Choroid plexus papillomas of the fourth ventricle. Report of 3 cases.

3 cases with benign choroid plexus papillomas (CPP) in the fourth ventricle are described. All patients were less than 2 months of age. They were among 14 cases with intracranial CPP in a series of 100 tumors of the posterior fossa. Correct diagnosis was not obtained by means of pneumoencephalography, with apparently normal repletion of the fourth ventricle being visualized. 2 of the 3 patients died after application of the ventriculoperitoneal shunt and before extirpation of the tumor. All 3 patients soon presented hydrocephalus in 1 case already 'intra utero'.

Cerebral Ventricle Neoplasms↗

Hydatid cyst: unusual cause of unilateral exophthalmos.

Intraorbital hydatid cysts are encountered unusually on neurosurgical services. Recently, 3 patients with intraorbital hydatid cysts have been studied by the Neurosurgical Service of C. S. "La Paz." In two of them, a computed tomography (CT) scan was performed; we think that these are the first cases of this type to be studied with CT scans. In each case, the CT scan showed a circular mass with a density similar to that of the eye. Frequency, localization, symptomatology, and neuroradiological aspects of intraorbital hydatid cysts are described. The different surgical approaches also are discussed.

[Dermoid and epidermoid skull cysts (author's transl)].

Thirteen children with dermoid and epidermoid cyst in the skull are presented (11 cases are dermoid and two epidermoid). Diagnosis was made between four weeks and seven years of life. A local prominence with lytic subjaccent osseous defect were observed in the zone of cystic implantation in every case. Surgery with total extirpation of the cyst was the treatment in all cases. Evolution without complications was observed in every patient.

[Intracranial teratomas of the newborn and childhood (author's transl)].

Two cases of intracranial teratomas are reported. Plain skull X ray is one of the principal procedures or diagnosis of these tumors, although CT scan usually showed the whole tumor. Cystic teratomas are low density masses and solid teratomas are high density masses. The behaviour of these tumors is different in the newborn and childhood. The former due to the size of these tumors usually has bad prognosis, but in childhood sometimes it is possible to achieve total removal. Regarding treatment, surgical approach is advised with postoperative radiotherapy in cystic teratomas and atypical teratomas.

Cerebral Ventricle Neoplasms↗

[Growing cranial fractures in infancy (author's transl)].

Seventeen cases of growing cranial fractures in infancy are presented, eleven males and six females. Seventy per cent of the cases suffered a traumatic episode before one year of life. Thirteen of the total group experienced surgical intervention in which a completely defective duramater and alterations of the leptomeninges and the subsequent cerebral cortex were encountered. Leptomeningeal cysts were found in nine patients or 70 per cent of the cases. Ventricular system revealed alterations in all cases that were studied by means of neumoencephalography or computerized axial tomography. Evolution of 15 of the cases was follow up for a period of time which varied between five months to eleven years of age. An analysis of electroencephalographical alterations and neurological sequelae is made. An uncommon form of clinical-electroencephalographic evolution is described in one of the cases.

[Cerebral tumours in infancy (author's transl)].

37 cases of cerebral hemispheric tumours are presented. These make up 23,1% of all the intracranial tumours observed over a period of 11 years in a Child Neurology Service. 19 cases were males and 18 females. Their ages were between 16 days and 7 1/2 years. Vomiting and headache were usually the first symptoms followed by seizures, frequently of the focal kind. Motor difficulties used to appear later. 69.6% of the cases presented a malfunctioning focus on the E.E.G. on the side of the tumor. The simple cranial X-Rays showed firstly widening of the fronto-parietal sutures, intracranial calcifications were seen in some of the cases with ependymonas and piloid astrocytoma in patients with Bourneville's disease. Pneumoencephalography as well as carotid angiography, radioisotope examination and computerized tomography gave us very positive results in the localization and determination of the size of the tumor. The ependymomas showed pathological vascularization regularly. The nature of the tumours corresponded to: 14 cases of ependymoma, 8 cases of astrocytoma I and II types, 1 case of astrocytoma of types III and IV, 3 cases of plexus papilloma, 2 cases of meningioma, 1 case of sarcoma of the basal ganglia, 1 case of teratoma, 3 cases of indifferentiated malignant tumours, 4 cases not proven. There was a 20% survival of patients five years later.

Age Factors↗

[Iatrogenic evolutive skull fracture (author's transl)].

A case of growing skull fracture secondary to a maxilofacial operation is reported. Frequency, clinical symptoms, phisiopathology and treatment of growing skull fractures are reviewed and the rarity of the iatrogenic mechanism is stressed.
