Maxillofacial fractures in the elderly: a comparative study.

Previous maxillofacial trauma research has dealt primarily with facial bone fractures in the general population. Very few studies have specifically addressed maxillofacial fractures in the elderly. We compared 45 elderly (65 years of age or older) and 201 younger adult (16 to 64 years of age) patients admitted to our hospital with maxillofacial fractures. The percentage of patients admitted with nasal bone fractures was much greater in the elderly population, while mandibular fractures were more common in the adult group. Motor vehicle accidents accounted for over half the injuries in both groups, while falls were more prevalent in the elderly. Management of the elderly patient may be complicated by their associated injuries or underlying medical problems, perhaps partially accounting for their longer median length of hospital stay. The elderly are a unique subpopulation of maxillofacial fracture patients and deserve further study regarding their injuries and optimal methods for treatment.

Herpetic whitlow of the digits.

Herpes simplex viral infection of the digits, also known as herpetic whitlow, is a rather common hand problem encountered in dental and medical personnel. Treatment is controversial. This report summarises the author's successful experience in a series of ten patients using a non-operative local care technique, supplemented when indicated by a systemic antiviral agent.

Plantar fibromatosis: an isolated disease.

Plantar fibromatosis is a relatively uncommon benign disease characterized by bilateral nodular thickening of the plantar fascia similar in appearance to the disease which occurs in the palm of the hand. Descriptions of plantar fibromatosis in the literature were usually included as a part of a syndrome described as ectopic diseases of Dupuytren. Isolated case reports have described Dupuytren-like contractures of the plantar fascia but often suggested a relationship with palmar lesions. We have recently treated four patients with isolated plantar fibromatosis without other stigmata of Dupuytren's disease. These patients were treated by removal of all the involved plantar fascia with a wide margin of normal-appearing fascia.

Spontaneous postpartum factor VIII inhibitor development with bleeding into the face and neck.

The coagulopathy caused by factor VIII inhibitor development can cause life-threatening bleeding that may be difficult to manage. Control of the bleeding, medically and surgically, and normalization of factor VIII and inhibitor levels are the goals of treatment; several modes of therapy are available. A case of spontaneous post-partum development of factor VIII inhibitors was presented and discussed.

Relationship between acid secretion and blood flow in the canine stomach: role of oxygen consumption.

The relationship of acid secretion to blood flow and oxygen consumption was investigated in the isolated canine stomach during intraarterial infusion of pentagastrin. Blood flows above and below control values were achieved by pump perfusion and partial arterial occlusion, respectively. When blood flow was increased from a control value of 38 ml X min-1 X 100 g-1 to 85 ml X min-1 X 100 g-1, both acid secretion and oxygen consumption remained constant at 0.08 mEq X min-1 X 100 g-1 and 2.1 ml X min-1 X 100 g-1, respectively. A reduction in blood flow to 9.0 ml X min-1 X 100 g-1 caused acid secretion and oxygen consumption to decrease to 26% and 44% of control values, respectively. The results indicate that acid secretion is independent of blood flow at flow rates above 38 ml X min-1 X 100 g-1. A model of the interrelationships between blood flow, oxygen consumption, and acid secretion is proposed that may explain much of the conflict in the literature concerning these three parameters.
