[Determination of serum vitamin B12 in hematologic diseases].

By means of a test set of the Isocommerz (GDR) determinations of vitamin B12 in the serum were carried out according to the principle of the competitive protein binding. The normal values lie between 200 and 1,000 pg/ml serum. Clearly decreased levels of vitamin B12 are found in the pernicious anaemia, in other megaloblastic anaemias and in disturbances of the resorption after resection of the stomach. Increased values can be stated in the untreated chronic myelosis and in the blast episode. Under Busulphan-therapy a significant decrease of the values of vitamin B12 develops. The method seems to be practically important for the well-timed recognition of deficiency conditions of vitamin B12 in beginning pernicious anaemia, in disturbances of intestinal resorption after resection of the stomach and for the observation of the course of the chronic myelosis.

[Prognosis of lymphogranulomatosis (stage III B-IV) treated with drugs--results of a retrospective 10 year study].

With the help of survival curves of 91 patients with histologically ascertained lymphogranulomatosis a report on experiences concerning the prognostic importance of different factors is given. The examination is based on a retrospective analysis of a well documented number of patients who were chemotherapeutically treated from 1967--1976 in the medical outpatient department and the medical clinic of the Erfurt Academy of Medicine. We found on the basis of the retrospective study that the following factors deteriorate the prognosis: Clinical stage IV, presence of general symptoms, type poor in lymphocytes. Older age at making the diagnosis as well as male sex also reveal a worse prognosis as to the 3-year-survival rate than the comparative groups. Furthermore it was shown that the quality of remission after initial chemotherapy is of decisive prognostic valency. When cytostatic combination schemata were used for the induction of the remission the survival rates could be improved. The problems of the remission-keeping chemotherapy are entered.

[Therapy of lymphogranulomatosis].

A survey is given on the therapy of lymphogranulomatosis. Keystones of the treatment are radiotherapy, antineoplastic chemotherapy and supportive measures of treatment. A therapy referred to diagnosis and stages including a clinical and pathological staging proves to be necessary in every case. The initial treatment consists in a radiotherapy in the stages I, II and III A, in a polychemotherapy according to the scheme MOPP and COPP, respectively, or CVPP in the stages III B and IV as well as possibly in a combined treatment in histomorphologically unfavourable forms also already in the early stages. The problems of the MOPP alternative programmes in primary failures, of the programmes of the post-MOPP-therapy in secondary failures and recidivations as well as of the remission-keeping long-term therapy need a further clarification. It is referred to the dangers and side-effects of an aggressive radio- and chemotherapy.

[Ischemia, hypothermia, length of death-perfusion and temperature related factors for preservation of organs].

In order to guarantee the comparability of the results of the various working teams and experimental conditions within the preservation of organs, above all of the preservation of kidneys, it is referred to the importance which belongs to the clear establishment of the used notions, such as time of hyperthermy (duration), time of ischaemia and dead time (warm and cold). For the practice of kidney taking simple time determinations between two steps of taking have made their way. The warm dead time is the duration between clamping of the renal vessels and the beginning of the flow with cold preservation solution. This establishment is justified by the standardised conditions of taking. For reserach these pauschal temporary data are not sufficient and the exact establishment of the notion used is necessary.

[Complications of urologic diseases in diabetes mellitus].

It is reported on complications of urological diseases in diabetes mellitus. Above all the diabetic late syndrome and its multiform and uncharacteristic symptomatology in the urogenital system are discussed in detail. The present form of diabetes control can not yet be regarded as optimum form. It should be striven for changes, since it is to be reckoned with an increase of diabetes mellitus all over the world.

[Our first results with cryotherapy on the prostate].

It is reported on our first experiences with the cryotherapy in 15 patients with adenomas of the neck of the bladder and carcinoma of the prostate who all on account of high quantities of residual urine underwent a permannt catheterisation and in whom an open operation of Tur were contraindicated due to severe concurring secondary diseases. 2/3 of the patients we could free from the permanent catheter, in 1/3 an undisturbed miction free from residual urine was possible In 1/3, however, the catheter had to be kept also after the end of the treatment.

[Severe Klebsiella serogenes infections].

It is reported on 2 cases of Klebsiella-septikaemia with difficult course. Germs of the group Klebsiella aerogenes could be proved microbiologically. On the basis of the peculiarities of the clinical course the author adopts a definite attitude to the development of the infection, the pathogenesis, to the microbiological proof and to the aimed antimicrobial chemotherapy. The antibiotic treatment was successful in the first case with karbenicillin and gentamycin and in the second case with cephalotin and gentamycin during 4 weeks when a penicillin allergy was present and following this with chloramphenicol and streptomycin during 3 weeks.

