[Therapeutic options in locally advanced breast cancers].

The authors are showing their experience on 394 locally advanced breast cancer in the last 10 years. In this trial were included the tumours larger than 5 cm, multiple tumours, tumours invading the skin or the thoracic wall with invaded or fixed axillary or supraclavicular nodes and acute carcinomatous mastitis. The optional therapeutic schedule was modified upon the stage. It consisted in pre- or postoperative radiotherapy, polychemotherapy or polychemo- or radiotherapy alone, associated with nonspecific immunotherapy and hormonotherapy. In patients with complex treatment the survival rate was 45.4% at 5 years and 12.3% at 10 years. The authors highlight the importance of the surgical treatment which offer a big potential in the context of the complex treatment.

Experimental studies in transient myocardial ischaemia.

The biology of the myocardium was studied under experimental conditions similar to angina pectoris. In some dogs the myocardium was adapted to ischaemia by progressive coronary occlusion of 1-5 min followed by restoration of circulation during 5 min. In other dogs adaption was followed by 20 to 35 min ischaemia. The animals were sacrificed immediately or after 2-10 days. Transient ischaemia produced less severe alterations then abrupt coronary obstruction. Adaptation followed by 20 and 35 min ischaemia induced foci that undergo cytolysis and scarring of maximum intensity on the 8th day. Activity of enzymes in the mitochondrial suspension, especially of cytochromoxidase, decreases and lysosomal hydrolases increase with focal necroses.

[The surgical treatment of haemophilic arthropathy (author's transl)].

The authors have operated on 13 haemophilic joints. The operation included 5 knee synovectomies of the knee, 2 corrective osteotomies, 4 soft tissue procedures on the knee or foot (tendon lengthening, capsulotomy), 1 arthrodesis of knee and 1 excision of heterotopic ossification. The type of operation needed varied with the degree of involvement. Indications for associated haematological treatment are given. The indications were both preventive and curative. The results were gratifying, although two cases of post-operative infection were seen.

Deuterium oxide effects upon three parameters characterizing the activity of glycerol-extracted muscle: phosphate release, ATP-induced shortening, and 22Na+ distribution.

ATP splitting activity is progressively reduced with increasing heavy water (D2)) concentration. In contrast, sarcomere shorteining inhibition produced by D2O does not significantly depend on its concentration. Even at low concentration, the presence of D2O does reduce the excessive accumulation of radioactive sodium within glycerinated frog muscle. These heavy water effects on muscular contraction and soidum distribution can be interpreted to indicate adsorbed water within the cells. Evaluation of these experimental results in terms of Gibbs free enthalpy of binding at the adsorption sites of D20 or H20 is in good agreement with the data in the literature.

Cytoenzymologic activities of some oxidreductases in thyreopathies.

The authors studied in cryostat sections and in smears from thyroid aspirates the cytoenzymic pattern of the following thyreopathies: euthyroid goitre, GRAVES' disease, hyperthyroidized goitre, HASHIMOTO's thyroiditis and folliculo-papillary carcinoma. A biochemical study was simultaneously performed. According to the investigated thyreopathies the highest enzymic activity was found in the GRAVES' disease especially for peroxidase, glucose-6-phosphate dehydrogenase and succinate dehydrogenase. Lactate dehydrogenase showed a great activity in thyroid cancer. The lowest activity was found in the HASHIMOTO's thyroiditis with strong fibrosclerosis. The same pattern was found in thyroid smears from fine needle aspirates. The biochemical analysi revealed a strong parallelism with cytoenzymic results. The isozymic pattern of lactate dehydrogenase showed no significant differences between the thyreopathies.

Potassium and sodium ions in the glycerinated skeletal muscle. Distribution changes induced by adenosine triphosphate and nondissociable anesthetic substances.

Investigation of the ionic behavior of glycerinated muscle fibers showed that the residual structures of this biologic cellular material, lacking functional membranes, are able to discriminate between alkaline ions. The characteristics of the ionic selectivity of the glycerinated fibers change with their functional state and with the presence in the medium of certain nonionic substances. Among the more important features of ionic distribution between the membrane-free fibers and the medium are the following: (1) There is evident adsorption of potassium on the fibers, in the absence of ATP. (2) This adsorption increases in contraction and decreases in relaxation. (3) At high ionic concentrations, in contrast to what occurs at low potassium concentrations, the glycerinated muscle prefers sodium to potassium, but even under these conditions both ions are accumulated in the fibers to far greater levels than in the medium. This strongly suggests a Donnan ionic equilibrium developing parallel to the adsorption process. (4) Nonionic substances of the general anesthetic group markedly alter the ionic selectivity of the glycerinated fibers, probably by their action on the water's physical state. A mechanism is proposed for the observed ionic adsorption specific of the muscle-a mechanism in which actin-myosin coupling plays the cardinal adsorption role. In the general interpretation of the data a synthetic concept is advanced according to which an entire set of processes and factors concurs with the distribution of ions between the muscle and the medium.

[ Epidemiological and clinical-therapeutical considerations on 500 operated cases of uterine cervix cancer].

AIM: Optimization of the treatment on uterine-cervix neoplasia, in a general surgery department. MATERIAL AND METHOD: Between 1984-1999, in Colţea Surgical Department were operated 746 uterine-cervix cancers. We selected two trials of 250 females, similarly regarding age and stadium. The A trial (250 females) were treated and operated 1984-1989, the study being retrospectively, and the B trial (250 females) treated and operated between 1990-1995, the study being prospectively by introduction of cisplatyn and carboplatyne chemotherapy in the advanced steadies. In the B trial, on registrated many mutations with negative epidemiological implications as: the grow the number of new cases, the diminution of immunity, the abandon of the screening, a possible grow of irradiations (Cernobâl accident), and the belated of presentation to medical consultation. RESULTS: The complex treatment, comparative in two trials, show a grow of lifetime, at 5 years in B trial from 82.1-93.4% in the 1st and 2nd steadies, 50.7-60.4% in the II B study and 12.9-28.5% in the III study. In the IV study the grow of lifetime was under 24 month. CONCLUSIONS: The resumption of the screening. Complexes therapeutical protocols for each study. Widely introduction of polichemotherapy in the advanced steadies.
