Calcium oxalate precipitation in urine following T.U.R. with glycine as irrigation fluid.

Precipitation of calcium oxalate was studied in a group of ten patients who underwent transurethral prostatectomy with glycine irrigation. Post operatively all patients showed hyperoxaluria; 60% with oxaluria higher than 10 times the critical concentration of calcium oxalate in urine. Other factors, known to favour crystallogenesis were modified during the same period: low urinary output, relative hypercalciuria and uraturia and hypomagnesuria. Thus, conditions for intraluminal precipitation of calcium oxalate were present post operatively. We suggest the use of a xanthine oxidase inhibitor to prevent hyperoxaluria.

Several isoforms of crotoxin are present in individual venoms from the South American rattlesnake Crotalus durissus terrificus.

Crotalus durissus terrificus venoms collected either from individual snakes or from a large number of animals (more than 30) have been fractionated by high performance liquid chromatography on gel-filtration and ion exchange columns. The chromatographic patterns obtained with individual venom samples indicated that each Crotalus durissus terrificus snake synthesizes five to ten different crotoxin isoforms in widely variable relative proportions. Furthermore, the heterogeneity of venom samples collected from a large number of snakes did not appear significantly larger than that observed with venoms obtained from individual snakes. The comparison of the chromatographic patterns that we obtained with the various (individual and pooled) venoms allowed us to identify about 15 crotoxin isoforms, which may result from the expression of isogenes, since two amino acid variants have been reported to occur at several positions in the sequence of crotoxin component B. These observations confirm the existence of numerous molecular isoforms of crotoxin and suggest that an individual Crotalus durissus terrificus snake possesses several genes coding for the various crotoxin isoforms. The heterogeneity of venom samples collected from a large number of animals is explained, in a large measure, by the complexity of the venom obtained from the individual snakes.

[Tumors of the organ of Zuckerlandl. Diagnostic approach apropos of 2 cases].

We report 2 cases of both functional and non functional paragangliomas. Emphasis is put on preoperative localization. CT along with echography is recommended as the initial radiographic procedure. If the tumor is not detected or in case of multiple tumors, a scintigram using MIBG is performed as a second still difficult to define. This feature is of importance in the long term follow up.

[Familial articular chondrocalcinosis].

Articular chondrocalcinosis is a well-defined radiological entity with or without clinical manifestations. A familial form was described by Sitaj and Zitnan in 1957. Attention has mostly been paid to chondrocalcinosis associated with metabolic diseases and to "sporadic" chondrocalcinosis, usually due to ageing of the joint. Familial chondrocalcinosis, of which more and more cases are reported, is increasingly under study and is used as a model for pathogenic research. It already appears that familial chondrocalcinosis, of dominant autosomal transmission, is a new metabolic disease.

Chewing-gum preservative induced toxidermic vasculitis.

This paper reports the case of a young female patient who presented with a cutaneous urticarial disseminated eruption. Drug-induced side effects were eliminated, and the only recent dietary change was the regular use of chewing-gums containing chlorophylla (E140), menthol and BHT (butylhydroxytoluene). Immunohistological analysis of a cutaneous lesion revealed signs of vasculitis. Within 1 week after stopping chewing gum, the eruption subsided. Oral provocation tests at 4-day intervals confirmed the responsibility of BHT by the reinduction of the cutaneous signs after a few hours.

Immunohistological identification of cell subsets in human gingiva after local treatment for gingivitis or periodontitis.

The cellular infiltrate present in human diseased gingiva was analyzed in biopsies from 12 patients with gingivitis or periodontitis. The samples studied had been obtained in the course of surgery at inflammatory sites remaining after institution of periodontal treatment. Histological and immunological techniques were used to identify macrophages, B-cells, plasma-cells, T-cells and T cell subsets, as well as cells expressing class II HLA membrane antigens. T-cells appeared as the predominant population, but plasma-cells were also visualized in nearly all samples. Both OKT4+ and OKT8+ cells were seen in all cases, the latter being more numerous in periodontitis patients. Interdigitating-like cells were observed, positively labelled for class II antigens, as well as macrophages which were more numerous in periodontitis patients. These results suggest the participation of all components of the immune response in gingival disease, in a way resembling chronic recurrent inflammatory diseases.

