[Intramedullary tumors--surgical factors determining the outcome].

The recent advances in the surgery of intramedullary tumors are due to the strategy of a total removal by microsurgical techniques and the early diagnosis by MRT. 84 patients with intramedullary tumors with a long-term follow-up up of more than 20 years were retrospectively analysed so that subgroups e.g. macroscopic versus microscopic, palliative procedures versus whether or not new technology like the ultrasonic aspirator and the CO2-Laser were used were included.

Bovine erythroid (CFU-E, BFU-E) and granulocyte-macrophage colony formation in culture.

Progenitor cells of bovine erythrocytes, megakaryocytes, and granulocyte-macrophages were cultured in agar or methylcellulose media. Colony formation was supported by cell-free conditioned medium from short-term cultures of concanavalin A-stimulated bovine peripheral blood leukocytes. Granulocyte-macrophage progenitors proliferated well in both types of semisolid culture media, giving rise to neutrophils (from granulocyte colony-forming units, CFU-G), eosinophils (from eosinophil CFU, CFU-Eo), monocyte-macrophages (from macrophage CFU, CFU-M), and mixed granulocyte-macrophages (from granulocyte-macrophage CFU, CFU-GM). Better growth of megakaryocytes, as well as late (erythroid CFU, CFU-E) and early (erythroid burst-forming units, BFU-E) erythroid progenitors was obtained with methylcellulose. Despite considerable variation in the numbers and types of colonies formed from different aspirates of bovine marrow, the numbers observed were generally comparable to those obtained from human and mouse bone marrow cells. The proliferation of bovine BFU-E and eosinophil progenitors herein described is the first successful report of the culture of these cell types.

Computed tomography in partial epilepsies in childhood.

The CT investigations of 156 children with partial epilepsies are analyzed. Pathological findings appeared in 39%, particularly atrophic changes and porencephalic lesions. According to the seizure type their incidence was higher in children who had secondary generalized seizures (46%) than in children with isolated partial seizures (30%) or complex partial seizures with or without secondary generalization (16%). Furthermore, pathological findings occurred more frequently in children with perinatal etiology (37%) than in familial (26%) or idiopathic cases (10%). A symptomatic epilepsy due to a cerebral tumor or a vascular lesion was found in 10%. There was a high correlation between neurodevelopmental disturbances and abnormal CT findings, however also 17 neurological normal children had pathological CT findings.

Glutathione and peroxide metabolism in malaria-parasitized erythrocytes.

The glutathione metabolism of Plasmodium falciparum, P. vinckei and P. berghei has been investigated. Human erythrocytes with low glutathione reductase and synthetase activity are still capable of harbouring P. falciparum. Both enzymes have been demonstrated in Plasmodium spp. Moreover, evidence is given for a selenium-independent glutathione peroxidase in malaria parasites.

Plasmodium falciparum: inhibition in vitro with lactoferrin, desferriferrithiocin, and desferricrocin.

The microbial iron chelators desferriferrithiocin and desferricrocin as well as human lactoferrin were tested in vitro against Plasmodium falciparum. The microbial chelators inhibit the growth of P. falciparum in a dose dependent way. Parasite multiplication is stopped at 25-30 microM desferriferrithiocin, whereas 60-90 microM desferricrocin are needed to exhibit the same effect. After iron saturation, the microbial chelators are ineffective. Human lactoferrin (30 microM), both iron free and iron saturated, inhibits P. falciparum. A 3-day preincubation of host erythrocytes with iron free and iron saturated lactoferrin prior to infection enhances this effect, which is therefore attributed to lactoferrin bound iron. It has been suggested that the lactoferrin/iron complex generates oxygen free radicals, which may cause membrane damage of both erythrocyte and parasite. This process can be considered to lead to growth inhibition of the parasite.

Studies on glutathione reductase and methemoglobin from human erythrocytes parasitized with Plasmodium falciparum.

An improved protein-blotting procedure and a thin layer isoelectric focusing technique are introduced to study glutathione reductase and methemoglobin (Met-Hb). According to our results, there is only one form of glutathione reductase in normal red blood cells. A similar protein was shown to be present at higher concentration in isolated merozoites. Both proteins have a subunit Mr of ca. 50,000 and react with anti-human glutathione reductase serum. Red cells with schizonts do not possess a higher proportion of Met-Hb than non-parasitized erythrocytes. This finding suggests that Met-Hb is not an indicator of metabolic alterations in malaria-infected erythrocytes.

