Soluble form of TRAIL, Fas and FasL in the serum of patients with B-CLL.

PURPOSE: Although many studies demonstrated expression of TNF family members in the course of B-CLL, there is a little known about relationships between soluble forms of these proteins. Furthermore, there is no study reported on effects of used therapy on this relation. The present study was designed to asses the relationships between the serum concentrations of sFas, sFasL and sTRAIL in patients with B-CLL regarding their correlation with clinical stage and used therapy. MATERIAL AND METHODS: We studied 40 patients with B-cell chronic lymphocytic leukemia (B-CLL) at diagnosis, before treatment and four weeks after therapy. To measure sFas, sFasL and sTRAIL levels in serum commercially available ELISA kits were used. RESULTS: We found increased concentrations of sFas in sera of all patients with B-CLL before treatment in comparison to the control group. There were no significant differences in concentrations of sFasL and sTRAIL between patients and control group. Increased sFasL concentrations after FC and CC therapy as well as decreased concentrations after 2CdA therapy in comparison to values before treatment were found. The concentrations of sTRAIL after FC and CC therapy were higher than those in patients before treatment. CONCLUSIONS: Results obtained suggest that relationship between sFas, sFasL and sTRAIL in sera of patients with B-CLL before treatment may facilitate the growth B leukemic cells. Changes in these relations after therapy with FC and CC can make a contribution to inhibit B cells growth on the apoptosis way in this patient group.

Collagen type I and III metabolism in assessment of mandible fractures healing.

PURPOSE: The aim of this work was estimation of the PIIINP, PICP and ICTP concentrations in blood serum during non-complicated mandible fracture healing; settlement of dependences between kinetics of changes of examined markers with reference to particular bone fracture phases and applied treatment methods; the determination of usefulness of collagen metabolism markers type III and I for the monitoring of mandibular fracture healing. MATERIAL AND METHODS: The material was blood serum of men aged 20-30 years, who were treated for mandible fractures in Maxillofacial Clinic Medical University of Bialystok. Depending on the treating method, examined patients were divided into two groups. Patients treated with non-operative method were I group (n = 31), II group was made of patients treated with surgery (n = 33). The concentrations of markers measured on the 2nd, 14th, 42nd, 90th day after trauma and in II group these substances were measured additionally on the 2nd and 14th day after surgery. Control group consisted of 20 healthy men the same age. Concentrations of markers were measured with the radioimmunological method (RIA). RESULTS: Regular process of mandible fracture healing in men in various periods occurs with PICP, PIIINP and ICTP concentration changes in blood serum. CONCLUSIONS: Changes of maker concentration show that, mandible fracture healing treated non-operatively is a more dynamic process than stable osteosynthesis method applied. Lack of positive correlation of the PIIINP and PICP concentration in blood serum of patients in two examined groups can indicate on the different mechanisms of mandible fracture healing connected with different methods of the treatment.

Cytokine and adhesion molecule concentrations in blood of patients with B-cell chronic lymphocytic leukaemia with regard to disease progression.

PURPOSE: To examine concentrations of IL-8, E-selectin and VCAM-1 in blood plasma, culture supernatant and isolated broken lymphocytes in patients with chronic lymphocytic leukaemia (CLL), at I and III stage of the disease according to Rai's classification and in healthy individuals constituting controls. Two types of lymphocyte cultures were carried out--nonstimulated and stimulated with mitogen. MATERIAL AND METHODS: A concentration assay of substances mentioned above was made using ready immuno-enzymatic sets of the ELISA type. RESULTS: In all cases, plasma concentrations of IL-8, E-selectin and VCAM-1 were the highest in III stage of CLL and moderately increased in I stage comparing to control. The concentrations of IL-8 in culture supernatants and in broken lymphocytes and concentration of VCAM-1 in lymphocytes were increased in both stages of CLL. In contrast concentrations of E-selectin were decreased in lymphocytes in both types of culture. Significant decrease of E-selectin and VCAM-1 concentrations were observed in supernatants of nonstimulated cultures. CONCLUSION: Significant increase of E-selectin, IL-8 and VCAM-1 concentration in blood plasma may support higher activity and suggest the higher ability of leukaemic lymphocytes to migration. Increased IL-8 concentration in isolated, broken, stimulated lymphocytes may be a characteristic feature of the biochemical processes of leukaemic lymphocytes. The lowest values of E-selectin and VCAM-1 concentrations in culture supernatants and broken lymphocytes may suggest their degradation during in culture.

