Secretory meningioma: a case report with immunohistochemical and ultrastructural study.

Secretory meningioma is a new concept proposed with the progress on immunohistochemistry, and has not sufficiently been discussed in the literature. We report a case with this rare type of meningioma, in a 54-year-old female. The tumor had hyaline inclusions that showed not only carcinoembryonic antigen (CEA) on immunohistochemical study but some secreting organelles on ultrastructural study. The value of these findings are emphasized in the histological diagnosis for secretory meningioma.

[Convexity angiomatous meningiomas in a mother and a daughter without evidence of neurofibromatosis].

The convexity angiomatous meningiomas that occurred in a mother and a daughter without any evidence of neurofibromatosis (NF) were reported. The 73-year-old mother was admitted to our clinic because of an episode of generalized convulsion and a five-month history of gait disturbance. She had the signs/symptoms of intracranial hypertension and frontal lobe dysfunctions. Computed tomography (CT) revealed a left frontal enhanced mass with a small intratumoral cyst and a remarkable perifocal edema. Angiography showed tumor stain fed from the external carotid artery. Frontal craniotomy was performed and a dark red tumor was totally resected. The nodular-surfaced tumor had adhered loosely to the dura mater. The coarse vascular meshwork and an intratumoral cyst were observed on the cut surface. When the patient was discharged she was able to leave the hospital on foot, but she died of acute pancreatitis in the local hospital. Histological examination of the tumor showed rich vasculatures with focally whorl-formed cells. Most tumor cells had intracytoplasmic microcysts. The pathological diagnosis, WHO's classification, 1991, was angiomatous meningioma. The patient's 41-year-old daughter was admitted due to an episode of fainting. All laboratory data were within normal limits, including the normal karyotype of the peripheral blood leukocytes. Papilloedemata were the only signs of neurological deficit. A CT scan and magnetic resonance images showed a left frontal convexity mass and angiography displayed the tumor strains from the middle meningeal artery. The convexity meningioma similar to her mother's was totally removed. The histological diagnosis was angiomatous meningioma, again.(ABSTRACT TRUNCATED AT 250 WORDS)

[Fatigue symptoms and their related factors of workers in institutions for the mentally retarded].

This study examined the fatigue symptoms and their relative factors in a group of nursery teachers and guidance workers (n = 480) belonging to institutions for the mentally retarded. 1) More than 50% of the respondents complained of neck stiffness and/or backache. The mean scores of the physical symptoms were higher than the standard cumulative fatigue symptoms index, while those of the mental symptoms were lower. 2) Analysis of variance showed that physical fatigue symptoms were higher in females, younger workers, those with longer years of service, those with more night duties per month, and those who expressed more dissatisfaction with their co-workers, wages, working hours, and work shift. Physical fatigue symptoms were not related to the severity of the mental disease of their clients. 3) Analysis of variance also showed that mental fatigue symptoms were higher in females, younger workers, and persons who expressed more dissatisfaction with their jobs, supervisors, co-workers, wages, working hours, and work shift. The severity of the mental disease of their clients was directly related to the mental fatigue symptoms. Furthermore, multiple classification analysis showed that those working with children had both higher irritability and morale than those working with adults.

[Two cases of recurrent brain tumor during GH replacement therapy].

Two cases of patients with recurrent brain tumor are presented. Each of them received growth hormone (GH) replacement therapy for growth failure secondary to cranial irradiation. The first case is that of a 10-year-old girl who was treated with a combination of surgical resection, radiotherapy and chemotherapy for cerebellar medulloblastoma at 1 y.o. At the age of 9, 10 month after the beginning of GH replacement therapy, she complained of headache. This was due to shunt malfunction when CSF cytology was class V. It revealed that there was recurrence of medulloblastoma. The second case is that of a 14-year-old girl who was treated with a combination of surgical resection, radiotherapy and chemotherapy for suprasellar germinoma at the age of 10. This tumor completely disappeared after these procedures. For her growth failure, we started GH replacement therapy and after 1 year, she complained of lt. leg pain due to tibial and pelvic bone metastasis. In medical literature, we found 15 recurrent brain tumors during GH replacement therapy. These include our 2 cases, and 9 cases in which there was recurrence within 1 year. Recently, receptors for some somatomedins have been found in brain tumors. Although these numbers are too small for us to arrive at conclusions, we think it is possible that there are some mechanisms connecting GH replacement therapy and recurrence of certain brain tumors.

[A study of the relationship between job type and/or position to life style and health of male white-collar workers].

