Patients seeking medical attention for respiratory infections were investigated with serology and culture for Chlamydia pneumoniae to investigate the prevalence of C. pneumoniae in a specific geographical area and its association with respiratory disease. Evidence for C. pneumoniae infection was found in approximately 25% of cases. 71% of patients with C. pneumoniae had symptoms from the lower respiratory tract as compared to 29% of the others. We conclude that C. pneumoniae can be a major cause of acute bronchitis.

A 57 year-old woman was hospitalized because of a sternal fracture inflicted during a car accident. During the following two weeks she became breathless and was readmitted after having syncope, induced by exertion. A perfusion scan of the lungs showed pulmonary emboli in both the lower lobes. After anti-coagulation treatment her symptoms faded, but the ultrasonographic signs of pulmonary hypertension persisted for nearly four months. The probable mechanism behind the pulmonary embolism was transmural contusion of the right ventricle, leading to a mural thrombus which eventually became detached. The authors review the literature and discuss problems connected to the diagnostic criteria for cardiac contusion, as well as delayed complications, of which pulmonary embolism is a potentially lethal condition.

Infections caused by Chlamydia pneumoniae were first described in 1985. The infection can cause common cold, sore throat, hoarseness, cough, headache, fatigue and sometimes influenza-like illness. Examination can indicate serous otitis media, sinusitis, laryngitis, bronchitis and pneumonia. The course can be long and relapsing. The recommended drugs for treatment are tetracycline or erythromycin for at least two weeks. Five verified cases are described in the article, four of them with symptoms from the upper respiratory tract only. It is concluded that Chlamydia pneumoniae is a not unusual cause of upper airway diseases. Up to now the diagnosis can best be verified by micro immunofluorescence. The authors call for a rapid and reliable test for use in physician's office. It is proposed that infections caused by Chlamydia pneumoniae be termed TWAR.
During an outbreak of streptococcal infections in a day-care center it is often difficult to determine the source and extent of the outbreak and the optimal means of management. An intervention strategy based on repeated throat cultures and initial antibiotic treatment of culture-positive individuals was used during an outbreak of respiratory tract infections with Group A streptococci at a day-care center. The spread of streptococci carriers was studied. Two weeks after the diagnosis of the index case, 61% of the 30 children were colonized with Group A streptococci. From 20 to 30% of the children remained streptococcal carriers, but they did not give rise to secondary cases. Initially the environment was heavily contaminated with Group A streptococci. Damp material might constitute a potential source of streptococcal spread. All Group A streptococcal strains belonged to the same serotype, T12M12. Prompt recognition and intervention are important to prevent spread of streptococcal infection to many day-care attenders.

This study included 1,416 graduates from the four medical schools in Norway, during the period 1978-81. The purpose was to determine which factors potentially influence choice of specialty. There were some differences in choice of specialty between physicians from different universities. Choice of specialty also varied with sex, age, and examination performance. Female physicians took no more time than their male colleagues did to become specialists. It is also concluded that the quality of the undergraduate teaching of the different specialties had little or no impact on future choice of career.
