A cross-validation of the European Organization for Research and Treatment of Cancer QLQ-C30 (EORTC QLQ-C30) for Japanese with lung cancer.

The EORTC QLQ-C30 was developed in English-speaking cultures. To determine if this instrument could cross a broad cultural divide and be used in Japan, the cross-cultural validity of its Japanese version was estimated. In evaluating psychometric testing, internal consistency by Cronbach's alpha, item-discrimination by multitrait scaling analysis, and validity analysis with ECOG performance score (PS) and Karnofsky Performance Status Scale (KPS) were performed. The QLQ-C30 (version 1.0) was given to 105 patients with lung cancer. Although the response rate was low in patients with PS 4, the questionnaire was well accepted by patients with PS 0-3. The Japanese QLQ-C30 has a weak scale of role functioning in terms of item discriminative validity. It also has a weak scale of cognitive functioning in items of discriminative validity and internal consistency. However, known-groups comparison showed the expected clinical validity with PS for all the scales except for financial impact, and longitudinally clinical validity with KPS was shown in scales of cognitive functioning, fatigue, and nausea and vomiting. Multitrait scaling analysis showed that the predicted scales constituting quality of life (QOL) in the English-speaking culture were extracted from the Japanese QLQ-C30, and found to be valid in Japan, indicating its possible usefulness as an instrument that is universally applicable across cultures.

Aortic valve replacement through the transected aorta in a patient with aortic stenosis and a calcified ascending aorta.

This report describes a surgical approach of aortic valve replacement in a patient with a calcified ascending aorta, calcified aortic valve stenosis and coronary artery disease. The aortic valve was replaced with a 19 mm St. Jude Medical prosthetic valve through the transected aorta while the distal ascending aorta was cross-clamped at a narrow but not calcified band approximately 4.5 cm distal to the aortic anulus. These procedures were successfully done and no neurological deficit was found after surgery. The aortic valve replacement through the transected aorta may be one of the alternatives in selected patients with porcelain aortas and calcified aortic valves.

[Pharmacokinetics of PF-402, sustained-release morphine capsule, in cancer patients with pain].

A once-daily dose of PF-402 60 mg and twice-daily doses of sustained-release morphine sulfate tablets (MSC) 30 mg were repeatedly administered in cancer patients in a cross-over design. Their plasma concentrations were measured, and the pharmacokinetics of PF-402 and MSC were compared. A total of 7 subjects in the study were taking commercially sold MSC 60 mg daily (30 mg twice-daily) prior to the study and had "mild" or "no" pain at the start of the study. Plasma morphine concentrations of PF-402 were longer-lasting and showed smaller fluctuations than those of MSC. Repeated administration of the same daily doses of PF-402 and MSC produced similar plasma concentrations for periods of 24 hr and 12 hr. PF-402 administration produced a Tmax of 7.4 hr, and an MRT of 9.8 hr, all longer than those with MSC. Moreover, no significant difference was observed in AUC between PF-402 and MSC. These results indicate that the sustained-release characteristics of PF-402 are superior to those of MSC, and that the two drugs have a similar absorption pattern. Adverse drug reactions (ADRs) were observed in all 7 subjects and consisted of 6 incidences of constipation, 3 incidences of nausea, 2 incidences of itching, and 1 incidence each of vomiting and somnolence. Study drug administration was not discontinued in any case due to ADRs, and no symptoms indicating physical or psychic drug dependence were observed. No abnormal laboratory values related to study drug administration were observed. The above results indicate that once-daily administration of PF-402 is sufficient to maintain plasma concentrations obtained with twice-daily administration of MSC. As the safety of PF-402 is confirmed, the drug is considered to be a useful sustained release formulation in the treatment of cancer pain.

[Postpartum depression and social support].

OBJECTIVES: To ascertain the state of a mother's depression three months after childbirth and to what sort of or to whose social support it is related. METHOD: In October 1993, a questionnaire survey was conducted on the attributes, state of depression by Zung Self-rating Depression Scale, and social support of 300 mothers who received health examinations of their 3 to 4 months' old infants at five health centers in Tokyo. Relationship between depression and social support of 256 mothers (rate of valid answer 85.3%) was examined by one-way analysis of variance, Pearson's product moment correlation coefficient and multiple regression analysis. RESULTS: The depression score averaged 37.3 points, with 73 persons (28.5%) scoring 40 to 47 points (light) and 27 persons (10.5%) scoring 48 or more points (medium level or higher). The following were the variables which individually showed a significant relationship to the depression score after controlling for age, education, number of children, type of family, and whether or not the mother was working: The emotional support score from the husband and his parents, such as the frequency in which the husband "listened to the mother's worries and anxieties" "was attentive or considerate to the mother" and "helped in feeding the child", the frequency in which the husband's parents "could be consulted on worries the mother had about childbirth, child care and child development" and "nursed and played with the child". It was found that the better the state of such support, the less the state of depression. On the other hand, support from the mother's parents, neighbors, and friends had no bearing on depression. CONCLUSION: The level of depression of the surveyed group was the same as that of the general female public. Postpartum depression was related to emotional support from the husband and emotional and practical support from the husband's parents. Therefore, from the aspect of preventing depression, we believe it is important that, firstly, the mother and family should understand the importance of support and improve the support by the family, and, secondly, the mother herself should improve her ability to cope.

