[Adhesion molecules and their role in pathogenesis of ARDS].

Adult Respiratory Distress Syndrome appears to develop as the acute lung injury in the course of many severe diseases, as the result of damage of alveolar-capillary barrier. There are many evidences, that neutrophils may participate in initiation and contribute to the severity of the syndrome course. Activated neutrophils demonstrate high adhesiveness to the endothelium. Leucocyte adhesion and further extravasation create the process named the adhesion cascade. In the course of this process, the adhesive molecules play the key role. Recent studies on animals shown, that anti-adhesive action of monoclonal antibodies and their chimeres decrease the degree of injury of alveolar-capillary barrier. Clinical observations demonstrate that analysis of dynamics of changes of E, P-selectin and ICAM-1 concentrations in the serum of patients with ARDS may be helpful in monitoring of the course and treatment of the disease.

[Examination of interleukin levels and growth factor from blood platelets in leukemia].

Plasma concentrations of interleukines 1, 3, 6, 8 and platelet derived growth factor (PDGF) were estimated in 45 patients with leukaemias. Among patients were 12 with acute myeloblastic leukaemia-AML (type M1 according to FAB classification), 9 with chronic granulocytic leukaemia-CGL, 10 with blastic crisis of CGL (CGL-BC) and 14 with chronic lymphocytic leukaemia-CLL (in 1 and 2 stage according to Rai classification). Additionally the concentration of IL-3 was measured in 10 patients with exacerbation of CLL. Control group consisted of 12 health volunteers. The concentrations of interleukines and PDGF were determined by means of radioimmunoassay or immunoradiometric assay. In the patients with AML, CGL-BC and CLL significant increase of concentrations of IL-1B, IL-6 were found. The patients suffers from CGL and CGL-BC had increase concentrations of IL-3, but patients with CLL-interleukin 8. The concentrations of PDGF were significantly decreased in the patients with AML, CGL-BC, and CGL. The differences of concentrations of studied interleukines can confirm their great significance in proliferation of leukaemic cells. The decrease of concentrations of PDGF in myelocytic leukaemias may reflect thrombopoietic disturbances in these illnesses.

Diagnostic and prognostic value of the new tumour marker CYFRA 21-1 in patients with squamous cell lung cancer.

We wanted to investigate the diagnostic and prognostic significance of serum CYFRA 21-1, especially in predicting the risk of recurrence in patients with operable squamous cell lung cancer. Serum levels of CYFRA 21-1 were measured using an immunoradiometric assay (CIS bio) in 76 patients with squamous cell lung cancer (64 operable and 12 with unresectable tumours), 22 with other non-small-cell type (12 with adenocarcinoma and 10 with large-cell type) and 45 with nonmalignant lung diseases. Elevated preoperative CYFRA 21-1 levels were identified in 63% of patients with squamous cell type (SqCC), 33% with adenocarcinoma, and 30% with large-cell carcinoma type. The diagnostic specificity of the assay was 96%. Positive CYFRA 21-1 levels were observed in 33% of stage I, 52% of stage II, 76% of stage IIIa and 83% of stage IIIb patients with SqCC type. Statistically significant differences were obtained between stages I and II and between II and IIIa, but not between stages IIIa and IIIb. Recurrence-free survival probability for patients with elevated serum CYFRA 21-1 levels before surgery was 63% (24/38) versus 92% (24/26) for patients with normal serum CYFRA 21-1 levels. However, the difference was not statistically significant when adjusted for the TNM stage (primary tumour, regional lymph node involvement, occurrence of distant metastasis). In 9 of the 10 patients with increased trend for CYFRA 21-1 during follow-up, elevated serum CYFRA 21-1 levels preceded (7) or coincided (2) with the clinical detection of tumour recurrence, providing a predictive value of an increased trend of 90%.(ABSTRACT TRUNCATED AT 250 WORDS)

["Prognostic markers" in serum of active and passive intravenous drug users infected with human immunodeficiency virus (HIV)].

The analyse of neopterin (NPT), beta 2-microglobulin (beta 2-M) and immunoglobulin A (IgA) in active and passive intravenous drug users was made to show the influence of intravenous drug taking on the level of so called "prognostic markers" in the serum of HIV infected persons. The intravenous drug users remained in the same, symptom-free stage of infection. The level of NPT in active drug users was higher than in passive drug users (statistically essential difference). In both analysed groups the level of NPT considerably exceeded the norm range (10 nmol/l). The differences in beta 2-M concentrations between analysed groups were not statistically essential, although higher values were observed in active drug users. The concentration of IgA in analysed persons were within the norm range. Current life conditions of the HIV-infected persons should be taken into consideration when estimating prognostic values of analysed indices.

Evaluation of CYFRA 21-1 as a new marker for non-small cell lung cancer.

The levels of the new tumour marker CYFRA 21-1 were assessed in 115 patients with non-small cell lung cancer (NSCLC) and in 45 patients with non-malignant lung disease. Increased levels of CYFRA 21-1 were observed in 47.8%, mostly in patients with squamous cell carcinoma (SCC; 69.1%). Serum CYFRA 21-1 levels were correlated with the stage of SCC type. Positive CYFRA 21-1 levels in patients with SCC were present in 40% of stage I, 61.1% of stage II, and 85.2% of stage III. In addition, SCC patients who presented mediastinal lymph nodes (N2) demonstrated higher serum CYFRA 21-1 levels, compared with patients without mediastinal lymph nodes metastases (N0 or N1). With regard to tumour size, significant difference was observed between T1, T2 and T3. The study also showed that the percentage of patients who survived 18 months with normal preoperative level of CYFRA 21-1 was higher compared with those patients with elevated preoperative levels of this marker, but the differences were not statistically significant.