Cell Adhesion Molecules↗

[Some adhesion molecules in blood of patients with chronic B-cell lymphocytic leukemia].

Adhesion molecules actively participate in all stages of leukocyte migration, directly or indirectly by means of appropriate ligands. Therefore the aim of study was the determination of concentrations of L-selectin and ICAM-1 in cell culture supernatante, broken lymphocyte and plasma. The measurement of cell adhesion molecules (CA) concentrations were performed by means of immunoenzymatic kits of ELISA type in the patients with chronic lymphocytic leukaemia in the clinicals stages I and III. The levels of L-selectin were increased in cell culture supernatante but in the plasma concentrations of L-selectin were decreased. The behaviour of concentrations of studied substances in the broken lymphocytes was differently. It is possible that these investigations may appear useful in the future as an additional diagnostic indecks.

Soluble selectin profiles associated with severe trauma.

Severe trauma acts as a trigger for the complex cascade of postinjury events leading to the release of different mediators and the development of generalized inflammation. Selectins are a family of adhesion proteins that are responsible for the adherence of polymorphonuclear neutrophils to the endothelium. This interaction plays an important role in the development of severe complications after multiple trauma. The aim of the present study is to follow the sequential alterations in circulating selectin levels after severe injury and to evaluate the clinical significance of these mediators in monitoring prognosis and outcome. Thirty four severely traumatized patients were entered into the study. Serum sE-selectin, plasma sP-selectin and sL-selectin concentrations were measured and an APACHE II score was calculated on admission to the intensive care unit and during the subsequent 5 days. The patients were divided into survivors and nonsurvivors. Initial soluble P- and E-selectin concentrations were significantly elevated in all trauma patients. The highest values of these adhesion molecules were measured in all the observed days in patients with poor prognosis and outcome. In survivors we found a systematic decrease in the sP-selectin concentrations. On admission, the sL-selectin concentrations in all trauma patients were decreased. There were stable, very low values in nonsurvivors and a slow increase in circulating L-selectin in patients who survived. The pattern of soluble selectins in patients with severe trauma is characterized by increased levels of P- and E-selectin and a decreased concentration of L-selectin. These findings suggest a widespread microvascular endothelial activation on injury in the early posttraumatic period, which may be associated with increased neutrophil-endothelial adhesion, neutrophil extravasation and migration. We suppose that these parameters of endothelial cell activation/injury may be useful as another early prognostic factor in severe trauma.

The concentration of C-terminal propeptide of type I procollagen in blood serum in the course of mandibular fracture healing (preliminary report).

One of the basic measures of quality of repairing processes of soft tissue and bone injury is collagen synthesis. C-terminal propeptide of type I procollagen (PICP) is admitted index of type I collagen biosynthesis. The aim of the study was estimation of PICP concentrations in the course of fracture repair process in relation to the treatment method. The material of investigations was blood serum of 25 men (age 20-30 years) with fracture of mandible. The patients were divided into 2 groups depending on the method of treatment: I group was patients treated non-surgical method (n = 12); II group--surgical method (n = 13). Blood samples in the first group were taken on 2nd, 14th, 42nd, 90th day of healing process and additionally on 2nd and 14th day after surgery--in the second group. The PICP concentrations in blood serum were determined by RIA method. Statistical analysis of results shown significant increase of PICP concentrations in I group of patients in 14th (167.1 +/- 42.8 micrograms/L) and in 42nd (216.0 +/- 59.1 micrograms/L) day of healing process in comparison to the values in the second day after injury--the first study (124.3 +/- 41.8 micrograms/L). In the II group of patients the statistically significant increase of PICP concentration in 14th day after surgical procedure (156.9 +/- 30.5 micrograms/L), 42nd (163.1 +/- 35.3 micrograms/L) and in 90th (153.1 +/- 40.2 micrograms/L) day of healing process after injury in comparison to the values in the second day after injury--the first study (119.9 +/- 35.8 micrograms/L). The results indicate, that a normal course of mandibular fracture healing influences the change in PICP concentrations in serum in examined men. A different dynamics of PICP concentrations, observed in the serum in patients with mandibular fractures treated surgically and non-surgically, suggests various mechanisms of bone tissue reconstruction connected with the method of treatment.

Insulin-like growth factor-I (IGF-I) in serum and bone tissue during rat mandible fracture healing.

