[Differential diagnosis of accident and resuscitation traumas].

The purpose of this paper is to define the criteria for the differential diagnosis of trauma following resuscitation and road accidents. To this end, 311 cases of thoracic and epigastric trauma were selected from the 2893 medico-legal autopsies carried out between 1979 and 1982 at the Institute of Forensic Medicine of the University of Heidelberg. Cardiopulmonary resuscitation had to be considered as the cause of trauma in 140 of these, but 45 of this group were excluded from further evaluation as they had been the victims of blunt trauma and no clear-cut distinction was possible between trauma resulting from an accident and trauma resulting from resuscitation. Thus, we were left with 95 cases of internal injury that presented as emergencies and in whom death followed resuscitation, as a group for comparison with 171 road accident victims who had not received cardiopulmonary resuscitation. Rib fractures, predominantly on the left side, were established in half the cases resuscitated, sternal fractures also being found in one-third of these victims. Bleeding at various sites, including hemato-thorax, was rare, with an incidence of 15%, thus making it highly unlikely that serious traumas caused by resuscitation were a major factor in the cause of death. This paper encompasses an extensive discussion on serious injuries, such as aortic and gastric ruptures, in this connection.

[Campylobacter pylori in duodenal ulcer--a concomitant phenomenon or causal association?].

The rediscovery of spiral bacteria in gastric mucosa in recent years has given raise to new speculations concerning the pathogenesis of peptic ulcer disease. Due to the features and localisation of this bacterium it was named campylobacter pylori. Findings pointing to a pathogenetic role of this bacterium are few and not definitely settled. The frequency of campylobacter on the gastroduodenal mucosa, its short term survival in acidic environment, its extraordinary mobility and content of specific enzymes makes it likely that this bacterium exerts a noxious effect on the gastroduodenal mucosa. Moreover a local and systemic immune response caused by campylobacter has been reported. We performed ultrastructural investigations on duodenal mucosa in patients with active peptic ulcer disease. An ultrastructural localization of the bacterium in duodenal mucosa of these patients was possible in 40%. Campylobacter pylori is localized preferentially on metaplastic cells of the antrum type, is seen penetrating in the intercellular spaces and is found also included in vacuoles of epithelial cells. These findings raise the hypothesis that campylobacter pylori infection can lead to direct damage of duodenal mucosa.

[Ultrastructural aspects of active and healed duodenal ulcer].

To characterize the fine structure of active and healed duodenal ulcers, tissue specimens of patients with active duodenal ulcer disease were examined under two different treatment schedules by various electron microscopic techniques. The striking histological feature of both, the active and healed duodenal ulcer, was the appearance of PAS-positive epithelial cells. Electron microscopy revealed similarity to a special type of mucus secreting cell of the antrum. The destroyed mucous structure during the ulcerative phase regains the normal net-like structure after treatment. The ultrastructural healing process shows constant features independent from the type of treatment and shows that the repair mechanisms are incomplete after a 4 week treatment.

Investigations on the exogenous influencing of the spontaneous tumour rate in rats.

72 male and 72 female rats of the strain Can-Hoe-Wiga received standard diet in a long-term study (24 months). A group of equal size received the beta-blocker BM 51.052 administered at a dose of 900 ppm in the feed. The animals in the drug group showed a 20 to 30% delay in body weight gain in comparison with the control animals. These drug-treated animals showed, similarly to experiments with a restrictive diet, a lower death rate, fewer glomerulopathies, and lower creatinine and protein levels in the serum. In addition, there were fewer tumour bearers, the total number of tumours was less, and the malignancy and formation of metastases was reduced. This experiment shows the importance which should be attached to the course of body weight gain during a long-term study. Only if these parameters are taken into consideration, an interpretation of the experimental results is possible.

[Histological and ultrastructural findings in the healing phase of duodenal ulcer].