The relationship between the type of job and/or position of male white-collar workers to their working life, life style, health behavior, and health condition was examined based on data obtained from a survey of 1129 male workers in a major computer manufacturing company in 1988. The results were as follows: 1. Work, life style, health behavior, and health condition of male white-collar workers are strongly affected by type of job and position when adjusted for education, income, and age. 2. Workers in the sales department and engineering department appeared to have the most problems associated with working life, life style, health behavior, and health condition. Workers in sales, especially, displayed Type A behavior and their job was characterized by a lot of business transactions in a working environment that requires them to work until late at night. This leads to an irregular diet, late bed time, and makes their family relationships worse. Workers in the engineering department had work loads that were heavy because they have the longest working hours, and the content of their jobs is a mixture of both research and sales. Consequently their health is not good. On the other hands, workers in the laboratory had the best situation, and with general workers about average in terms of working life, life style, health behavior, and the state of health. 3. The department heads have the best conditions of life and health over all, except for their diet. The chiefs of sections are generally in the middle between the department heads and ordinary workers in terms of life and health. They drink and smoke heavily and show Type A behavior characteristics. Ordinary workers appear to have the most problems in working life, life style, health behavior, and health condition. From these results it may be concluded that this is associated with the difference in work type and position of these workers compared to managerial class and also with a low level of morale.

[Human herpesviruses infections (I)--EBV infection and immunity in cancer patients].

Human herpesviruses have been known to be reactivated from latent infection iin immunosuppressed individuals. To assess the immunity, antibodies to human herpesviruses (VZV, HSV, CMV and EBV) were measured in patients with gastrointestinal cancer and normal persons ranging from 40 to 80 in age. CF antibodies to herpesviruses except for EBV were negative in normal persons below the age of 60 and cancer patients aged 60 and over. The CF antibody titer higher than 1:64 occurred only in young cancer patients and older normal persons. The titer of VCA antibody to EBV ranged from 1:40 to 1:160 in more than 80% of normal persons and was higher than 1:640 in less than 7%. The ratio those with antibody negative or below 1:40 increased with the age. In contrast, the titer was higher than 1:320 in 70% of the patients, and the antibody was negative in none of them. In addition, the low titer occurred only in less than 5%. The rate of patients having higher VCA antibody titers increased with age, and the titer was high in more than 50% of patients in the 8th decade. In cancer patients, the higher the VCA antibody titer was, the less number of mature B cells, target cells of EBV, and the B cell functions were observed. This suggests that reactivation of EBV in hosts bearing carcinomas results in a fall of production of the specific antibody.

A study of oral adsorbent in chronic renal failure.

In order to examine the effect on the progression of chronic renal failure (CRF), we have applied an oral adsorbent (AST-120) composed of spherical porous carbon particles to patients with chronic renal failure undergoing conservative therapy. Its effect was observed in improvement of uremic symptoms, improvement of slope in linear regression of reciprocal of serum creatinine vs. time plots and delayed initiation of hemodialysis, compared to control patients, together with reduced uremic peak 2a in HPLC analysis of serum and lower levels of beta 2 microglobulin in AST-120 group than in control. The improvements of uremic symptoms, creatinine and 2a levels were confirmed in double blind study where background of patients were evenly randomized between AST-120 and placebo groups and no improvement was observed in placebo group. The result leads us to conclude that the oral adsorbent therapy is expected as an useful therapy for retardation of progression of CRF.

Changing attitudes towards narcotic use in cancer pain management in Japan.

Morphine consumption for medical purposes in Japan showed a 17-fold increase between 1979 and 1989, due to increased use in cancer pain management. This increase is a reflection of the improving attitude of the health care professionals and health policy makers towards narcotics use. The WHO Cancer Pain Relief Programme has ultimately become the basis for a national cancer pain relief programme. The Ministry of Health and Welfare amended the Narcotics and Psychotropics Control Law in 1990, to improve accessibility of morphine preparations to cancer patients with pain, and edited four manuals for palliative care, that include guidelines on cancer pain relief, and legislative management of narcotics use in hospital, clinic and pharmacy.

The incidence of cancer pain and improvement of pain management in Japan.

A nation-wide survey (1987) of cancer pain and analgesic methods showed that the incidence of pain in the terminal stage was in the range of 68 to 72% without any significant difference between hospital groups. Irrespective of the stage of illness, a certain analgesic effect was obtainable with oral/parenteral use of opioids. As a result of a year-to-year comparison of pain in cancer clinics, it was found that the rate of complete pain relief has increased for all stages of illness, especially in the terminal stage. Here the rate of complete pain relief steadily increased from 37.8% in 1986 to 42.7% in 1987 and 48.6% in 1988. Propagation of WHO-advocated cancer pain therapeutics has led to an improvement of the rate of pain relief in the terminal stage. Marketing of MS Contin tablets resulted in a dramatic increase in the consumption of morphine, but there was no increase in the rate of pain relief due to poor measures to counter adverse reactions, and to administration of morphine in insufficient doses. The consumption of morphine for medical use increased year by year, but a greater number of doctors experienced in the use of opioids would further improve the management of pain. To realize that, it would be necessary to incorporate terminal care into medical education programmes as soon as possible. Further efforts will be required for extensive research and propagation of analgesic methods in various fields including education in medicine, science of nursing and postgraduate education.