Insulin-like growth factor-I (IGF-I) is potent stimulator of proliferation and differentiation of osteoblasts, the biosynthesis of collagen type-I and noncollagenous proteins and alkaline phosphatase activity. The role of IGF-I in bone repair has not as yet been clearly defined. The aim of the present study was the quantitative analysis of IGF-I in the serum and tissue in four phases of fractured jaws healing in rat models. IGF-I concentrations in the serum and bone extracts were determined by RIA. In respect to the control group (K) the significant increase of IGF-I occurred in the serum in phase I (211 +/- 68 ng/ml, K-153 +/- 50 ng/ml) (p < 0.05). At the tissue levels a statistically significant increase in IGF-I was confirmed in phase II (262 +/- 60 ng/g, K-182 +/- 56 ng/g) (p < 0.05). The present results demonstrate that in rat models with fractured jaws in the first two phases of healing elevated levels of IGF-I in the serum and bone tissue were observed which indicate the significant role of this polypeptide in the early healing stages.

[Diagnostic value of some cytokines levels in chronic myelogenous leukemia].

Some cytokines play an important role in aetiology and pathogenesis of leukaemia. Majority of data available in the literature relates to a single cytokine in the cell culture in vitro. In patients with chronic myelogenous leukaemia several different cytokines are probably active. Therefore, this study aimed to determine the concentrations of interleukin-1 beta, G-CSF, L-selectin in leukaemia cell culture supernatant, broken granulocytes and plasma in the course of disease exacerbation and remission. Cytokines have been assayed with available immunoenzymatic kits of ELISA type. IL-1 beta, G-CSF and L-selectin levels have been increased in cell supernatant but IL-1 beta level has been decreased in non-stimulated broken granulocytes. G-CSF levels have been low both in stimulated and non-stimulated granulocytes. In comparison to control the lowered levels of G-CSF and L-selectin have been observed in blood plasma of patients with CML. Different levels of assayed cytokines and adhesion molecule may suggest their contribution to leukaemia cells proliferation regulation.

[Iodine, sodium-iodine symporter and thyroid diseases].

The Na+/I- symporter (NIS) is the plasma membrane protein that catalyzes the accumulation of iodine into thyroid cells against an electrochemical gradient. This iodine uptake is the first step in thyroid hormone biogenesis which is stimulated by TSH--the main hormone regulating thyroid--specific functions. This process requires energy from the action of Na+/K+ ATP-ase. TSH upregulates NIS gene expression, and this induction can be modulated by cytokines. Analysis of the distribution of NIS mRNA demonstrated variable levels of NIS transcription in different tissue samples. Increased NIS expression has been found in autonomous adenoma and Graves' disease, decreased levels of NIS protein and/or mRNA NIS were observed in Hashimoto's disease, scintigraphically cold nodules and most thyroid cancers. NIS mutation has been found in congenital hypothyroidism. Potential defects in the NIS gene are currently under investigation in various thyroid diseases.

[Cytokines in acute myeloid leukemia].

The cytokines belong to the group of main factors regulating of leukaemia cell proliferation. Most of information comprised in the literature concern the behaviour of single cytokines in the cell culture in vitro. In the organism of leukaemia patient many different cytokines, adhesion molecules, growth factors and other substances probably act in the same time. Therefore the aim of study was the determination of plasma concentrations of interleukin 1B, 3, 4, 6, 8, G-CSF and P-selection in 26 patients with acute myeloblastic leukaemia-aml, among them 14 patient in the course of exacerbation-aml-e and 12 being in the remission-aml-r, classified as type M1 and M2 according to the FAB classification. Control group consisted of 15 healthy volunteers. The cytokine measurements were performed by means of immunoradiometric, immunoenzymatic and radioimmunologic kits. In the patients with aml-e significant increase of plasma concentrations of IL-1B, IL-3, IL-6, G-CSF, P-selectin and essential decrease of IL-4 was found. Significant decrease of IL-4 and P-selecting concentrations in the patients with aml-r and lack of changes in IL-8 concentrations in the both groups of patients was demonstrated. The observed differences of concentrations may additionally confirm the role of studied cytokines and P-selectin in the regulation of leukaemia cell proliferation.

Concentration of the cross-linked carboxyterminal telopeptide of type I collagen in serum of young growing rats fed a low calcium and vitamin D-deficient diet.