[Using "Care Note" to measure the level of satisfaction patients feel with their care, in palliative cancer care, as a measure of their quality of life].

In this study, a new Questionnaire, "Care Note", was developed for palliative cancer care and it was validated in relation to the level of satisfaction in their daily lives felt by patients during supportive care. Care Note and EORTC QLQ-C 30 were administered to 97 patients with various forms of malignant disease (mean age 61, M/F 73/24). They filled in a Care Note questionnaire twice a week, on average they completed 6.77 questionnaires (a total of 657 questionnaires). The average time to answer was 5.49 minutes, and patients compliance was good. Evaluation indices in the first examination (n = 97) were shown as: content validity, test-retest reliability, item-scale correlation revealed discriminant validity and convergent validity with internal consistency, using Cronbach in the range alpha = 0.695-0.814, were proved. Concurrent validity with EORTC QLQ-C 30 was satisfied. A multiple regression analysis showed that social well-being (p < 0.0001) and global quality of life (p = 0.0001) were explanatory valuables for expressing satisfaction in relation to daily life.

Central neurocytoma with lipofuscin granules--case report.

A 30-year-old male presented with a large calcified intraventricular mass. The tumor was completely resected via the transventricular route. Light microscopy showed the tumor was composed of small round cells with perinuclear halo, typical of oligodendroglioma. Electron microscopy revealed marked neuronal differentiation, including neurosecretory granules, well-formed numerous synapses, and lipofuscin granules. The tumor was diagnosed as well-differentiated central neurocytoma.

[Survey of allergic diseases in 3-year old children in Okinawa City].

A questionnaire survey was conducted between September 1993 and March 1994 to ascertain the presence of allergic diseases among 3-year old children in Okinawa City of Okinawa Prefecture. Nine hundred and twenty-two questionnaires were sent to caretakers of 3-year old children, with 697 responses of which five invalid cases were deleted for a total of 692 responses which were analyzed. About 30% of these children had allergic diseases as diagnosed by the medical doctor. Among the respondents 15.1% had experience of atopic dermatitis, while 13.9% had experience of asthma. The incidence of atopic dermatitis in this survey was lower than those reported in other prefectures of Japan. Symptoms for atopic dermatitis were more severe during summer. About 15% of the children had been on a diet regimen for prevention or treatment of allergic diseases since the time of conception. Eighteen percent of the respondents used a vacuum cleaner on their bedding. A high proportion of respondents (65.9%) obtained information from newspapers or magazines about allergic diseases.

Diagnosis of brainstem abscess in the cerebritis stage by magnetic resonance imaging--case report.

A 52-year-old male presented with a brainstem abscess manifesting as high fever, diplopia, and left hemiparesis. Magnetic resonance (MR) imaging with gadolinium diethylenetriamine pentaacetic acid showed the lesion as a ring-like enhanced mass consisting of a necrotic center with surrounding edema, whereas postcontrast computed tomography revealed no such confirmatory findings. He was treated with antibiotics as the lesion had been detected in the acute cerebritis stage. Serial MR images showed that the lesion decreased remarkably in size. MR imaging can detect brain abscess in the earliest inflammatory stage.

[A hemodialysis patient with HTLV-1-associated myelopathy (HAM)].

We report here a case of maintenance hemodialysis with HTLV-1 associated myelopathy (HAM). A 53-year-old female hemodialysis patient was admitted to Teikyo University Ichihara hospital because of paralytic ileus. She had a history of blood transfusion and had been under dialysis treatment for 8 years. She had experienced gait disturbance and sensory disturbance in her lower extremities for 8 years. Neurological examination revealed myelopathy and neuropathy. Laboratory data revealed that serum anti-HTLV-1-antibody was over 1260 x (PA) and liquor HTLV-1-antibody was over 160 x (PA). Her liquor revealed a cell count of 4/3 cmm, protein 22 mg/dl, glucose 45 mg/dl, chloride 125 mEq/l and no atypical cells. To the best of our knowledge this is a rare case in Japan. Our findings suggest that HAM should be taken into consideration when we diagnose a maintenance hemodialysis patient showing neuropathy and myelopathy.

Factors affecting functional prognosis in survivors of primary central nervous system germinal tumors.

