Arthroscopic arthrolysis of intra-articular knee stiffness.

The authors report 15 cases of intra-articular knee stiffness treated by arthroscopic arthrolysis with or without section of the alar ligaments. The advantages to using this type of treatment as compared to mobilization in narcosis or arthrotomic arthrolysis are emphasized.

[The biological levels of chloroform in the general population].

The paper reports the results of determination, via gas chromatography and mass spectrometry, of chloroform in expired air, blood and urine of a group of non-occupationally exposed blood donors. Chloroform concentrations were also measured for each subject in the air of the environment where samples were taken.

[Mechanisms and effects of recurrences following active treatment of insufficient perforating veins].

In this study we examine the mechanisms and effects of relapse of incompetence perforating veins occurring after sclerosant injections or surgical operations involving ligation or removal. Five relapse mechanisms are anticipated: 1) faulty surgical technique, 2) sclero-resistance or recanalization of injected perforated, 3) dilatation of healthy perforators as a result of persistent vertical saphenous reflux, 4) growth, forced by the same disorders, of newly formed perforators. There are also "pseudo-successes", when the treated perforator is part of a "deprived saphenofemoral circulation" and is destined to right itself spontaneously after the suppression of the vertical reflux which had dilated it; and there are "pseudo-failures" too, when relapse occurs after the reestablishment of a good local vascular-tissular equilibrium and continues to be well tolerated by the surrounding skin for a very long time.

[Use of a crochet hook for stripping incompetent, perforating veins as an adjunct to sclerosant therapy].

If during the course of sclerosant treatment an incompetent perforating vein situated along the course of a varicose trunk is not obliterated, or alternatively recanalises just after a temporary obliteration, the therapeutic phlebitis has an irregular appearance, being hard and solid in its proximal course, weak or even absent at the point of leakage, and soft and inflamed below it. At this level there often develops a periphlebitis of long duration, followed by recurrence of the varicosity. In such cases the author attacks the incompetent perforator using a hook. Analysis of 68 ruptures carried out in this way (15 on perforating veins on the medial side and 37 on "Cockett 3") shows that this small operation is quite harmless and rapidly improves the quality of the sclerosis.
