[VATS resection of peripheral pulmonary nodules following CT-guided cytologic assessment].

In a thorax surgery domain the V.A.T.S. is the important new treatment for particular thorax diseases. authors would like to show the advantages using V.A.T.S. for the therapy of peripheric lung nodules, also using a CT-guided extemporary cytologic tipization. The CT-guided biopsy and then an extemporary cytologic tipization, represent the first steps for lung nodules candidated to the V.A.T.S. These lesions have got singular aspects: benign lesions, methastasts, specific and unknown lesions. In that way traditional surgery is for primitive lung neoplasm in order to have a certain oncologic solution and because it is possible to test the complication of large resection. The CT-guided cytologic tipization can be done by a particular kit invented from the authors: a flessible needle works like an anchor and is a good repair of a nodule in order to proceed with V.A.T.S. This method has been utilized for 45 patients with good results.

[Process approach experience of university trust].

This job has the scope to make to comprise the importance of the quality system applied to a complex organization describing the phases that have characterized the definition and the improvement of one organizational-managerial procedure for the prevention and the treatment of a problem of great importance in the within of the care process (ulcer pressure), demonstrating as the personnel involvement is one of change strategy used .

[Nephropathy in rheumatoid arthritis].

Occurrence of renal involvement in Rheumatoid arthritis is uncommon; when present it has been attributed to amyloid or secondary effects of drugs. Nevertheless, recent observations seem to indicate the existence of primary renal lesions associated with the disease. The purpose of this paper is to review the data suggesting that the kidney can be directly involved in Rheumatoid arthritis. The main points of interest are the occurrence of glomerulonephritis and/or amyloidosis, the presence of vasculitis and other arterial changes, the existence of chronic interstitial nephritis and papillary necrosis and lesions associated with gold and penicillamine therapy.

Urothelial cell changes due to busulfan and cyclophosphamide treatment in bone marrow transplantation.

Since the administration of cyclophosphamide and busulfan can cause hemorrhagic cystitis and changes in urothelial cells, an investigation was carried out to see whether patients undergoing bone marrow transplantation (BMT) who were treated with these drugs showed such urothelial changes and whether exfoliative urinary cytology can contribute to the early diagnosis and monitoring of such changes. Morphologic and morphometric analyses were performed on cytocentrifuged, Papanicolaou-stained preparations of 700 samples from 107 patients. Various degrees of urothelial cell changes were found in 30.8% of the cases. These changes consisted mainly of a considerable increase in the size of the nucleus and of a cytoplasm that was often bizarrely shaped. Even the structures of the nucleus and the cytoplasm changed. The results of this study showed that exfoliative urinary cytology permits an early diagnosis and monitoring of urothelial cell changes related to the administration of busulfan and cyclophosphamide in connection with BMT.

Urinary cytologic abnormalities in bone marrow transplant recipients of cyclosporin.

The role of urinary cytology in the early diagnosis of cyclosporin nephrotoxicity was studied in 20 bone marrow transplant recipients. There was an evident cyclosporin cytopathy in 35% of bone marrow transplant recipients examined, consisting mainly of degenerative and necrotic abnormalities in cells of the proximal convoluted tubules seen in urinary samples. These findings regressed to normal after reduction of the cyclosporin dose. Urinary cytology proved to be a useful method for the early diagnosis of cyclosporin nephrotoxicity.