We evaluated 79 patients of whom 62 survived treatment for intracranial germinal tumors between 1964 and 1992. The survivors were assessed for their Karnofsky scores and intellectual and reproductive functions to verify factors associated with better prognosis. Thirty-one tumors were pineal, 17 were suprasellar, and ten were in the basal ganglia. These included 50 germinomas, five teratomas, and seven tumors of extra-embryonic origin producing alpha-fetoprotein or human chorionic gonadotropin. Although patients with germinoma or benign teratoma had better Kaplan-Meier survival rates than those with extra-embryonic origin tumors, the mean Karnofsky scores of the survivors did not differ significantly among the three histologic groups. On the other hand, although survival rates did not differ significantly among different tumor site groups, the mean Karnofsky score of the pineal group was higher than that of the suprasellar or basal ganglia group. Intellectual and reproductive functions were severely affected in the basal ganglia and suprasellar tumor groups, respectively. The Karnofsky score of survivors treated after 1977 was higher than for those treated before 1977. The score of patients who received less than 55 Gy irradiation was better than those who received more than 55 Gy. No patient who received less than 45 Gy developed a recurrence, suggesting that a lower dose may suffice for pure germinomas. Tumors of extra-embryonic origin, however, should be treated with combined chemotherapy to minimize subsequent irradiation. Factors affecting survivors' capabilities are discussed.

[Depressive state and related factors in nursery teachers and guidance workers in homes for mentally retarded children].

A questionnaire survey was conducted in 1991 in order to elucidate depressive state among 352 nursing teachers and guidance workers working in 71 homes for mentally retarded children. The data were analyzed by means of Zung's SDS to study the causative factors in relation to personal attributes, conditions and variables pertaining to work, professional characteristics, and nearby people's expectations and mental support. The results obtained were as follows: 1) The level of depression in the subject population was not particularly higher than that in the general public or other professional groups. 2) The results of multiple regression analysis indicated that labor-related problems, including dissatisfaction with one's work and sense of being overburdened, constituted a factor that led to depression. The condition worsened in proportion to the excess in workload, work hours, and responsibility, irregularity of work shift, along with lack of a sense of fulfillment and teamwork. 3) The factor which counteracted depression was support and mutual reliance from colleagues and supervisors as well as from the children and their families. The more strongly a subject felt mentally supported by these people, and had someone to turn to for advice concerning personal problems, the lower his or her depression score tended to be. 4) Of the professional characteristics specific to the guidance of mentally retarded children, the factors leading to depression were not workload but dissatisfaction or doubt concerning one's own instruction and negative mental attitude toward the children and their families, that is, consciousness and emotion that partly resembled burnout syndrome.

Recent progress in cancer pain management and palliative care in Japan.

One out of every four deaths in Japan is due to cancer, so that health-care workers and the lay public have gradually become aware of the importance of cancer pain relief and palliative care in recent years. In 1984, the feasibility and effectiveness of the WHO method for relief of cancer pain was demonstrated in Japanese cancer patients. Thereafter, information on the latest knowledge and skills in cancer pain relief and palliative care has been disseminated through medical meetings, publications and mass communication networks. The national government published manuals of care for terminally ill cancer patients and amended narcotics regulation in order to improve the accessibility of opioid analgesics, especially morphine, to cancer patients with pain. These activities resulted in a 35-fold increase in the annual consumption of morphine preparations for medical purposes between 1979 and 1992. However, the annual consumption per capita is still much smaller than that in other developed countries, indicating the need for further information dissemination and professional education in the implementation of palliative care programmes.

[Evaluation of health education in health care services for the aged].

Evaluative classifications and rating scales as presently applied to health education were reviewed and summarized. In addition, evaluative activities and research directions in health education conducted as part of health care services for the aged were reviewed, and future challenges surveyed. 1. Evaluative classifications were divided according to the time of application between: A) evaluation of performance and B) evaluation of results. 2. Evaluative scales for Category A) were classified as: (1)quantitative volume of performance; (2)process; (3)fundamental supportive systems. Those for Category B) were classified as: (1)changes in knowledge, attitude, behavior; (2)objective state of health; (3)financial aspects; (4)subjective state of health, QOL; (5)social support. 3. On the basis of the previous studies, evaluative activities and research directions were examined in light of: evaluation of performance; evaluation of results; correspondence of performance to results; accuracy of evaluative scales; evaluative activities in practice. 4. In view of the foregoing findings, it appears that more efforts should be directed in the future to the study of "process" and "fundamental supportive systems" for Category A) and "subjective health, QOL" and "social support" for Category B). In addition, it is necessary to develop evaluative scales for assessing subjective changes and changes in knowledge, attitude, and behavior. Assessment of the validity of various scales is also required.
