Computer tomography in children with epilepsy.

Computer tomography investigations in 72 children with epilepsy showed 42 to be normal and 30 abnormal. An abnormal scan was more common children with partial seizures and in the presence of neurological deficits and/or mental retardation.

[Bio-availability of phenytoin free-acid serum level determinations whilst using a lactose-based tablet-preparation (author's transl)].

Phenytoin serum levels were determined in 103 children during the steady state of phenytoin therapy using a radio-immuno-assay. 94% of the patients had been treated as outpatients, 54% received phenytoin in combination with other antiepileptic drugs. The preparation used was phenytoin as free acid in lactose tablets (Epilan D). A therapeutic serum level of 10-20 mcg/ml was reached by an oral intake of 6,1 +/- 1,6 mg/kg/d. 27 children had plasma levels above 20 mcg/ml, their oral dose was 7,1 +/- 1,8 mg/kg/d. Most of these children showed clinical signs of intoxication. The bioavailability of phenytoin preparations is not only depended on the type of the substance used (salt, free acid), but also on the excipients used for the preparation of tablets. Therefore, general dosage recommendations are useless and even dangerous, unless the preparations used are extensively specified.

[Ketamine anesthesia for outpatient procedures in children (author's transl)].

Ketamine anesthesia was used in children to perform 200 outpatient procedures (mainly spinal taps, bone marrow aspirations and biopsies). Atropine was administered in advance and a ketamine dose of 1.8--2.0 mg/kg i.v. was sufficient in most instances. The general anesthesia allows calm and accurate performance of the necessary procedures. Ketamine can be used repeatedly and was preferred by the majority of children for subsequent procedures. The application of Diazepam (2 mg i.v. or 5 mg rectally) reduced unpleasant wake-up dreams in older children. The children were under close observation until consciousness was regained. They left the outpatient clinic attended by their parents approximately one hour after the procedure.

[Tuberous sclerosis: first manifestation as cardiac arrhythmia in a newborn].

A newborn baby is found to have multifocal atrial tachycardia. Myoclonic seizures and depigmented nevi appear at the age of five months. The clinical diagnosis of tuberous sclerosis is confirmed by the result of cranial computerized tomography, which shows typical lesions. Though cardiac angiography fails to show an intracavitary tumor, multiple intramural rhabdomyomas of the myocardium are probably the cause of the persistent arrhythmia.

[Arteriovenous aneurysm of the left internal thoracic artery with hypertrophy of the left upper limb (author's transl)].

Case report on a 5 years old boy suffering from a congenital arteriovenous fistula of the left internal thoracic artery. At the age of 9 months hypertrophy of the left arm combined with dilated veins in this area developed. Diagnosis was made by clinical examination together with a density in the left upper chest on x-ray and was confirmed by phlebography and thoracic aortography. A dilatation of the left heart, which was marked at the age of 9 months vanished in childhood without any therapy. The child was operated successfully.
