Indoprofen in rheumatic patients with acute episodes: a multicentre trial.

The results are reported of an open multicentre trial in 228 rheumatic patients with flare-ups. Fourteen centres adopting the same investigational protocol collaborated in the study. Indoprofen was administered for 1 week at a daily dosage of 1000 mg according to a treatment schedule used with success in acute gouty arthritis: a 400 mg i.v. bolus was followed by 200 mg (1 tablet) t.i.d. Subjective (pain) and objective variables were used for reliable assessment of activity. Marked reduction of pain intensity was already noticeable on day 1 of treatment and was followed by progressive improvement in subjective and objective variables for all the diagnoses considered. According to the patients' own overall assessments, results were good or very good in more than 50% of cases. The best outcomes were obtained in low back pain, acute gout and psoriatic arthritis. At the end of treatment only 7.4% of patients experienced no change or deterioration of symptoms. Adverse reactions, consisting mostly of mild and reversible gastrointestinal disturbances, were reported by 9.2% of patients, but only in 1.8% was treatment discontinued. Indoprofen administered according to the above schedule is an appropriate treatment for acute episodes of rheumatic diseases.

Posterior-approach osteosynthesis of the lower cervical spine by the R. Roy-Camille technique. (Indications and first results).

Four cases are reported of traumatic myelic lesions to the lower cervical spine (C3-C7) using the Roy-Camille technique. Three were complicated by tetraplegia, the fourth by upper right monoparesis. The optimal application for this technique is in stabilization of predominantly posterior lesions, which are far the most frequent: pure dislocations, fractures and fracture-dislocations of the articular apophysis, fractures with separation of the articular mass, most of the tear-drop patterns, some compound fractures, cervical distortions with lesion to the mobile segment of the spine. These metal implants have the advantage of being solid, of keeping stable the reduction obtained during surgery, of being applicable even in case of laminectomy or fracture of the laminae, of the spinous apophyses or the articular apophyses and, lastly, in case of tetraplegia, to allow abolition of the Minerva jacket in post-operative treatment.

[The treatment of varicose veins].

In the treatment of varicose veins and their complications most European phlebologists use today, unlike the past, surgery, sclerotherapy and compression without any preconceived preference, guided solely by the indications of choice of each method. Within this framework, it is generally admitted that surgery is by far the best treatment for incompetent saphenous terminations, main saphenous trunks and insufficient bulky perforating veins. Sclerotherapy, on the other hand, is given the task of eliminating the varicose tributaries of the removed saphenous conduits, as well for treating reticular non-saphenous varices, dermal varicosities and recurrent varicose veins after correct operations. As far compression treatment, it requires today respect for "posology" that is expressed in terms of mmHg of pressures to be exerted at different levels of the extremity; depending on venous and skin changes, semi-stiff sticky bandages or various types of elastic stockings will be chosen. Often two or more of the methods listed are used during the same treatment or in successive stages.

[Stereophonic apparatus: our hearing aid test].

The authors wanted to verify the advantages produced by binaural amplification in the loudness and in the speech identification. To value the improvement in speech identification they used the Jerger's test (1976); and to value the loudness improvement they measured, gain characteristics of the aid with monoaural and binaural amplification. They executed the test on 50 hearing aid users, chosen among 10 and 84 years old, suffering from sensorineural bilateral hearing loss between 40 and 80 dB. It results that : 1. The gain obtained by the use of binaural amplification is 5 dB lower than the one obtained by monoaural. 2. The binaural amplification permits a remarkable advantage in speech identification, the advantage is directly proportionated to deafness. The authors conclude that the described test is valid, simple and that it is possible to apply it by cheap instruments.

[Early diagnosis of pulmonary embolism by means of scintigraphy].

The occasions on which lung embolism is likely to occur and proceed with or without initial clinical symptoms are described. Lung scintiscanning technique, complemented by special expedients and personal experience, is explained. It is felt that the method helps in the early detection of suspect or clinically silent forms, as well as in the corroboration or ruling out of a clinical diagnosis as soon as this is possible. Scintiscanning is also of assistance in showing the anatomopathological features and extent of the process. The technique is simple and readily tolerated, even by patients with slight respiratory or circulatory disturbances. On the other hand, it is not competitive with conventional and well-tried angiopneumography.
