Direct Lyapunov exponent analysis enables parametric study of transient signalling governing cell behaviour.

Computational models aid in the quantitative understanding of cell signalling networks. One important goal is to ascertain how multiple network components work together to govern cellular responses, that is, to determine cell 'signal-response' relationships. Several methods exist to study steady-state signals in the context of differential equation-based models. However, many biological networks influence cell behaviour through time-varying signals operating during a transient activated state that ultimately returns to a basal steady-state. A computational approach adapted from dynamical systems analysis to discern how diverse transient signals relate to alternative cell fates is described. Direct finite-time Lyapunov exponents (DLEs) are employed to identify phase-space domains of high sensitivity to initial conditions. These domains delineate regions exhibiting qualitatively different transient activities that would be indistinguishable using steady-state analysis but which correspond to different outcomes. These methods are applied to a physicochemical model of molecular interactions among caspase-3, caspase-8 and X-linked inhibitor of apoptosis--proteins whose transient activation determines cell death against survival fates. DLE analysis enabled identification of a separatrix that quantitatively characterises network behaviour by defining initial conditions leading to apoptotic cell death. It is anticipated that DLE analysis will facilitate theoretical investigation of phenotypic outcomes in larger models of signalling networks.

Stretching, alignment, and shear in slowly varying velocity fields.

We derive criteria that locate intense material stretching and shear in two-dimensional flows with slow time dependence. Our derivation makes use of the near integrability of the equation of variations along trajectories of the slowly varying flow. The criteria yield two diagnostic scalar fields for use in real-time Lagrangian predictions in geophysical flows.

Trail-induced apoptosis and interaction with cytotoxic agents in soft tissue sarcoma cell lines.

Five human soft tissue sarcoma (STS) cell lines (HTB-82 rhabdomyosarcoma, HTB-91 fibrosarcoma, HTB-92 liposarcoma, HTB-93 synovial sarcoma and HTB-94 chondrosarcoma) were analysed for their sensitivity to tumour necrosis factor (TNF)-related apoptosis-inducing ligand (TRAIL) and the function of the TRAIL apoptotic pathway in these cells. TRAIL induced significant apoptosis (>90%) in HTB-92 and HTB-93 cells, whereas no effect was observed in HTB-82, HTB-91 and HTB-94 cells. TRAIL-Receptor 1 (TRAIL-R1) was expressed in TRAIL-sensitive HTB-92 and HTB-93 cell lines, but not in TRAIL-resistant HTB-91 and HTB-94 cells. HTB-82 cells, which expressed the long (c-FLIP(L)) and short (c-FLIP(S)) splice variants of the FLICE-like inhibitory protein (FLIP), were resistant to TRAIL in spite of the presence of TRAIL-R1. TRAIL-R2,-R3,-R4 and osteoprotegerin (OPG) expression did not correlate with TRAIL sensitivity. Coincubation of TRAIL and doxorubicin led to the overexpression of TRAIL-R2 resulting in a synergistic effect of doxorubicin and TRAIL in TRAIL-sensitive cell lines and in the overcoming of TRAIL-resistance in all of the TRAIL-resistant cell lines, except HTB-91, which lacked caspase 8 expression. These data suggest that TRAIL, either as a single agent or in combination with cytotoxic agents, might represent a new treatment option for advanced STS, which constitutes a largely chemotherapy-resistant disease.

Experimental measurements of stretching fields in fluid mixing.

Using precision measurements of tracer particle trajectories in a two-dimensional fluid flow producing chaotic mixing, we directly measure the time-dependent stretching field. This quantity, previously available only numerically, attains local maxima along lines that coincide with the stable and unstable manifolds of hyperbolic fixed points of Poincaré maps. Contours of a passive impurity field are found at each instant to be oriented parallel to the lines that have recently experienced large stretching. The local stretching varies by 12 orders of magnitude.

Oxygen embolism after hydrogen peroxide irrigation of a vulvar abscess.

We report a case of venous oxygen embolism in a 33-yr-old healthy woman after irrigation of a vulvar abscess with 25 ml of 3% hydrogen peroxide. Venous oxygen embolism was diagnosed by the development of sudden hypoxia associated with a decrease in end-tidal carbon dioxide concentration from 5.3 kPa to 3.2 kPa, and a 'mill-wheel' sound on cardiac auscultation soon after injection of the solution. The patient responded to corrective treatment including the Trendelenburg position and 100% oxygen. She made an uneventful recovery. We discuss the possible causative mechanism of this embolism, the different diagnostic methods, and the controversial aspects of available treatments. We emphasize that hydrogen peroxide is a dangerous and unsuitable agent for routine wound irrigation and debridement.