[Coagulation disorders following surgical intervention for urologic tumors].

Coagulation disturbances were discussed which appeared after operative removal of a hypernephroid carcinoma and teratocarcinoma. Despite taking into consideration preoperative coagulation tests after operation secondary haemorrhages appeared which could at first not be explained and which must be seen in the connection with the tumour metabolism changing the coagulation. Diagnostic and therapeutic propositions follow.

[Frequency of diabetes mellitus and nature of treatment in urologic diseases].

In a comparative period of 20 years is reported on the frequency of diabetes mellitus in urological diseases. It was found that 0.87% of the patients suffer from a concomitant diabetes. The peak of the disease is between the 60th and 70th year. As to the distribution of sex was established that the concomitant diabetes is to be found more frequently in males (ratio 2.4: 1). The lethality in diabetics with a urological disease is with 9.4% more than twice as high as in the other urological patients (4.3%). At the top of the immediate causes of death is the cardiovascular failure (30.7%), followed by the pulmonary blood clot embolism and the uraemic coma with 15.4% each. A diabetic coma never appeared. In the analysis of the urological diseases with concomitant diabetes the lithiasis (34.4%) is in the first place; then follow the adenoma of the vesical cervix (32.4%), the chronic relapsing pyelonephritis (12.9%), and the malignant tumours (7.1%). Many urological forms of diseases appeared combined. In the investigation of the complications without lethal exitus which appeared in 25.1% of all cases with concomitant diabetes the cardiovascular failure is again in the first place, then follow thrombotic diseases, urea-nitrogen disturbances. Peculiarities in conduction and treatment of the diabetes mellitus are shown and a close collaboration between several specialities is considered necessary.

[Strategy of antineoplastic chemotherapy].

After introductory remarks on the present state of the antineoplastic chemotherapy the experiences of the Erfurt treatment centre are described. In close cooperation between the haematologico-oncological department of the Medical clinic and the haematologico-oncological special ambulatory of the Medical policlinic of the Medical Academy Erfurt including the general practitioners from 1969 to 1977 altogether 745 patients with tumours, out of them 580 cases with haemoblastoses and malignant systemic diseases, underwent an intensive care. The Erfurt model well proved in practice and is a constructive basis for the further development of a complex strategy of the antineoplastic chemotherapy.

[Chemotherapy of microbial diseases of the respiratory tract].

It is given a survey on the chemotherapy of the microbial diseases of the respiration tract. A chemotherapy is taken into consideration only for such cases in which a bacterial etiology is ascertained or is assumed with a great probability. The antimicrobial chemotherapy is of particular importance for the acute and chronic bronchitis, bronchiectases, pneumonias, pulmonary abscess and empyema of the pleura. The author deals with the special problems of bacteriological findings of the respiratory tract. The choice of the chemotherapeutic remedy is done in most cases according to the empirical points of view, in which cases development of resistance, side-effects and changing of the infection must be taken into consideration. Apart from the otherwise usual oral and parenteral application special forms of application, such as aerosol therapy and intrapleural instillation are used. It is referred to atypical pneumonias, such as the mycoplasma pneumonia which gives a good response to tetracyclines. Apart from the sulphonamides and trimethoprim penicillins, chloramphenicol, tetracyclines, cephalosporines, more rarely streptomycin and linkomycin are the most important antimicrobiotics.

[Significance of prostatic biopsy in the diagnosis of chronic prostatitis].

It is reported on the importance of the prostate biopsy for the limitation of the prostate neurosis and the real chronic prostatitis. In own patients the tentative diagnosis "chronic prostatitis" could be proved histologically in only 8 out of 30 cases. We think the biopsy necessary in cases of chronic recurrent prostatitis, because the therapy depends extremely on the exact diagnosis and the complicating quota is relatively small in the prostate biopsy.

[Urethral carcinoma in the female].

It is reported on a case of the rare carcinoma of the female urethra. In the 68-year-old patient the tumour was removed by electrocaustic excision, a suprapubic incision into the bladder was made and local radiotherapy was performed.

[Increased insulin-sensitivity as a 1st indication of Addison's disease in a juvenile diabetic patient].

Case report on a 25-year-old patient with diabetes mellitus which is difficult to be controlled. Relapsing vomitting, loss of body-weight, retention of substances normally contained in the urine, hyperpotassaemia, hypernatraemia as well as an increased insulin sensitivity render an additionally existing morbus Addison probable. Confirmation by means of the determination of the Porter-Silver chromogenes. It is referred to the apparantly frequently not understood syntropy in diabetes mellitus and Addison's disease. An autoimmunological process is discussed.
