Vigorous achalasia. Manometric response to atropine and nitroglycerin.

A patient with vigorous achalasia demonstrated manometric response to administration of atropine sulfate and nitroglycerin. Clinical improvement was achieved with the use of an oral anticholinergic agent and long-acting nitrates. The relationship of vigorous achalasia and diffuse esophageal spasm is discussed.

Giant sigmoid diverticulitis. A cause of partial small bowel obstruction.

Partial small bowel obstruction developed in a patient with associated giant sigmoid diverticulitis. This complication resulted from the wall of the diverticulum adhering to a loop of terminal ileum. Small bowel obstruction had not been observed in the 15 cases of giant sigmoid diverticula reported in the literature.

Arrhythmias during upper gastrointestinal endoscopy.

Electrocardiographic monitoring of 52 consecutive patients undergoing upper gastrointestinal endoscopy revealed that arrhythmias during the procedures were common (38.5%) and tended to occur more frequently in the elderly (75%), in persons with heart disease (54.5%), and in persons with chronic lung disease (89%). The incidence of ventricular premature contractions in patient with no evidence of heart or lung disease was 19%. In one patient with advanced chronic lung disease, the procedure had to be terminated because of development of high-degree atrioventricular block. A high incidence of arrhythmias during endoscopy has not been previously reported in patients with chronic lung disease.

Candida fungus balls in the common bile duct. Unusual manifestation of disseminated candidiasis.

Disseminated candidiasis appearing as Candida fungus balls in the common bile duct developed in a patient with acute myelogenous leukemia. The patient had received broad-spectrum antimicrobial therapy for fever of unclear origin and subsequently manifested signs of liver cell dysfunction. A percutaneous transhepatic cholangiogram demonstrated three radiolucent defects in the distal part of the common bile duct, which proved to be fungus balls at operation. The liver was studded with microabscesses that disclosed C albicans on biopsy. The biliary tract fungus balls were removed surgically, and the patient was treated with 2,001 mg of intravenously administered amphotericin B over a 2 1/2-month period. No evidence of Candida infection is evident eight months after completion of therapy.

Alpha 1 antitrypsin deficiency and plasma thromboplastin antecedent deficiency--a new association?

We report a 74-year-old woman presenting with vaginal bleeding and hepatomegaly and was found to have plasma thromboplastin antecedent deficiency and alpha 1 antitrypsin deficiency. She also had liver and lung disease of the type associated with the antiprotease deficiency. The relative rarity of the two deficient states led to family and genetic studies. The possibility of a new genetic syndrome is entertained.