Pulmonary permeability oedema. Methods of estimation of pulmonary capillary permeability.

Lung permeability oedema (acute respiratory distress syndrome-ARDS) is characterized by the increase of microvessels and epithelial permeability. In the estimation of pulmonary capillary permeability, the mathematical modeling is very useful. Some models and practical clinical methods concerning this problem are reviewed in this paper.

[Cyfra 21-1 new marker for non-small cell lung cancer].

The aim of the study is to estimate the new tumor marker CYFRA 21-1 in non-small cell lung cancer (NSCLC) patients and comparison of this results with SCC-Ag. The investigation was carried out on 115 NSCLC patients (55 with squamous cell, 35 with adenocarcinoma, 25 with large cell) qualified for surgical treatment and in 48 nonmalignant lung diseases patients. CYFRA 21-1 was determined by the means of IRMA method (CIS bio international--GIF-SUR-Yvette, France) and SCC-Ag--MEIA method (IMx system Abbott). Elevated levels of CYFRA 21-1 were obtained in 48.7% and SCC-Ag in 39.1%. Elevated levels of examined markers most frequently occurred in squamous cell type (SCC). It was found out that CYFRA 21-1 dependent on: a) SCC stage (I-40%, II-61.1%, III-85.2%), b) tumor size (T1-38.4%, T2-73.1%, T3-87.5%), c) mediastinal lymph nodes metastases (No and N1-53.8% and N2-86.9%). Similar correlations were not observed in SCC-Ag examination. Simultaneous determination of CYFRA 21-1 and SCC-Ag showed minimal sensitivity increase from 48.7% to 52.1% in NSCLC and from 69.1% to 70.1% in SCC and decrease of specificity from 95.8% to 85.4%. To sum up, determination of CYFRA 21-1 in NSCLC patients (especially in SCC patients) is useful in diagnosis and clinical stage determination.

[Levels of neopterin in serum of narcotics abusers infected with human immunodeficiency virus (HIV)].

Concentration of neopterin (NPT) was measured in 3-6 months intervals in the serum of HIV-infected drug users taking continuously intravenous drugs and those who stopped the habit (MONAR, ZOZ). After 6-12 months of observation the mean NPT concentration was higher in the active drug users what was statistically essential. The outcome of our study indicates that current state of the patient concerning taking of drugs must be taken under consideration when NPT concentration is assessed as marker of progression of HIV disease.

[Value of plasma testosterone, carcinoembryonic antigen and CA 19-9 in the differential diagnosis of pancreatic carcinoma and chronic pancreatitis].

Diagnostic value was assessed of serum testosterone concentration and compared with that of serum assay of CEA and CA 19-9 in the differential diagnosis of pancreatic cancer (PC) and chronic pancreatitis (CP). Thirty-six patients with PC were compared with thirty-two CP patients. The sensitivity of CA 19-9 (76.9%) in detecting PC was greater than that of testosterone and CEA (30.6% and 30.8%, respectively). The specificity of testosterone and CA 19-9 were comparable (93.7% and 96.4%, respectively). The combination of tests did not enhance the sensitivity and specificity of each test when used alone. The serum CA 19-9 concentration in PC patients was significantly higher then in patients with colon cancer, gastric cancer and benign gastrointestinal diseases.

Chondroprotective action of salmon calcitonin in experimental arthropathies.

To assess whether calcitonin exerts an influence on cartilage, three models of arthropathies in rabbits--representing three different modes of cartilage destruction--were used: (1) corticosteroid administration (endocrinological disturbances model); (2) meniscectomy (mechanical stress model); and (3) immobilization of the hind leg (nutritional disorder model). After 12 weeks of methylprednisolone (MP) administration, the rabbit femur heads displayed cartilage erosions, marked decrease of glycosaminoglycans (GAG) content, and narrowing of joint spaces. Elevation of serum uronic acid, activity of alkaline phosphatase, and calmodulin content was evident. All these changes were minimal--close to normal--in the group treated for 12 weeks with MP + salmon calcitonin (sCT). Partial meniscectomy and hind leg immobilization caused statistically significant loss of GAG from the cartilage and narrowing of the knee joint space during the same experimental period, 12 weeks. In both these models the groups of rabbits treated simultaneously with sCT showed only insignificantly smaller joint spaces and GAG content. These results support our hypothesis of a chondroprotective property of calcitonin. However, the mechanism through which calcitonin influences joint cartilage remains unknown. A direct effect of calcitonin on cultivated chondrocytes, as well as the role of calmodulin, beta-endorphins, calcium, and interleukin-1 in the process are discussed.

[The 131I-fibrinogen turnover during activation of blood fibrinolysis].

The influence of stanozolol Stromba and phenformin Dibotin (Winthrop) and complamin (Wulfing) on transformation of 131J-fibrinogen administered intravenously was examined in 39 patients with arteriosclerosis obliterans. The patients were divided into 3 groups; one of the groups included untreated patients. Radioactivity of the plasma and of the urine was determined by means of a scintillation counter. The volume of plasma, plasma fibrinogen (g%, g, g/kg), half time of persistance (T 1/2) and degradation of fibrinogen were determined. The patients who received Dibotin and Stromba simultaneously showed statistically significant decrease of the plasma fibrinogen degradation im comparison to the control group. But in patients who received complamin in spite of significant decrease of the plasma fibrinogen (g%), the fibrinogen degradation was not different from the value obtained in the group.