Tolerance to solid organ transplants through transfer of MHC class II genes.

Donor/recipient MHC class II matching permits survival of experimental allografts without permanent immunosuppression, but is not clinically applicable due to the extensive polymorphism of this locus. As an alternative, we have tested a gene therapy approach in a preclinical animal model to determine whether expression of allogeneic class II transgenes (Tg's) in recipient bone marrow cells would allow survival of subsequent Tg-matched renal allografts. Somatic matching between donor kidney class II and the recipient Tg's, in combination with a short treatment of cyclosporine A, prolonged graft survival with DR and promoted tolerance with DQ. Class II Tg expression in the lymphoid lineage and the graft itself were sequentially implicated in this tolerance induction. These results demonstrate the potential of MHC class II gene transfer to permit tolerance to solid organ allografts.

[Assessment of intubation time and conditions under the influence of rocuronium].

PURPOSE: To evaluate the tracheal intubating conditions when intubation time is determined by the onset time of the neuromuscular block either of the adductor pollicis (AP) or of the orbicularis oculi muscle (OO). METHODS: In this prospective, double blind, randomised study, 40 adults ASA I-II undergoing general anaesthesia with tracheal intubation were allocated to two groups (n = 20) according to the reference muscle (AP or OO) used to determine the appropriate intubation time. Induction of anaesthesia was achieved with 5-7 mg.kg-1 thiopentone, 1.5-2.5 micrograms.kg-1 fentanyl and 0.9 mg.kg-1 rocuronium (3 x ED95) for muscular relaxation. Supramaximal train of four stimulation of the ulnar and facial nerve every 10 sec was used to monitor the neuromuscular block. After visual loss of AP or OO contraction, tracheal intubation and quality of intubation assessment were performed by two independent anaesthetists. Data are expressed as mean and standard error of the mean (X +/- SEM). RESULTS: Curarisation time of the OO was shorter (110 +/- 4.9 sec) than that of the AP (144 +/- 5.5 sec; P < 0.0001). Intubation conditions were excellent in 95% and good in 5% of the patients in the AP group whereas in the OO group only 65% of the patients had excellent and 20% good intubation conditions (P < 0.05). Coughing was observed in 15% of patients in the OO group during tracheal intubation. CONCLUSION: Monitoring neuro-muscular activity of the AP using TOF to determine the appropriate tracheal intubation time and conditions in patients paralysed with rocurorium is more clinically relevant than monitoring the OO muscle.

A new oral formulation of cyclosporine for early oral immunosuppressive therapy in liver transplant recipients.

CsA-ME is a new oral microemulsion formulation of CsA. Studies in stable liver grafted patients with cholestasis and subsequent poor absorption of the conventional cyclosporine formulation showed a substantial increase in CsA absorption after conversion to CsA-ME. To investigate its use in patients during the early course after liver transplantation we recruited 50 liver transplant recipients in two centers. During the first study phase A CsA-ME was administered to 20 patients in incremental doses after a short initial course of intravenous cyclosporine. During the second study phase B (30 patients) CsA-ME was administered from the time of transplantation. One year actual patient and graft survival of patients included in phase A and B of the trial was between 90% and 93.3%; 50% and 60% of the patients enrolled in phase A and phase B of the trial were free from rejection at month 3, respectively. Chronic rejection was diagnosed in one patient. No increase in the incidence of CsA related side effects was observed. The optimum CsA-ME starting dose was found to be 10 mg/kgbw/day for patients without external biliary diversion and 15 mg/kgbw/day for patients with a T tube in situ. Using these starting doses, 26 consecutive patients with external bile diversion via T tube were treated with CsA-ME from the day of transplantation. Intravenous CsA was necessary only in three patients. When CsA-ME absorption in patients with stable liver function was compared with that in patients with early liver dysfunction, no difference in the pharmacokinetic profiles was observed between the groups. Our results indicate that CsA-ME therapy is effective and well tolerated in liver graft recipients, even in patients with external biliary diversion during the early posttransplant phase. Thus, CsA-ME is a useful alternative to intravenous CsA treatment in these patients.

The effect of temporary hepatic ischemia on liver surface pH and potassium ion activity in the rat.

