Rapid intestinal transit and postgastrectomy malnutrition. An experimental study in growing rats.

An experimental study was carried out in young, growing rats submitted to total gastrectomy followed by Roux-en-Y esophagojejunostomy (RY) or jejunal loop interposition (JI) to test the role of rapid intestinal transit time in postgastrectomy malnutrition. One to two months postoperatively esophago-intestinal transit times were measured and nutrition evaluated by serial body weights. Small intestinal transit times were consistently rapid in the rats submitted to RY reconstruction, in comparison with JI (p less than 0.01). Four months after the operations the growth curves of the three groups (RY, JI and controls) were markedly different. After an initial fall in weight of 33% in the gastrectomized rats, a sizeable increase was observed in the rats with JI (p less than 0.05). This difference in growth may be due to malabsorption and accelerated intestinal transit as a result of bypassing the duodenum after RY.

[Xeroangiography. Technical principals and applied developments].

The physical patterns ruling the information of a xeroradiographic image allow the visualization of the arterial system with a lower concentration of contrast medium. So you can visualize the arterial branches both with intravenous injection of the usual angiographic concentration of contrast medium or with intraarterial injection of diluted contrast medium at a very low concentration. A prerequisite to successful xeroangiography was the commercial diffusion of a changer for xerox cassettes allowing several exposures according to standardized programs.

[Transvenous xeroangiography of arteriovenous fistulas of patients undergoing periodic hemodialysis].

Xeroradiography allows visualisation of the arterial system after intravenous injection of contrast media. This method has proved particularly useful for diagnosis of changes in the arteriovenous fistulas of 20 patients on chronic haemodialysis. In all cases transvenous xeroangiography gave information about morphologic alterations of vessels and soft tissues. Among the commonly used angiographic methods, transvenous xeroangiography appears the technique of choice.

[Changes in thyroid hormone status after injections of iodinated contrast media].

Administration of four different contrast media exerted slight but significant and prolonged effects on the hormonal thyroid status, in 53 euthyroid subjects (four subgroups), without any clinical expression of dysthyroidism. Changes in serum T3, rT3 and T4 demonstrated different trends for each iodinated compound, probably depending on the regulatory thyroid mechanisms, and CM pharmacokinetics. Plasma profiles of free fractions (FT3, FT4) slightly reflected consensual changes with circulating iodothyronines, at various time periods of study, demonstrating high correlation indexes with total fractions. The five hormones profiles correlation study suggest that different mechanisms could be responsible for some features encountered of "low T3 syndrome". These effects may be not only equivalent to giving various sources of iodide, but suggest more complex mechanisms. It is apparent that man has a remarkable and possibly unique system of homeostatic thyroid hormone regulation.

[Embolization in bladder hemorrhage of neoplastic origin].

The authors report on their two years experience about hypogastric transarterial embolization in the treatment of urinary bladder intractable neoplastic hemorrhage. They discuss about materials for embolization and treatment of pain for acute ischaemia secondary to embolization.

[Value and limits of echography and abdominal lymphography in malignant lymphomas].

Diagnostic results obtained by echography and lymphography in the study of abdominal lymphoglandular involvement in patients with lymphomas are compared. The data indicate that while lymphography is more revealing, the two examinations should be used together in staging. Echography, on the other hand, should be used for long-term surveillance, once the lymphographic contrast medium has been absorbed.

[Renal contrast radiography study in hypertensive patients: comparison of the nephrotoxicity of ionic and non-ionic contrast media].

The results of a study carried out on 32 patients with arterial hypertension about nephrotoxic effect obtained with a new non-ionic contrast agent have been reported. The diagnostic approach was based on the determination of the urinary excretion of two characteristic enzymes of the proximal tubule-epithelial cells (alfa-glycosidase and gamma-glutamil-transferase) and of a microprotein (beta-2-microglobulin) filtered by glomeruli and readsorbed and catalized by epithelial cells. The method used show an increased sensitivity and reliability in the early recognition of a kidney damage as well as in the control of anatomic and functional changes, in comparison with the classic parameters (azotemia, creatininemia). The results show a significantly lower enzymuric and microproteinuric level using non-ionic contrast media, also, in the patients at relatively higher risk of kidney damage. This results should be interpreted in favour to a lower potential nephrotoxicity of non-ionic contrast media and their elective use in the patients with a higher risk of kidney damage.

[Radiology of the thoracic duct in liver cirrhosis (author's transl)].

The authors on the ground of the lymphographic patterns of 54 cirrhotic patients have emphasized the morpho-functional alterations of the thoracic duct of this patients on the basis of three different parameters: 1) morphology of the thoracic duct; 2) dilatation of the thoracic duct; 3) mouth of the thoracic duct.

Trans-arterial chemoembolization (TACE) of liver metastases from colorectal cancer using irinotecan-eluting beads: preliminary results.

PURPOSE: The purpose of the study was to evalutate the feasibility of irinotecan drug-eluting beads (DC Bead) administered as trans-arterial chemoembolization (TACE) in patients with liver metastases from colorectal cancer (CRC). PATIENTS AND METHODS: Ten patients with liver metastases from CRC were treated with TACE adopting irinotecan-eluting beads at a dose of 100 mg every 3 weeks. Computed Tomography (CT) was performed 24h before and after TACE. RESULTS: TACE with irinotecan eluting beads was found to be feasible and well-tolerated. Right upper quadrant pain (RUQP) lasting 4 days (range 2-7) was reported by all the patients. After 30 days, a reduction >50% of CEA levels and of the lesional contrast enhancement was observed in all the patients. CONCLUSION: Irinotecan drug-eluting beads administered as TACE were shown to be active and safe in patients with liver metastases from CRC.
