[Considerations on 988 breast cancers].

AIM: Therapy schedule improvement. MATERIAL & METHOD: The study includes 988 patients (which 16 men) with breast cancer who underwent surgery between 1984-1998. Out of them 63.2% were in advanced stages. The patients were divided in 2 homogenous groups: trial A = 520 patients treated between 1984-1991 (in stade: I = 2, II = 240, III = 246, IV = 32) studied retrospectively, and trial B = 468 patients treated between 1992-1998 (in stade I = 3, II = 212, III = 235, IV = 18) studied prospectively. In trial B the complex therapy schedule was improved according to disease's stade, local breast aspect and patient's biological status, straining on neoadjuvant therapy. To the entire group 945 radical mastectomies (95.6%) were performed. Only 628 (63.5%) could be properly followed up. RESULTS: Global 5 years survival rate was 72% (improved from 69% in trial A to 72% in trial B). The survival rate varied according the stage from 100% (stade I), 88% (stade II), to 22% (stade III) and 2% (stade IV). In the advanced states, the local recurrencies at 5 years were of 22% and the methastases of 17%. CONCLUSIONS: The neoadjuvant therapy, selectively applied upon stade and patient improves the 5 year survival rate. Every patient with an advanced breast cancer can benefit of a complex, differentiated and well guided treatment. The adequate operation earn the important role in powering the neoadjuvant and adjuvant therapies. Further results improvement requires restarting the collectivities and high risk persons screening.

Experimental investigations in acute myocardial ischaemia. Note II. Studies concerning morphophysiologie alterations of the ischaemic myocardium under vascularization conditions.

The biology of the myocardial fiber was studied under experimental conditions similar to angina and the premonitory syndrome of myocardial infarction. The morphophysiologic data obtained particularly concerning the mitochondria demonstrated that affection of the myocardium by progressive ischaemia induced less severe lesions than the abrupt onset of ischaemia, also lowering the frequency of ventricular fibrillations.

Morphological and biochemical characteristics in the experimentally treated rats with indigenous synthetical products used as pesticides.

In acute experimental intoxication with 3 indigenous pesticides (Tripinacloraz, Lemax and Maleic Hydrazide-Na salt) the changes of some biochemical parameters, reflecting the degree of hepatic structural alterations, were studied. The pesticides were administered by gastric gavage in a single dose close to DL50 to Wistar male rats. The morphological alterations were in good correlation with a different hepatocellular levels localization. Reversible morphological lesions (granulovascuolar like dystrophy and circulatory disorders) without any pathological significance on dose-effect correlation induced a progressive response especially on serum ceruloplasmine, leucinaminopeptidase and lactatdehydrogenase.

Simultaneous investigation of the rat serum and liver response in an acute toxic substances exposure.

Some biochemical parameters were simultaneously investigated in serum and hepatic homogenate in acute experiment with three pesticides in rat. The one-dose exposure response was relatively well correlated with the chemical toxicity and showed the statistically significant values, preferentially placed 48 hours after dosing. The liver morphological alteration might be associated with the serum and tissue comportment of the bioenzymological factors. The various genotoxicity induced by the three pesticides, appreciated by the frequency of micronucleus induction in the bone marrow cells, completes the data about the one-dose exposure effects of the chemical in rat.

The biology of the myocardium in chronic hypoxia. Note I. Myocardial lesions in experimental chronic heart failure.

The biology of the myocardium was studied in chronic heart failure, especially in the course of compensatory hypertrophy induced experimentally by partial stenosis of the aorta for 10 months and the administration of large isoproterenol doses for 7 months. In the stage of acute aggression, varied disseminated ultrastructural lesions are predominant, with the decrease of energy reserves, hydroelectrolytic and ECG perturbations. In the stage of ultrastructural compensatory hypertrophy, there is a prevalence of normal myocytes or others presenting regenerative aspects next to progressive fibrosis reflected biochemically by return to almost normal values, with certain oscillations due to the presence of some lesional foci, also recorded on the ECG tracings. The question of the pathogenesis and prognosis of these lesions is discussed.