Distribution and co-existence of Met-enkephalin-like and mesotocin-like immunoreactivity in the neural lobe of the pituitary of the frog.

Content and distribution of Met-enkephalin (Met-ENK)-like immunoreactivity in the hypothalamo-hypophysial system of bovine, rat and frog were examined using specific radioimmunoassay and immunohistochemistry. Ultrastructural localization and co-existence of Met-ENK-, mesotocin (MT)- and vasotocin (VT)-like immunoreactivity in the neural lobe of the frog pituitary was examined by a method combining pre-embedding peroxidase-antiperoxidase immunostaining for Met-ENK with post-embedding immunocolloidal gold staining for MT or VT. The highest concentrations of immunoassayable Met-ENK were present in the neural lobe of the pituitary of the frog. In addition to nerve fibers showing only MT-like or Met-ENK-like immunoreactivity, nerve fibers containing neurosecretory granules showing both MT- and Met-ENK-like immunoreactivities were very rich. But VT-like and Met-ENK-like immunoreactivity was confirmed separately in different axon terminals.

[Prevalence of the antibody against human polyoma viruses (JCV and BKV) in aged persons].

JC virus (JCV) and BK virus (BKV) are known as small DNA type tumor viruses belonging to the human polyoma virus. The infectious state of these viruses has not yet been examined extensively in the aged persons; therefore, antibody measurements were made in 349 healthy volunteers from 20 to 90 years old and 383 cases of in-patients from 60 to 100 years old. As controls, measurements were made on the antibody titers to the herpes-type viruses which are known to be reactivated as the immunologic state of hosts decrease. The results obtained were as follows; 1) In the aged cases over 60 years old, the average HI antibody titers for JCV were significantly higher than that for BKV. 2) In the aged persons of the prevalence rate the high antibody titer (1024 less than) for JCV was also higher than that for BKV. 3) In the results with EIA (EIA test human polyoma viruses common antigen), the significant increase in the antibody titer was observed in the aged older than 70. 4) The average antibody titers of HSV and CMV, tended to increase as the age advanced until 60 and to decrease with age over 70. These results indicate that JCV is reactivated in a high rate in the aged (70 less than) persons.

Long-term prognosis of patients with congestive heart failure.

Factors determining prognosis in 100 patients with recent onset of congestive heart failure (CHF) were evaluated. The 1 year, 3 year, 5 year, and 10 year survival rates in the entire CHF group were 78.5%, 59.8%, 50.4% and 14.7%, respectively. No correlations between age, sex, heart rate and cardiothoracic ratio, and the cumulative survival rate were found. The prognosis of patients with CHF due to underlying coronary artery disease or primary cardiomyopathy was poor compared with that of patients with other types of heart disease. Patients whose NYHA classification was class III or VI had a significantly lower survival rate than those in class II. Patients with lower left ventricular stroke work and consecutive ventricular premature depolarization also had a significantly lower survival rate. These results suggest that functional status, underlying heart disease, left ventricular stroke work, and the presence of ventricular tachycardia provide important information regarding the long-term prognosis in patients with congestive heart failure.

Effect of endothelium-derived relaxing factor on the gastric lesion induced by HCl in rats.

Effect of HCl on the endothelium-dependent increase in mucosal blood flow and effect of endothelium-derived relaxing factor (EDRF) inhibitors or nitrites on the HCl-induced gastric lesion were studied to clarify the effect of EDRF on the formation of gastric lesion in rats. Topical application of 0.6 N HCl on the gastric mucosa inhibited the endothelium-dependent increase in mucosal hemodynamics estimated using organ-reflectance spectrophotometry induced by vagal stimulation or acetylcholine, but not by papaverine. Collagenase, gossypol, hemoglobin and ascorbic acid have been reported to inhibit the endothelium-dependent vasodilation, inhibited increase in mucosal hemodynamics induced by vagal stimulation and acetylcholine. These inhibitors and methylene blue significantly enhanced the gastric lesion induced by 0.45 N HCl. Intra-arterial or topical application of nitrites (sodium nitrite and isoamyl nitrite) increased mucosal hemodynamics. Oral administration of nitrites prevented the formation of 0.6 N HCl-induced gastric lesion. These results suggest that EDRF plays an important role in the protection of gastric mucosa against HCl. Reduced endothelium-dependent increase in mucosal blood flow may be an etiology of gastric lesion in rats.
