37.5%. There was no statistically significant correlation with the duration of illness and the presence of gangrenous loops or the mortality rate (p > 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[A meta-analysis of controlled studies of the treatment of chronic hepatitis due to the hepatitis B virus].

BACKGROUND: Since the results obtained in controlled studies of antiviral treatment of chronic hepatitis by the hepatitis B virus are frequently insignificant the authors have undertaken a combined study of the different controlled trials with the meta-analysis technique. METHODS: The results of 7 controlled trials of treatment with adenosin arabinoside (AAR) and 18 published studies on therapy of a control group with interferon were studied. Meta-analysis was performed by two methods: Mantel expressing the results in odds ratio (OR) and Cochram-Dersimonian-Laird which consider the results in differences of risk (RD). RESULTS: The meta-analysis of AAR treatment shows a mean difference of response of 11% with no homogeneity among the different studies published. This lack of homogeneity may be due to the different selection criteria of the patients which determine very variable degrees of response. The controlled studies with interferon showed a mean difference of response of 20% with homogeneity of effects. No differences were observed in the degrees of response according to whether recombinant lymphoblastoid interferon was used or if the dose was of 5 or 10 megaunits per day. Non were differences observed in degrees of response with respect to the time in which the loss of viral DNA was evaluated. CONCLUSIONS: Following analysis of the results it was concluded that interferon is clearly effective in the treatment of chronic hepatitis B and based on published studies adenosin arabinoside demonstrate less efficacy.

[Chronic necrotizing pulmonary aspergillosis caused by Aspergillus niger].

Chronic necrotizing pulmonary aspergillosis (CNPA), also known as semi-invasive pulmonary aspergillosis , is a recently defined entity. CNPA is characterized by a pulmonary infiltration with cavitation of chronic evolution in patients with chronic pulmonary disease, slight immunodeficiency or healthy patients. Good evolution is obtained with antimicotic treatment. The isolation of Aspergillus niger as a cause of CNPA is infrequent and may bear worse prognosis. A patient who presented CNPA by Aspergillus niger is described. The patient had received radiotherapy for epidermal carcinoma of the esophagus. Three other cases have been reported in the literature. The diagnostic aspects, treatment and prognostic factors of CNPA are commented upon.

Cloning, sequencing, and expression of a cDNA encoding rat LIMP II, a novel 74-kDa lysosomal membrane protein related to the surface adhesion protein CD36.

LIMP II is a glycoprotein expressed in the membrane of lysosomes and secretory granules with lysosomal properties. Sequence analysis of a CNBr-cleaved peptide allowed the synthesis of a 47-mer oligonucleotide that was used to screen a rat liver cDNA library in lambda gt11. This resulted in isolation of a 2-kilobase cDNA containing 1,434 bases encoding the entire protein. The deduced amino acid sequence indicates that LIMP II consists of 478 amino acid residues. The segment spanning residues 4-6 to 26 constitute an uncleavable signal peptide. LIMP II possesses a hydrophobic amino acid segment near the carboxyl end, that together with the uncleaved signal peptide may anchor the protein to the membrane through two distant segments. The major portion of the protein resides on the luminal side and displays 11 potential N-glycosylation sites and 5 cysteine residues. Two short cytoplasmic tails, 2-4 and 20-21 amino acids long, correspond to the NH2- and COOH-terminal ends of the protein, respectively. Transfection of COS cells with the cDNA of LIMP II resulted in expression of the protein and its transport to lysosomes. Comparison of the entire sequence to various data bases of known proteins revealed extensive homology between LIMP II and the cell surface protein CD36 involved in cell adhesion. No significant homology was detected with the two families of lysosomal membrane proteins A and B, recently described.

The general stochastic model of nucleotide substitution.

DNA sequence evolution through nucleotide substitution may be assimilated to a stationary Markov process. The fundamental equations of the general model, with 12 independent substitution parameters, are used to obtain a formula which corrects the effect of multiple and parallel substitutions on the measure of evolutionary divergence between two homologous sequences. We show that only reversible models, with six independent parameters, allow the calculation of the substitution rates. Simulation experiments on DNA sequence evolution through nucleotide substitution call into question the effectiveness of the general model (and of any other more detailed description); nevertheless, the general model results are slightly superior to any of its particular cases.