[Migraine accompagnée in childhood and adolescence].

The term migraine accompagnée or complicated migraine is applied to those cases of migraine in whom a neurological deficit, which is transient in most instances, occurs. The three main varieties are hemiplegic (unilateral sensory loss, hemiplegia, aphasia), basilar (brain stem dysfunction) and ophthalmoplegic migraine. The clinical symptoms of migraine accompagnée in children and adolescents are discussed on the basis of the observations in 26 patients (11 boys, 15 girls) and the results of EEG-, CT- and angiographic investigations are referred. A special case of "migraine without headache" and a case of "transient global amnesia" are reported.

14C-desferrioxamine B: uptake into erythrocytes infected with Plasmodium falciparum.

Plasmodium falciparum-infected human erythrocytes (early trophozoite stages) and non-infected erythrocytes were incubated in 1.7 mM 14C-desferrioxamine B (specific activity 1 microCi/2.6 mg desferrioxamine B). After 270 min the cells were washed and the radioactivity was measured in the cell pellet and, after lysis, in cytoplasm and membranes. The results indicate that Desferrioxamine B can the red blood cell and pass through the parasite membrane and that the parasites are killed by the intracellular action of the chelator.

[Cerebrovascular diseases in childhood--etiology, clinical aspects and prognosis].

Over a period of nine years we observed 52 children with acute neurological symptoms which were caused by a cerebrovascular disease. Fourteen patients had congenital vascular malformations, most frequently AV-angiomas (9 patients). A Sturge-Weber-Syndrome and a venous angioma were found in two cases and one patient had an aneurysm of the middle cerebral artery. Thirty-eight patients had acquired cerebrovascular diseases such as ischaemic infarctions (22), intracranial haemorrhages without vascular malformations (14) and thromboses of the dural sinus (2). The cerebral infraction was a complication of a congenital heart disease in 8 children, two others suffered from chronic renal insufficiency and were on haemodialysis. Two children had a trauma of the internal carotid artery and in one patient a large haemorrhagic infarct was caused by hypernatremic dehydration. In 9 patients (6 females, 3 males) no obvious aetiology of the infarct could be found. However, in most of these cases a nonspecific febrile illness preceded the neurological manifestations. The thrombosis of the dural sinus occurred in a 6-week old previously healthy infant and in a 3-year old boy as a complication of a nephrotic syndrome. Intracranial haemorrhages (without cerebrovascular malformations) occurred in 14 patients, mainly as a complication of haematological diseases (acute lymphatic leukaemia, severe aplastic anaemia, haemophilia A, lupus erythematodes). Four children had spontaneous intracerebral haemorrhages without obvious causes. The prognosis for survival was good in children with infarcts, but persisting neurological deficits were more severe than in children with haemorrhages. At the acute stage the lethality was higher in children with intracranial haemorrhages.

Mobile long-term EEG monitoring in generalized seizure disorders of different etiology.

Eighteen EEG long-term recordings were carried out in patients with generalized fits of different etiologies using a mobile 4-channel monitoring system in order to observe individual patterns of epileptic activity. In patients with idiopathic tonic-clonic seizures (A) no connection of frequency of spike-wave bursts with the sleep-wakefulness cycle was observed. In patients suffering from fits of organic nature (B) a peak of paroxysms was obtained in the hours before sleep onset, whereas in patients with absence seizures (C) a maximum was found in the morning hours after awakening. In patients with idiopathic fits (tonic-clonic and absence seizures) the minimum of bursts was observed in REM sleep, whereas stage 0 (tonic-clonic) and stage 1 (absence) were most prone to discharges. Patients with secondary generalized epilepsy had most spike-wave bursts in deep sleep. In the absence seizure group the transitional periods within the light non-REM-sleep stages were more susceptible to paroxysms than the retained stages. In patients with idiopathic grand mal seizures, the highest frequency of paroxysmal discharges was found during time spent awake in the night when retained over a longer period of time. The group including cases with attacks of organic nature did not show any preference to shifting times or preservation episodes of sleep stages.

[Studies of brain stem function in infants with apnea syndrome using acoustically evoked brain stem potentials and heart rate variability].

