The increase of perceived exertion, aches and pain in the legs, heart rate and blood lactate during exercise on a bicycle ergometer.

This study was designed to show the general increase in perceived exertion, perception of aches or pain in the legs, heart rate (HR), and blood lactate, and the covariance between these variables during bicycle ergometer work, and to describe individual differences both within and between power levels by testing a large group (28 male students). Estimates of perceived exertion and feelings of aches or pain in the legs were recorded using Borg's category-ratio scale (CR-10). The subjects were tested with a stepwise increase of power levels with 40 W increments up to a voluntary maximum. Though HR increases fairly linearly with power, the other variables follow positively accelerating functions with exponents of about 1.6-2 for the perceptual variables, and an exponent of about 3 for blood lactate. The results from the 8 most fit subjects could be described in the same way as for the whole group except for blood lactate, where there was a need to include a threshold value (b), that, together with a rest value (a), shows the starting point of the function (R = a + c(W - Wo)n). The data support the idea that a combination of heart rate and blood lactate is a better predictor of perceived exertion and feelings of aches and pain in the legs, than is each of the single physiological variables taken alone.

[Osteomalacia and pregnancy].

On the basis of two recent clinical cases seen in immigrants, the authors review osteomalacia and its relationship with childhood rickets. They demonstrate that treatment with a combination of calcium and vitamin D readily halts the progression of the disease during pregnancy; the major problem therefore consists of making the diagnosis.

[Postoperative paralysis of the leg].

Gynaecologists and bowel surgeons should realise that paralysis of the muscles of the lower limb following operation, although rare, is a definite risk which can be avoided by pre- and post-operative measures, especially as nowadays it is customary to operate through small narrow scars of aesthetic appearance. This does increase the risks, particularly when heavy retractors are used.

[Water intoxication following oxytocin perfusion].

The authors report a case of water intoxication in a woman who received 140 I.U. of oxytocin in 3 litres of glucose serum in 15 hours to induce a therapeutic abortion. This water intoxication led to loss of consciousness with increase in weight and hyponatraemia. The treatment consisted in restricting fluid intake, giving sodium and 20% Mannitol. A short review of the literature with an explanation of the physiopathology is given. This is the antidiuretic action of oxytocin when it is given with too much fluid without electrolytes.

Discomfort and pain from loaded passive joint structures.

Sensations of strain and pain from healthy elbow and knee joints kept in an extreme position were reported on Borg's category-ratio scale (4) by 12 male volunteers in an experimental situation. The purpose of the study was to find out whether passive joint structures give sensations of discomfort and pain when moderately loaded for a certain time in an extreme position. Sensations of discomfort and pain were reported every 15th second. The mean values of ratings followed a straight line of increasing values during the first four minutes. Great variations were found between individuals. The elbow joint was six times more sensitive to the loading dose expressed as the product of the moment of force and time than was the knee joint. After the load was removed the provoked sensation slowly decreased. Sometimes the initial after-load rating was higher than the last rating with the load. The results imply that static work postures should also be analyzed with respect to joint position.

[The syndrome of premature ovarian failure].

The syndrome of early ovarian failure gives rise to several pathogenic and psychological problems because it often condemns the woman to permanent sterility. Because of this a precise diagnosis has to be made, and this is done by: biological examination of serum F.S.H.-L.H., Prolactin, delta 4, 17 O.H.C.S. and 17 C.S., T3 and T4, and T.S.H. and Oestradiol levels. Karyotyping should also be carried out because that allows a dysgenesis to be eliminated. Auto-immune antibodies should be sought and laparoscopy carried out for ovarian biopsy. Finally, karyotyping can be carried out on the ovarian fibroblasts. Primary follicles should also be sought because these alone could theoretically give rise to pregnancy when oestrogen therapy is stopped.

Ratings of perceived exertion and heart rates during short-term cycle exercise and their use in a new cycling strength test.

Interest in tests of short-term maximal exercise capacity has increased during recent years. The purpose of this investigation was therefore to study how heart rates and ratings of perceived exertion increase during a work test when the subjects only have to exercise at each work load for 0.5 min and to study predictability of maximal performances. A differential test is obtained by using a series of submaximal ratings to estimate the exercise intensity that can be maintained for only 30 s. The validity of the estimated intensity was checked by having the subject exercise at that load. The test time obtained was then used to correct the estimated exercise intensity according to the general function describing the relation between exercise intensity and exercise time for maximal performances. In the validation the test performances were compared to results from (a) common submaximal ergometer test, and (b) a special test measuring dynamic muscular strength.