Histoimmunological discrepancies in primary IgA nephropathy and anaphylactoid purpura sustain relationships between mucosa and kidney.

Immunohistomorphometric analysis of the tonsils from a series of patients with anaphylactoid purpura or Berger's disease showed abnormal proportions of gamma- and alpha-isotype-producing cells. Conversely, renal immunofluorescence (IF) performed on kidney biopsies from 2 patients with hematuria and clinical symptoms suggestive of one or the other of these conditions failed to confirm the suspected diagnosis. Immunohistomorphometric analysis of these patient's tonsils showed normal proportions of gamma- and alpha-isotype-producing cells. These negative results suggest that strong relationships exist between mucosal abnormalities and renal deposition of IgA in hematuria.

[Mononuclear phagocytic cells in human rheumatoid synovial membrane. Histochemical and immunohistological study].

Non specific immunity in human rheumatoid synovium: histochemical and immunohistological analysis. Enzymatic activities and monocyte-specific membrane antigens were looked for on frozen sections from 25 synovial membrane samples from patients with rheumatoid arthritis. Classical histochemical reactions were used to identify non specific esterases, alkaline and acid phosphatases, ATPase and peroxidase. Indirect immunofluorescence was performed with a series of monoclonal antibodies to monocyte membrane antigens and HLA class II molecules. Technical pitfalls were successfully overcome, and specific labelings demonstrated the variety and heterogeneity of these markers among synovial cells and vascular endothelia. Reported data indicated that such a panel of investigations is useful to better define the non-specific immunological phenomenons which take place in this active pathological tissue. They suggest that numerous metabolic activities concur to sustain chronic inflammation.

[Recent traumatic lesions of the male anterior urethra. Apropos of 17 cases].

Seventeen cases of anterior urethral injuries are presented. The authors define the management of these lesions: emergency IVP and suprapubic catheter drainage; after three to five days, retrograde and voiding urethrography and surgical repair: end to end anastomosis when the rupture is complete (8 cases with 7 good results, 1 fair result), in case of partial rupture, surgical repair should be preferred (2 cases with 2 good results); the other alternatives, urethral catheter (2 cases), or suprapubic catheter drainage with delayed treatment of stricture (3 cases), appeared to be less comfortable, contusions must be treated conservatively.

Salicylate kinetics in old age.

Salicylate kinetics were determined in 28 subjects 25 to 92 years old who received single, oral doses of sodium salicylate (1 gm/1.73 m2). The serum AUCinfinity of total salicylate did not correlate with age. There was a weak positive correlation between the AUCinfinity of free (unbound) drug and age, but there was no apparent difference between the AUCinfinity values of the 15 women and 13 men. Seven of the 16 subjects greater than 70 years of age cleared salicylate at about the same rate as the younger subjects. A comparison of these seven subjects with the nine greater than 70 years old who were slow eliminators of salicylate revealed that the latter group consisted of more bedridden patients and that these patients had somewhat lower serum albumin concentrations, but they did not differ from the more rapid eliminators with respect to serum creatinine or urea nitrogen levels, SGOT, average age, female/male ratio, and average body weight. The serum protein binding of salicylate decreased with increasing age, apparently due mainly to decreasing serum albumin concentrations.

Silicon-containing particles in synovial fluid: scanning electron microscopy coupled with analytical techniques allows an easy identification and differentiation from pathologically relevant crystals.

Silicon-containing particles were observed by scanning electron microscopy (SEM) in synovial fluid samples from patients with crystal-induced or inflammatory synovitis, or both. This material was an artefact produced by the technical procedures, but these particles could be easily differentiated from naturally occurring compounds by their morphology and their composition determined by analytical spectrometry.