The effects of hepatic ischemia can be analyzed with a wide variety of functional and morphological methods. In this study we used a new organ monitoring device that accurately determines pH and K+ activities on the liver surface with ion-selective electrodes. Four groups of rats (n = 6 each) were subjected to complete, arterial or portal warm ischemia for 15 min (achieved by clamping of the hepatoduodenal ligament, the hepatic artery or the portal vein) and subsequent reperfusion. One group was sham-operated. Complete and portal ischemia were characterized by an immediate decline of pH and a more retarded rise of K+ activity on the liver surface. Arterial ischemia had almost no effect on these two parameters when compared with the sham group. Upon reperfusion the shifts of pH and K+ activity reversed towards initial baseline values. The organ monitoring system offers the option to assess ional shifts non-invasively during organ procurement, ischemia and reperfusion and may be used as an additional criterion for the estimation of organ viability.

Verapamil and flunarizine protect the isolated perfused rat liver against warm ischemia and reperfusion injury.

Using the model of the isolated perfused rat liver, we investigated the influence of the two pharmacologically different calcium channel blockers, verapamil and flunarizine, on changes of ion homeostasis, liver weights, pH deviations and enzyme activities during warm ischemia (37 degrees C) and reperfusion. The LDH and GLDH activities were determined and the calcium, potassium, and sodium concentrations were measured in the effluent. Warm ischemia (180 min) caused an increased enzyme release, a high influx of calcium and sodium into the liver and a massive potassium efflux current. Normoxic reperfusion led to a further increase in hepatic enzyme release and although the loss of potassium ceased, the calcium influx into the liver continued. By the end of reperfusion the liver weight had increased significantly (P < 0.01) in the control group. The two calcium entry blockers were added to the perfusate in various concentrations. Both substances protected the liver against warm ischemia and normoxic reperfusion damage, but they did not inhibit calcium inflow. However, the potassium efflux was significantly reduced by all concentration tasted (P < 0.001). After reperfusion the liver weights were significantly lower in the treated groups (P < 0.001) than in control animals. Thus, the calcium entry blockers verapamil and flunarizine protect liver cells against damage caused by warm ischemia and reperfusion. Furthermore, they prevent the disruption of intracellular potassium homeostasis, which seems to be related to improved volume regulation of liver cells.

[Reference values for progesterone, beta-HCG, estriol and human placental lactogen in the 10th-16th weeks of pregnancy].

Progesterone, beta-hCG, estriol and hPL in sera from 745 pregnant women have been measured in weeks 10 to 16 of pregnancy. The determined reference intervalls included the 10th and 90th percentile. All confidence intervalls have been calculated for a probability level of 10 per cent. The following levels were determined during the concerning weeks of pregnancy: progesterone 32-165 nmol/l, beta-hCG 31.5-260 micrograms/l, estriol 0.04-4.94 nmol l, and hPL 0.03-2.65 mg/l. The levels of progesterone are increasing weakly with the rising weeks of pregnancy, but the levels of beta-hCG are going down continuously. The levels estriol and hPL are showing a marked growth.

[No age dependence of biochemical parameters in pregnancy].

The parameters AFP, progesterone, HPL, estriol, SP1 and beta-HCG were evaluated in the sera of 994 pregnant women. The women were in the 5th to 25th weeks of pregnancy. Evaluation was carried out by means of radioimmunoassay and radial immunodiffusion. In collecting the data, we wished to determine whether there was a relationship between the age of the pregnant woman and the changes in parameter values during the course of the pregnancy. With the exception of beta-HCG concentrations, no age dependency was observed. However, the beta-HCG/age interrelationship was minor and without practical consequence.

[Latex immunoassay].

The latex immunoassay can be considered a quickly practicable and simple methodic variant of the immunoassay. In this work principles and present capability of this assay are stated. Own experiments deal with the adsorption reaction of alkaline phosphatase and horseradish peroxidase at polystyrol respectively melanin formaldehyde lattices. The adsorption reaction of several lattices is partly extremely different. Problems concerning the construction of an assay useful for practical purposes are discussed.

Isolation of human alpha-1 fetoprotein (AFP) from induced abortion material.

The isolation of alpha-1 fetoprotein (AFP) from material of an induced abortion is described. AFP is purified by anion exchange chromatography on DEAE-Sephadex A 50, affinity chromatography on Con-A-Sepharose, and preparative electrophoresis on agarose gel. The yield is about 10% of the original material. The purity was demonstrated by agarose gel electrophoresis and two-dimensional immunoelectrophoresis. The high concentration of AFP in material of induced abortion makes it particularly suitable for deriving human AFP.