To evaluate and further characterize duodenal ulcer healing macroscopical, histological, and ultrastructural criteria were examined in 24 patients with active duodenal ulcer disease under two different treatment schedules. After the endoscopic confirmation of an active duodenal ulcer patients were randomly assigned to treatment with either an antacid (group A) or with an H2 blocker (group B). Tissue specimen were taken during endoscopy from the margin and 1 cm from the ulcer before therapy and from the ulcer scar or the previous ulcerated region after a 4 week treatment. A macroscopic ulcer healing by endoscopic criteria was confirmed in 83% of the patients in group A and in 83% of group B. The prominent histological feature of duodenal ulcer healing was a significant increase of PAS positive epithelial cells. Ultrastructural changes of the duodenal mucosa were still present at the end of the treatment, even in the presence of complete macroscopic healing. The ultrastructural healing process shows constant patterns independent from the type of treatment. The characteristic morphological feature by electron microscopy of the healing duodenal ulcer is the appearance of a metaplastic mucus secreting cell of the antrum-type. The devastated mucous structure during the ulcerative phase regains the normal net-like structure after 4 weeks therapy.

Incidence of spontaneous tumors in laboratory rats.

The Caw-Hoe-Wiga strain of the Sprague-Dawley rat was observed for a period of ten years (1974--1983). In toxicity studies, a gradual increase of food intake could be noted in 12- to 13-week-old rats. The food intake of males was 21 g in 1974 and 27 g in 1983; in females 16 g in 1974 and 19 g in 1984. Correspondingly, a gradual increase of body weight was measured. The body weight of males was 400 g in 1974 and 470 g in 1983; of females 240 g in 1974 and 285 g in 1983. The gradual increase of food intake and body weight was followed by a gradual increase in the incidence of spontaneous tumors, 1974: 5% and 1983: 13%. Our experiments support the opinion of a causal connection between food intake, body weight and incidence of spontaneous tumors. Tumors can appear in any age group, but tumors. occur more often in older animals. In our studies, the increase in the incidence of spontaneous tumors was proportional to the increase in age: At 15 months only 6%, at 32 months 86%. 32% of the tumors were located in the mammary glands, 27% in the hypophysis, 12% in skin and appendages, and 9% in other endocrine organs. The comparison of toxicity and carcinogenicity studies revealed no change in the tumor spectrum, but strain-related tumors appeared earlier in life, more frequently and more often multifocally towards the end of the 10-year observation period.(ABSTRACT TRUNCATED AT 250 WORDS)

[Bezafibrate in primary hyperlipidemias (author's transl)].

The effect of long-term treatment over 40 weeks with Bezafibrate on lipids and lipoproteins was investigated in 27 patients with primary hyperlipoproteinemias (hlp) (12 patients with hlp type IV, 7 patients with type IIb, 3 patients with type IIa, 4 patients with type V and 1 patient with type III). Bezafibrate reduced total cholesterol by 16%, whereas HDL-cholesterol increased by 28% and 36% (p less than 0.05). Serumtriglycerides decreased by 59% (450 mg Bezafibrate daily) and by 66% (600 mg Bezafibrate daily) statistically significant (p less than 0.05). In hyperlipidemias type IV, IIb, IIa and V increases of HDL-cholesterol could be observed. The course of LDL-cholesterol was different in the various types of hlp. The postheparin-lipoprotein-lipase (PHLA) was activated by treatment with Bezafibrate from 10.5 +/- 0.7 to 14.7 +/- 0.7 and 15.5 +/- 0.8 mumol FFA/ml/h or by 30% (p less than 0.05). Only few side-effects during treatment with Bezafibrate could be ascertained.

[Traumatic infarction following stab wound of the heart (author's transl)].

Case report about a myocardial infarction following stab wound of the left atrium including a complete severance of the left coronary artery: An athletic 35-year-old man suffered a latero-medial stab wound of the thorax. The knife had penetrated the left lung and the atrium of the hypertrophic left heart. Postoperatively, the patient developed a heart infarction; 40 h after trauma he died. The postmortem revealed traumatic necrosis of the left atrium, an intraoperatively undiscovered complete severance of the ramus circumflexus arteriae coronariae sinistrae and an antero-lateral ischemic cardionecrosis of the left ventricle. The location of the infarction could be explained by an extreme right preponderance of the coronary blood supply. Traumatic and ischemic myocardial necrosis differed distinctly in the amount of contraction bands. Other causes for such morphological changes like catecholamine-cardiomyopathy (so-called epinephrine myocarditis) or heart operations are discussed.