The variation of hand steadiness with physical stress.

The performance of 12 subjects in a hand steadiness task was compared at different work loads. Hand steadiness expressed in the form of values of hand shakiness was found not to increase linearly with work intensity. On the contrary, the relation between hand shakiness and activation level, induced by physical work, was found to be positively accelerated by an exponent of 1.6. Subjective effort measured by a ratio estimation method grew, likewise, according to a positively accelerating function with an exponent of about 1.6, while subjective effort according to the "RPE" category scale like hear rate grew linearly with work load.

Quantitative evaluation of chest pain.

The perception of pain in three groups of patients with angina pectoris has been studied. All patients went through a bicycle ergometer test with an increase of 10 wat each minute. Heart rates and ratings of pain were collected towards the end of each work load. Reliable results of diagnostic interest were obtained. The rating procedure is discussed and a new category rating scale with ratio properties is also presented.

Somatic sensation and discomfort in the arm of post-mastectomy patients.

From a group of 928 randomly allocated women that had undergone mastectomy for cancer, 475 were examined with respect to the somatic sensations and discomfort in the arm on the side of the operation. The women rated psychological variables such as sensations and discomfort according to an 8-degree scale. The variables investigated were: heaviness of the arm and hardness of the arm tissue, tension, weakness and paresthesia. Eighty per cent of the women recorded a rating of 0. For most of the variables the means were thus low and corresponded to a rating of less than 1. The standard deviation was fairly large. In the women with a rating of 1 or more there was a weak but significant correlation between some of the psychological and somatic variables, including volume (positive correlation) and gripping force (negative correlation). Women with 15% oedema or more recorded the highest rating for the variables heaviness, hardness, weakness. The sensation and discomfort were rated significantly higher by the patients with than by those without discomfort or pain in the cervical and thoracic spine, chest and shoulder. The ratings were significantly higher by the women receiving radiotherapy than in those receiving only surgical treatment. There was no significant difference between the ratings for the women given radiotherapy before and those so treated after the operation.

[Pelvic exenteration for carcinoma of the uterine cervix (author's transl)].

From 1953 to 1972, 149 patients at Centre Leon Bérard, were treated by pelvic exenteration for carcinoma of the cervix. A review of the literature and of our cases showed that the mortality rate varied between 12 and 38 per cent according to the authors. In our experience, the final cause of death has been essentially pelvic reccurences within 18 months of the operation. For Ketcham, the patients died principally from metastases. To illustrate these results, criteria for patient selection for pelvic exenteration are outlined, with some suggestions for operative and post-operative management. The pre-operative medical status, the roentgenographic studies and finally exploratory laparotomy eliminated all but a very small number of patients, 15 to 20 per cent. Pelvic exenteration is appreciably beneficial only for this small group. Indeed, the exenteration is acceptable only when it does not result in excessive mutilation disproportinate with the chances of survival. It is conceivable only as curative treatment requiring a radical loco-regional excision, not only for the involved viscerae but also for the pelvic lymph nodes.

The validity of four bicycle ergometer tests.

The aim of this study was to compare the validity of four physical work and performance tests on bicycle ergometers commonly used in Sweden. The following four tests were compared: a) a work test of submaximal character designed for estimation of the maximal oxygen uptake of the individual (Vo2 max); b) a work test of maximal character designed for estimation of the highest work load an individual can sustain for 6 minutes (Wmax6min); c) the Cycling Strength and Endurance Test (CSET)-a performance test of maximal character consisting of a series of intermittently determined terminal thresholds; and d) a flexible work test of maximal character, the Individually Adapted Work Test (the IA-test): by a feedback mechanism the highest work load an individual can be expected to sustain for 4 minutes is determined. Criteria were cross-country runs (altogether 4) arranged a short time before and after the tests on the bicycle ergometer. Subjects were military conscripts, aged 17-25. When comparisons were made between the best prediction variable of each test, no significant differences were obtained. The size of the validity coefficients was .55-.70. However, the maximal work tests showed the highest validity coefficients. The investigation indicates that, when a work test is to be chosen, the problem, the practical conditions and the resources available should guide this choice.
