We report five children with transverse defects of the upper limb who developed calcified deposits at the distal end of the limb. The lesions were excised from two children and had the typical histological appearance of tumoral calcinosis but on clinical grounds it is unlikely that this is the same condition as that previously described as tumoral calcinosis.
In order to determine if fibrous flexor sheath ganglia had any histological features that could distinguish them from other ganglia, wer reviewed 50 consecutive ganglia treated by surgical excision. Of these, eight were fibrous flexor sheath ganglia. They did not have any specific features to distinguish them from the other ganglia. Furthermore, an absence of true synovial lining in them refutes the theory that they may result from synovial herniation around the A1 pulley.
The initial deposition pattern in the nasal passages and subsequent clearance of an insulin formulation labelled with 99mTc-human serum albumin have been determined in 12 healthy male volunteers. Four different administration modes from a novel aqueous spray device were compared, involving delivered volumes of 80-160 microL, and with either gentle or vigorous inhalation while firing the device. The entire dose was deposited in the nasal cavity, and no significant radioactivity was deposited in the lungs. A mean 25-33% of the radiolabel remained in the nose after 4 h. A significantly smaller area of the nasal mucosa was covered by the smallest (80 microL) bolus, but subsequent clearance rates did not vary significantly with mode of administration. Blood glucose levels fell after administration of the insulin formulation, but no serious episodes of hypoglycaemia occurred.
A comparison of aerosol delivery has been made between two open-vent Pari jet nebulizers. Intermittent and continuous delivery were compared for one of the nebulizers. Ten healthy volunteers inhaled 99mTc-labelled diethylenetriamine penta-acetic acid (DTPA) aerosols on three occasions. The Pari LC device was operated both intermittently (by a manual interrupter that generated aerosol only in synchrony with inhalation) and continuously. The Pari LL nebulizer was operated only in the intermittent mode. A system of inspiratory and expiratory valves was fitted to each nebulizer in order to direct airflow. Both nebulizers were powered by Pari Boy compressors. The mean (SD) whole lung deposition for the LL nebulizer was 11.1 (4.0)% of the nominal dose, compared to 15.3 (12.8)% and 12.8 (7.9)% for the LC used with intermittent and continuous operation, respectively. These differences were not statistically significant. Regional deposition patterns within the lungs were similar for the three nebulizer systems. These data show that efficient nebulizer systems using relatively low power compressors are possible, and suggest that continuously operated open-vent nebulizers may be designed to give lung deposition comparable to that achieved by nebulizers fitted with manual interrupters.

66 patients with congenital or acquired amputations of the hand have been studied after fitting partial or complete silicone hand prostheses which can be manufactured at relatively low cost. Most patients used their prostheses on social occasions only and all but four expressed satisfaction with them.

We report eight patients who sustained simultaneous dislocations of two joints in a digit. The little finger was involved in five patients.

We review 17 patients who had 21 amputations of the little finger. Transmetacarpal amputations gave satisfactory results in older patients, but in young patients loss of breadth of the palm caused significant disability. Disarticulation through the metacarpophalangeal joint is recommended in younger patients.
This is a case report of a hemangiopericytoma located within the median nerve at the wrist in a 79-year-old man. The tumor was removed without loss of function.
Furazolidone and nitrofurazone showed in vitro activity against amastigotes of Leishmania donovani, L. enriettii and L. major in macrophages, at concentrations which were also toxic to the macrophages. A low grade of activity was observed against L. donovani infections in BALB/c mice by furazolidone but not with nitrofurazone. Nitrofurazone, in two concentrations, was not active when applied to the lesions of cutaneous leishmaniasis due to L. enriettii (guinea-pig infection) or L. major strain P (BALB/c mouse infection). After systemic administration to BALB/c mice infected with L. major strain JISH 252 clone 1, low-grade activity was observed at the highest level tested.
Acute traumatic dislocation of the trapezio-metacarpal joint is not invariably associated with instability of the joint after reduction and the injury should not be considered simply as a "Bennett's fracture without a fracture".