Twenty-four infants aged between 6 days and 7 months and 2 infants (16 m and 3 y) were subjects of combined brain-stem auditory evoked potential and heart rate variability measurements. Ten infants had a sleep apnea syndrome and one a Near miss sudden infant death syndrome; thirteen infants formed the control group. Seventeen of the twenty-six infants had normal, age-depended brain stem potentials. All infants of the apnea syndrome-group, which had no therapy, had increased I to V interpeak latencies outside of the one-sigma level, the six infants treated with aminophylline were within the normal range. Heart rate variability measurements revealed the following mean +/- standard deviation of the control group: 4.5% +/- 1.5%; of the apnea syndrome-group without therapy: 5.4% +/- 2.0%.

[Tumor removal using performance ultrasound. Report of 50 brain tumor operations].

A report is given on the method of the tumour aspiration of cerebral tumours by means of power ultrasound. After classical exposure of the tumour--obligatory performance with physical-pharmacological reduction of the metabolism and in most cases under hypotension--the tumour is destroyed by ultrasonic power and simultaneously aspirated. A report is given on 50 cerebral tumour operations, which were carried out in Karl-Marx-Stadt within two years, using the tumour aspiration system "Aspirus 120". Finally the advantages of the method are summarised and dangers interpreted. At the end a valuation of the method is attempted and an outlook is given.

[Intracranial hemorrhages in mature and premature infants. Computer tomography and clinical follow-up studies].

The findings of CT-follow-up-studies on a group of 94 infants, who had suffered intracranial hemorrhage, were correlated with the clinical and neurological outcome. Purpose of the retrospective study was to work out prognostic criteria dependent on the typ and location of the hemorrhage. Among the infants with intraventricular hemorrhage, there was a direct relationship between the severity of the hemorrhage and major handicaps on the one hand and the mortality rate on the other hand. Neurologic sequelae were noted in 16.5% of the infants with tentorial hemorrhages, in 56% of the infants with intracerebral hemorrhages and 37.5% of the infants with subarachnoid hemorrhages. Our results indicate, that the outcome of infants with intracranial hemorrhages is not dependent on the typ and severity of the bleeding only, but is also influenced by additional factors, like perinatal asphyxia.

[Clinical aspects and course of Sturge-Weber syndrome in childhood].

We report on symptoms, clinical course and prognostic factors of eight children (6 girls and 2 boys) with Sturge-Weber syndrome. The period of observation was 1 to 17 years (mean 8.8 years). All patients had the classical symptoms with facial port-wine stain nevus and intracranial calcifications. In two cases a bilateral distribution of the nevus was found. All patients but one had epileptic seizures which were followed by Todd's palsies in five. Four children additionally suffered from hydrophthalmia. Another patient had an angiomatosis of the retina and cataract. Neurological findings were normal in three patients, minimal signs were found in three, one patient had a spastic hemiplegia and another one hypotone tetraplegia. The mental development was normal in three patients, however severe dyscalculia occurred in one of them. Five patients were mentally retarded, three of them severely. The prognosis was the worse the earlier the cerebral seizures appeared.

[Neurologic, psychologic and logopedic follow-up of 6 to 8-year-old former premature infants with a birth weight below 1,501 g].

The purpose of the present study was to find out if formerly premature babies (birth weight less than or equal to 1,501 g) show differences in their neurological, psychological and linguistic development of school-age when compared to children without perinatal problems. Discrete neurological dysfunctions (clumsy child syndromes, discrete hemi-syndromes) and left-handedness were found significantly more frequently in the premature group. The test psychological investigation of the intellectual development and the development of language did not reveal significant differences between the two groups. Also with regard to the cognitive partial functions (attention, visual differentiation, serial memory performance and visuomotor coordination) the prematures were significantly behind only in serial memory performance. On the other hand the results of the psychological investigations were significantly worse in children of lower social classes independent of birth weight.

Plasmodium vinckei: suppression of mouse infections with desferrioxamine B.

Plasmodium vinckei kills NMRI mice within 6 days after infection. Treatment of infected animals with desferrioxamine B for 5 days was found to suppress the parasitemia in a dose-dependent manner. The desferrioxamine B-iron complex (DFO/Fe3+) was ineffective, which suggests that the iron-chelating capacity of free desferrioxamine B is the antimalarial principle. All mice survived when they were given 0.3 mg desferrioxamine B/g every 12 hr for 14 days after infection. In addition, they were resistant to reinfection for at least 8 weeks. Eight months after desferrioxamine B treatment, all mice had lost their induced immunity and were as susceptible to malaria as controls. These results illustrate the dependence of the malarial parasite on ionic iron and suggests new methods for the therapy of parasitic diseases.
