Plasma cell lesions of the thyroid: report of a case of solitary plasmacytoma and a review of the literature.

We report a case of a solitary plasmacytoma arising from a thyroid with longstanding Hashimoto's disease, and diagnosed by fine-needle aspiration cytology. Serum protein electrophoresis revealed an M-spike in the gamma-globulin region due to monoclonal IgG-lambda immunoglobulin. The thyroid tumor was treated with near-total thyroidectomy and irradiation, and the patient was well 6 years after surgery without evidence of multiple myeloma. The serum M-spike disappeared after the tumor resection and radiation therapy. Plasma cell lesions of the thyroid reported in the world literature are extensively reviewed. Solitary plasmacytomas occur most commonly in patients with Hashimoto's disease, and must be distinguished from plasma cell granulomas and involvement of the thyroid in multiple myeloma. Plasmacytomas should be considered in the differential diagnosis of a rapidly enlarging thyroid mass in a patient with known Hashimoto's disease.

Merkel cell carcinoma of the scrotum.

We report a rare case of Merkel cell carcinoma arising from the scrotal skin of an 84-year-old man. The tumor was treated by wide, local excision and radiation. The man died due to extensive metastatic disease 16 months after his surgery. The natural history, histology, and pathogenesis of Merkel cell carcinoma are discussed briefly.

Olfactory neuroblastoma: report of a case associated with inappropriate antidiuretic hormone secretion.

Esthesioneuroblastoma or olfactory neuroblastoma, is a rare tumour. It was first described by Berger et al. in 1924. Since then, approximately 250 cases have been reported. These neuroendocrine neoplasms are rarely associated with hormone excess syndromes. We report a case of olfactory neuroblastoma initially manifested with a syndrome of inappropriate secretion of antidiuretic hormone (SIADH) in a 27-year-old man. A literature review is briefly presented.

Cardiac tamponade as the initial presentation of papillary thyroid carcinoma.

Hypothyroidism is a well known cause of pericardial effusions and cardiac tamponade; thyroid carcinoma metastatic to the heart and pericardium is rarely encountered in clinical practice. The authors report the case of a 68-year-old male who presented in acute distress from cardiac tamponade. Cytological examination of the pericardial and pleural fluid, and histological examination of a pericardial biopsy revealed a metastatic papillary carcinoma with psammoma bodies. Subsequent reexamination of the patient demonstrated a thyroid nodule and bilateral cervical lymphadenopathy. Fine needle aspiration of the thyroid nodule and cervical lymph nodes detected a papillary thyroid carcinoma with metastases to the lymph nodes. The literature relevant to hypothyroidism, thyroid malignancy and cardiac tamponade is briefly reviewed.

Disseminated leiomyomatosis peritonealis: report of a case in a postmenopausal woman.

Disseminated leiomyomatosis peritonealis (DLP) is a rare condition that to date has been reported only in premenopausal women. The author reports a case of DPL occurring in a postmenopausal woman who had undergone total hysterectomy 30 years before and had received no hormonal therapy subsequently. The lesions were multiple and consisted of smooth-muscle cells that proliferated from medium-sized blood vessels. The author concludes that the patient's lesions likely represent a histologic variant of DLP.

Heterotopic adrenocortical oncocytoma. A case report with light and electron microscopic studies.

BACKGROUND: The case of a benign oncocytoma arising in a patient from retroperitoneal heterotopic adrenocortical tissue is reported; this tumor was found in a 44-year-old man. METHODS: The tumor consisted of polygonal cells with granular eosinophilic cytoplasm packed with swollen mitochondria. RESULTS: Scattered tumor cells showed abundant smooth endoplasmic reticulum suggesting that the tumor was of adrenocortical origin.

Aspiration biopsy of granulosa-cell tumor of the ovary: cytologic findings and differential diagnosis.

Fine-needle aspirates from four histologically confirmed adult granulosa-cell tumors of the ovary revealed irregular sheets, loose aggregates or tight clusters of small tumor cells with round or oval nuclei and scant cytoplasm. Nuclear grooves and indentations were noted in a small number of cells in two cases. Tumor cells arranged in follicular pattern mimicking Call-Exner bodies were not identified in all cases. A cytologic differential diagnosis of "small-cell" tumors of the ovary is briefly discussed.

Aspiration biopsy cytology of papillary carcinoma of the breast.

The fine-needle aspirates of two cases of noninfiltrating papillary carcinoma (PC) and three examples of early invasive PC of the breast were examined. In three cases in which the tumors displayed cuboidal or polygonal cells the aspirates showed papilla-like clusters of tumor cells with relatively "strong" cellular cohesiveness. Single and small aggregates of tumor cells as well as hemosiderin-laden or foamy macrophages were also present. Aspirates from the two PCs predominantly consisting of tall columnar epithelial cells revealed only monolayered and multilayered epithelial fragments with folding in one case. In the other case large epithelial fragments and small tight clusters of polygonal tumor cells were present. No bipolar nuclei of myoepithelial cells were identified in all cases. No specific cellular features permitting the differentiation between noninfiltrating and early invasive breast PCs were identified in this small series. Staining for carcinoembryonic antigen using the peroxidase-antiperoxidase technique was performed on aspiration smears of three cases. It revealed a positive cytoplasmic reaction in two cases.

The value of submucous needle aspiration in the prediction of surgical resection line of bronchogenic carcinoma.

The fiberoptic bronchoscope was used to perform a series of SMNAs from the tumor site in the bronchial tree toward the carina to decide on an appropriate surgical line of resection in 20 patients. The line of resection was examined histologically for tumor invasion. Of the 20 patients who underwent surgery 16 had lobectomies and four had pneumonectomies. Eighteen patients had no evidence of tumor invasion at the surgical resection site. Of the remaining two, one had a cancer-positive SMNA proximal to the resection line and demonstrated tumor cells in the surgical resection line. The second demonstrated tumor cells in the right lower and upper lobes with sparing of the right bronchus intermedius. This patient underwent lobectomy with subsequent resection of the remaining right upper lobe. On the basis of this experience we conclude that SMNA is effective in predicting the optimal surgical line of resection in lung carcinoma.

Dissecting aneurysm of the pulmonary trunk.

Dissecting aneurysms of the pulmonary trunk are a rare cause of sudden death. We report a case in a patient with pulmonary hypertension secondary to a patent ductus arteriosus. A brief review of the condition is also presented.

Clear-cell epithelial neoplasms of the large intestine.

We report four cases of primary clear-cell adenoma and adenocarcinoma of the large intestine. The neoplasms grossly resembled ordinary colonic adenomas and adenocarcinomas but microscopically were composed of uniform cells with optically clear cytoplasm. Mucin stains were negative, and the clear nature of the cytoplasm was due to glycogen accumulation. Areas of transition between normal colonic epithelial constituents and the clear-cell lesion were observed. Three of the four cases stained strongly positively for carcinoembryonic antigen. These lesions are apt to give rise to considerable diagnostic confusion and, in particular, resemble metastatic renal cell carcinoma. The usual strong positive carcinoembryonic antigen reaction is helpful in establishing this diagnosis.