Type I collagen is the main type found in mineralized bone. Specific radioimmunoassay for the cross-linked carboxyterminal telopeptide of type I collagen allows assessing the degradation of type I collagen in serum samples. The aim of the present investigation was to determine the concentration of cross-linked carboxyterminal telopeptide of type I collagen in serum of dietary calcium and vitamin D-deficient rats, a good model disease of decreased formation and mineralization of bone matrix and excessive bone resorption. The studies were carried out on 20 young growing Wistar rats. Serum concentration of the cross-linked carboxyterminal telopeptide of type I collagen was analyzed by the Rat Telopeptide [125I]ICTP Radioimmunoassay Kit obtained from Orion Diagnostica (Finland). The data obtained from biochemical analysis showed increased concentration of the cross-linked carboxyterminal telopeptide of type I collagen in the serum of rats fed a low calcium and vitamin D-deficient diet after 14 days of the experiment. At the end of the experiment (day 21), the concentration of carboxyterminal telopeptide of type I collagen in serum was still elevated in these animals. In conclusion, dietary calcium and vitamin D-deficiency in rats produces hypocalcaemia together with the increased concentration of the cross-linked carboxyterminal telopeptide of type I collagen in serum.

[Radioisotopic methods of estimating perfusion and viability (metabolism) of the myocardium].

Radioisotope methods play a special role in cardiological diagnostics. At present, studies of myocardial perfusion also allow the estimation of cardiomyocyte metabolism. These methods are suitable for the detection of stunned, hibernating or scarred myocardium as well. The radiopharmaceuticals which accumulate in myocardium proportionally to regional blood flow (thallium-201, MIBI-Tc99m, teboroxime-Tc99m, tetrophosmin-Tc99m, furifosmin-tc99m) are used for the estimation of perfusion. In the nuclear medicine departments myocardial perfusion imaging can be performed with planar or tomographic (SPECT) methods. The techniques have high sensitivity (80%, 90%) and specificity (90%, 80%) for detection of coronary artery diseases. The radioisotopic methods have higher sensitivity than other noninvasive techniques (ECG sensitivity-60%, specificity-85%, ECHO sensitivity-81%, specificity-89%). Positron emission tomography is still the "gold" standard for the estimation of myocardial viability. This method uses substrates for different metabolic pathways (acetate-C-11, amino-acids-C-11, fatty acids-C-11, fluorodeoxyglocuse-F-18) labelled with positron emitters. Modifications in thallium protocols make estimation of myocardial metabolism possible. Recently MIBI-Tc99m has also been used in the assessment of myocardial viability. Results of these studies are of great clinical importance. They allow to predict a success of a surgical revascularysation and the return of normal myocardial function.

[Usefulness of computed tomography and scintigraphy in diagnosis of emphysematous bullae in the lung].

UNLABELLED: The x-ray examinations usually do not reveal morbid changes after lung expansion in the treatment of spontaneous pneumothorax. In our observation computed tomography (CT) and scintigraphy enable not only the exact determination of the extent of changes but also they disclose bullae invisible in conventional chest radiographs. 15 patients with cured spontaneous pneumothorax and 10 patients with radiographic evidence of a bulla or bullae were examined. CT scans showed bullae from 3 to 18 cm in diameter involving predominantly the upper lobes. 4 patients had additionally subpleural or intraparenchymal bullae of various degrees. In all patients with cured spontaneous pneumothorax, CT scans revealed intraparenchymal bullae, and in 6 cases bilateral intraparenchymal bullae were revealed. Only in sites of large bullae, no isotopic marker or its low elimination was shown in perfusion and inhalation scintigraphy. 10 patients with giant bullous emphysema were operated; in 6 patients enucleation of bullae, in 3 lobectomy and in one patient bullectomy were performed. CONCLUSION: CT is a method of choice in the diagnosis of lung emphysematous bullae and it enables the detection of the changes undetectable in chest radiographs. Perfusion and inhalation scintigraphy is helpful in the diagnosis of large emphysematous bullae and postoperative follow-up examination.

[The role of some cytokines in the physiology of proliferation of hematopoietic cells].

Cytokines are of great importance in the regulation of proliferation of haematopoietic system cells. Most of known cytokines stimulate the proliferation of cells of granulocyte-macrophage, monocyte, and lymphocyte system. Some of them stimulate the proliferation of cells from erythrocyte, eosinophil and megakaryocyte systems and proliferation of B and T lymphocytes.
